Mastocytosis is divided into two groups; systemic and cutaneous. Symptoms depend on excessive cytokine release and their local and systemic affects. Symptoms may be mild or cause life-threatening anaphylactic reactions. In the literature anaphylactic reactions reported in systemic disease of adult and childhood mastocytosis. We here present an anaphylaxis in the newborn with a diagnosis of cutaneous mastocytosis.Öğe Comparison of Clinical and Neuroimaging Characteristics of Congenital and Possible Congenital Cytomegalovirus Infected Infants: A Retrospective Study(Georg Thieme Verlag Kg, 2019) Tarakci, Nuriye; Bulut, Cahide; Caksen, Huseyin; Ors, RahmiBackground To evaluate clinical and neurodevelopmental outcomes and neuroimaging results of patients diagnosed with congenital cytomegalovirus (cCMV) infection and those diagnosed with possible congenital CMV (possible cCMV) infection. Methods From January 2014 to January 2016, 24 patients were diagnosed with congenital CMV infection. Clinical and neuroimaging results of these patients were examined from their medical records. Results Of the 24 patients, 6 were diagnosed with cCMV, whereas 18 patients were diagnosed with possible cCMV. Petechia was the most common symptom observed in patients with cCMV infection, whereas microcephaly was more common in patients with possible cCMV. Of the 24 patients, 5 (20.8%) showed normal neuroimaging characteristics. The most commonly encountered abnormal findings in brain magnetic resonance imaging were hypoplasia of corpus callosum (16.7%) and hydrocephalous (16.7%). All of the patients had poor neurodevelopmental outcomes. Conclusions Early diagnosis of asymptomatic cCMV infection is difficult. This study suggests that possible cCMV infections are more prevalent compared with cCMV infections, and delays in diagnosis occur due to not keeping suspicion of CMV infection in early phase. Clinicians should consider long-term neurodevelopmental sequelae of cCMV and possible cCMV infections and remember to screen congenital CMV infection in suspected individuals.Öğe Comparison of the Efficacy of Three Natural Surfactants in Preterm Turkish Newborns with Respiratory Distress Syndrome(Galenos Yayincilik, 2021) Yilmaz, Fatma Hilal; Tarakci, Nuriye; Gultekin, Nazli Dilay; Yucel, Mehmet; Kececi, Ramazan; Ozturk, Elif Nur Yildirim; Altunhan, HuseyinAim: To determine the efficacy of three natural surfactant preparations in our community and the short- and long-term results of these on preterm infants. Materials and Methods: This was a retrospective research on 193 premature babies with respiratory distress syndrome (RDS). The patients were divided into three groups, each of which received one of three surfactants: Group 1; beractant (100 mg/kg); group 2; poractant alfa (first dose of 200 mg/kg, recurrent doses of 100 mg/kg); group 3; calfactant (100 mg/kg). The groups were compared according to demographic characteristics, 1- and 5-minute Apgar scores, weight percentiles by gestational week, presence of pulmonary hemorrhage, surfactant dose repetition, air leak, bronchopulmonary dysplasia (BPD), stage of intraventricular hemorrhage (IVH), necrotizing enterocolitis (NEC), hemodynamically significantpatent ductus arteriosus (hs-PDA) and its medical or surgical treatment, retinopathy of premature (ROP) and its treatment, sepsis, ventilation time (both non-invasive and invasive), free oxygen need time, time to start full enteral feeding, discharge time, and mortality. Results: A total of 193 preterm infants with a mean gestational age of 28.9 +/- 3.1 weeks and mean birth weight of 1,190.4 +/- 504.3 grams were included in this study. The neonates were allocated into three different groups randomly, namely group-1 (n=77), group-2 (n=59), and group-3 (n=57). There were no differences in the clinical and demographic features of the groups. The incidence of pulmonary hemorrhage, surfactant dose repetition, air leak, ventilation time for both non-invasive and invasive, free oxygen need time, hs-PDA and surgical treatment of PDA, BPD, NEC (>= stage II), IVH (>stage III), ROP, time to start full enteral feeding, and discharge time were similar between the study groups. The sepsis and mortality rates were lower in group 3 compared to groups 1 and 2 (p=0.015, p=0.001). Conclusion: In this study, beractant, proctant alfa and calfactant had clinically similar efficacy in patients with RDS.