Öğe Alstrom syndrome with liver cirrhosis: First case from Turkey(Turkish Soc Gastroenterology, 2013) Biyik, Murat; Ucar, Ramazan; Gungor, Gokhan; Ozer Cakir, Ozlem; Esen, Hasan; Aksan, Serkan; Ataseven, HuseyinAlstrom syndrome is a rare autosomal recessive genetic disorder characterized by cone-rod dystrophy, hearing loss, childhood truncal obesity, insulin resistance and hyperinsulinemia, type 2 diabetes, hypertriglyceridemia, short stature in adulthood, cardiomyopathy, and progressive pulmonary, hepatic, and renal dysfunction. Alstrom syndrome is a very rare cause of liver cirrhosis. Post-mortem biopsies of patients with Alstrom syndrome show relevant fibrosis in multiple organs especially in the liver, kidneys, heart, and lungs. We report the case of a patient with Alstrom syndrome who presented to emergency department with esophageal variceal bleeding and who was not known to have hepatic cirrhosis before.Öğe Beneficial Effects of Moxonidine on Cerebral Vasospasm After Experimental Subarachnoid Hemorrhage(2014) Ilık, Mustafa Kemal; Kocaoğullar, Yalçın; Koç, Osman; Esen, HasanAMAÇ: Çalışma I1-imidazolin bölgeleri için selektif bir ligand olan merkezi etkili antihipertansif ajan moksonidinin bir tavşan serebral vazospazm modelinde etkinliğini incelemek üzere tasarlanmıştır. YÖNTEM ve GEREÇlER: Yirmi dört beyaz, 2500-3200 gr. ağırlığında erkek, Yeni Zelanda tavşanı rasgele olarak üç gruba bölünmüştür grup 1 kontrol grubu, grup 2sadece subaraknoid kanama (SAK) grubu ve grup 3SAK moksonidin (tedavi) grubu. Tüm tavşanlarda SAK indüksiyonu öncesinde (Gün0, bazal anj iyografi) ve 72 saat sonrasında serebral anj iyografi yapılmıştır. SAK indüksiyonundan sonra tedavi grubunda intraperitoneal moksonidin (0,5 mg/kg) tedavisi başlatılmış ve günde bir kez olmak üzere 72 saat devam ettirilmiştir. BULGULAR: Gruplar arasında baziller arterin anj iyografik luminal çapı açısından istatistiksel olarak önemli bir fark bulunmamıştır (p0,005). SAK sonrasında takip anj iyografik baziller arter luminal çapı sadece SAK grubu ile karşılaştırıldığında tedavi grubunda istatistiksel olarak anlamlı şekilde değişiklik göstermiştir (p0,001). Patoloj ik olarak incelenen baziller arter lümeni alanı bu iki grup arasında istatistiksel açıdan anlamlı fark göstermiştir (p0,005). SONUÇ: Merkezi etkili bir antihipertansif ajan olarak moksonidin tedavisi anj iyografik çapı ve patoloj ide lümen alanını artırıp müsküler duvar kalınlığını azaltarak vazospazm tedavisinde çok faydalı bulunmuştur.Öğe The biomechanical and histological effects of platelet-rich plasma on fracture healing(Springer, 2015) Guzel, Yunus; Karalezli, Nazim; Bilge, Onur; Kacira, Burkay K.; Esen, Hasan; Karadag, Hakan; Toker, SerdarPlatelet-rich plasma (PRP) includes growth factors and proteins that accelerate and stimulate bone regeneration and tissue recovery. The aim of this study was to evaluate the effects of PRP on fracture healing in terms of biomechanics and histology. Seventy female rats were included in this experimental study. They were divided into three groups: Group I (no PRP, n = 30), Group II (PRP added, n = 30) and Group III (control, n = 10). The left femurs of the rats in Groups I and II were osteotomized and fixed by K-wires. Although no additional intervention was performed on Group I rats, PRP was applied to the fracture sites of Group II rats. The remaining ten rats were used as the control group of the biomechanical test (Group III). In the fourth week, nine femurs from Group I and ten femurs from Group II, and in the ninth week, nine femurs from each group were removed, and bone recovery was assessed histologically according to Modified Lane-Sandhu histological scoring criteria. Three-point bending test was applied to femurs for biomechanical evaluation in the ninth week. Histological healing was found to be significantly higher in Group II than in Group I (p < 0.05). Furthermore, biomechanical test results showed that healing quantity and bone strength were significantly better in Group II than in Group I (p < 0.05). PRP is a widely studied material in the physiology of fracture healing. The results of this study demonstrated the ameliorative biomechanical effects of PRP on fracture healing, in addition to accelerating the histological union of fractures. In the light of these results, PRP could be a viable alternative to accelerate the healing of fractures, late unions or non-unions. We describe the first case of pulmonary hypertension secondary to