Öğe A congenital cranial dysinnervation disorder: Mobius' syndrome(Aves, 2017) Albayrak, Hatice Mutlu; Tarakci, Nuriye; Altunhan, Huseyin; Ors, Rahmi; Caksen, HuseyinMbius' syndrome, also known as Mbius' sequence, is a nonprogressive cranial dysinnervation disorder characterized by congenital facial and abducens nerve paralysis. Here, we report a 5-day-old girl who was conceived after in vitro fertilization with poor suck and facial paralysis. She had bilaterally ptosis and lateral gaze limitation, left-sided deviation of the tongue, dysmorphic face, hypoplastic fingers and finger nails on the left hand, and was diagnosed as having Mbius' syndrome. Involvement of other cranial nerves such as three, four, five, nine, 9 and 12, and limb malformations may accompany this syndrome. However, several factors have been proposed for the etiology, some rare cases have also been reported with artificial reproductive technologies. Feeding difficulties and aspiration are the main problems encountered in infancy. The other cranial nerves should be examined further in newborns who present with congenital facial palsy, and other cranial dysinnervation disorders should be considered in the differential diagnosis.Öğe Effects of Prednisolone, l-Asparaginase, Gemfibrozil, and Combinations of These Elements on Mice Lipid Profile, Liver, and Pancreas(Lippincott Williams & Wilkins, 2016) Kose, Dogan; Tarakci, Nuriye; Celik, Zeliha Esin; Vatansev, Husamettin; Cimbek, Emine Ayca; Ugras, Serdar; Sen, YasarThe aim of this study is to determine the effects of l-asparaginase (l-ASP), corticosteroids (CSs), and antilipidemics, separately and in combination, on the lipid profiles and the liver and pancreas histology in mice. This study included 8 groups of 7 mice each. Before any drug administration, serum samples were taken from all of the mice. Then, normal saline was applied to the control group, and a medication or combination of medications was applied to the other groups. Levels of triglycerides, cholesterol (COL), and high-density lipoprotein (HDL) and low-density lipoprotein (LDL) were determined, and the livers and pancreases were evaluated histologically at the end of the study. Triglycerides increased significantly in the CS-only and the l-ASP-only groups, COL increased significantly in the CS-only group, and HDL increased significantly in the CS-only and the antilipidemic-only groups. LDL was significantly lower in the CS-only and the l-ASP-only groups. CSs and l-ASP were significantly effective in liver necrosis, l-ASP was significantly effective in liver balloon degeneration, and CS were significantly effective in pancreas vacuolization. Triglyceride measurement is recommended before/during CS and/or l-ASP treatment. Starting with an antilipidemic agent can be considered to avoid possible complications in patients with significantly high rates. In these patients, risk of sudden cardiac death due to ventricular tachyarrhythmias is high. Bradycardia may exhibit as a result of intrauterine fetal atrioventricular block, sinus bradycardia, tachycardia in these patient. Prolonged QT interval and multisystem involvement such as sensorineural hearing loss, muscle paralysis, immune deficiency, syndactyly have been reported in these patient. We have detected hyperpyrexia without clinical immunodeficiency and infection in our patient. To our knowledge, our patient is the first case in the literatureÖğe Late-onset Streptococcus pasteurianus sepsis in a preterm baby in a neonatal intensive care unit(Aves, 2014) Tarakci, Nuriye; Dagi, Hatice Turk; Ugur, Ayse Ruveyda; Tuncer, Inci; Tastekin, AyhanApnea, cyanosis, lethargy and prolongation in capillary filling time developed on the postnatal 37th day in a preterm baby who was born at the 30th gestational week with a birth weight of 1 300 g. Acute phase reactants and immature/total neutrophil count ratio were found to be high. The patient who was diagnosed with sepsis was successfully treated with meropenem which was started empirically. In his blood culture Streptococcus pasteurianus grew. S. pasteurianus is in the subgroup of streptococcus bovis which is one of the D group streptococci and its previous name is S. bovis type II/2. In the literature, there are very few cases of neonatal infection related with this bacterium. As far as we know, this is first case of neonatal sepsis caused by S. pasteurianus in Turkey. In addition, we tried to determine the clinical properties of neonatal infections arising from S. pasteurianus by reviewing the literature.Öğe Lneonatal meningitis due to streptoccocus dysgalactiae subspecies equisimilis: a case report and literature review(Derman Medical Publ, 2018) Tarakci, Nuriye; Yilmaz, Fatma Hilal; Gultekin, Nazli Dilay; Altunhan, Huseyin; Ors, RahmiStreptococcus dysgalactiae subsp. equisimilis (SDSE) was firstly reported as a toxon among human streptococcal pathogens in the year 1996. Diseases caused by SDSE may vary from milder skin involvements including wound infection, erysipelas, and cellulitis to life-threatening clinical pictures as streptococcal toxic shock syndrome and necrotizing fasciitis. We describe a case of SDSE sepsis and meningitis in a 2-day-old newborn. He was referred to our intensive care unit (ICU) with fever, respiratory distress, seizures, peripheral cyanosis, and somnolence. SDSE grew out of the CSF and blood cultures obtained on admission to the ICU. SDSE cases reported in the literature are frequently older patients with an underlying disease. To the best of our knowledge, this is the first case of mosaic trisomy 14 with an aortopulmonary window to be described in the literature.Öğe Neonatal Outcomes of Immigrant and Turkish Preterm Infants Treated in a Level-3 Neonatal Intensive Care Unit: A Retrospective Study(Galenos Yayincilik, 2021) Tarakci, Nuriye; Yilmaz, Fatma Hilal; Altunhan, HuseyinAim: To evaluate the neonatal outcomes of immigrant preterm babies and compare them with Turkish preterm babies in a Neonatal Intensive Care Unit (NICU). Materials and Methods: The present study was conducted using a retrospective design in the NICU of Necmettin Erbakan University between October 2015 and October 2019. Antenatal, natal and postnatal outcomes of the premature infants were evaluated and compared. Results: Maternal age (p<0.001), antenatal follow-up (p<0.001), antenatal steroid use (p<0.001), patent ductus arteriosus (PDA) (p=0.03) and bronchopulmonary dysplasia (BPD) (p=0.05) were statistically higher in the Turkish patients. Additionally, the rates of breastfeeding (p<0.01) and kangaroo mother care (p<0.01) were found to be higher among Turkish mothers. On the other hand, the necrotizing enterocolitis (NEC) rate (p=0.002) was found to be higher among immigrant babies. Duration of mechanicalventilation and oxygen supply were similar for both groups; however, the duration of nasal-continuous positive airway pressure use was longer in the Turkish patients (p=0.01). Conclusion: In this study, immigrant mothers were younger than their Turkish counterparts and their antenatal follow-ups were insufficient. The rate of hospitalization in a NICU and NEC rates were higher in immigrant preterm infants, respiratory distress syndrome, PDA and BPD were more commonly encountered in Turkish preterm infants.Öğe Our Tigecycline Treatment Experiences in Three Newborns who had Klebsiella Pneumonia Sepsis(Soc Argentina Pediatria, 2018) Yilmaz, Fatma H.; Gultekin, Nazli D.; Tarakci, Nuriye; Altunhan, Huseyin[Abstract Not Availabe]Öğe Procalcitonin Level at 24 Hours of Age May be Predictive for Transient Tachypnea of the Newborn(Georg Thieme Verlag Kg, 2020) Tarakci, Nuriye; Altunhan, Huseyin; Sari, Eyup; Uyar, MehmetObjective It is an important problem to differentiate transient tachypnea of the newborn (TTN) from bacterial pneumonia or other conditions in patients admitted to Neonatal Intensive Care Unit. The aimof this study was to evaluate the predictive value of procalcitonin (PCT) for TTN. Methods A total of 52 infants were contained in the study. The patients were divided into three groups. Group 1 consisted of patients with pronounced grunting at more than 2 hours postnatal age (n = 16). Group 2 consisted of patients whose grunt was reduced or gone at 2 hours postnatal age but in whom tachypnea persisted until 24 hours postnatal age (n = 18). Group 3 consisted of patients with minimal or no respiratory distress at 24 hours postnatal age (n = 18). In all groups, PCT concentrations were determined at birth and 24 hours postnatal age. Results PCT concentrations at birth were significantly higher in Group 1 than other groups, but there was no difference between Groups 2 and 3. PCT concentrations at 24 hours postnatal age were significantly higher in Groups 1 and 2 than Group 3. No difference was found between Group 1 and Group 2 at 24 hours postnatal age. All PCT concentrations in Group 3 were significantly lower than other groups. PCT thresholds for the diagnosis of TTN were 0.44 ng/mL at birth (sensitivity 58%, specificity 50%) and 5.11 ng/mL at 24 hours postnatal age (sensitivity 79.4%, specificity 89.1%). Conclusion Serial PCT measurements at birth and postnatal 24 hours may be helpful in differentiating between pneumonia and TTN. Premature newborns were randomized into two groups based on the delivery method of the lipid solution, separately (Group 1) or in mixture with TPN solutions (Group 2). Total antioxidant status (TAS), total oxidant status (TOS) and paraoxonase (PON-1) levels were analyzed in both blood samples, and oxidative stress index (OSI) was also calculated.Results: Thirty cases from both groups were included in the study. Statistically significant decrease in the level of TAS and increase in the level of PON-1 were detected at 72h of TPN therapy in both groups (p<0.05). Statistically significant decrease in both TOS and OSI levels were observed in Group 2 (p<0.05). In association with these findings, any statistically significant intergroup difference was not detected in both parameters regarding oxidant balance (p>0.05).Conclusion: Our study showed that according to lipid administration method any difference for oxidant-antioxidant balance was not detected.
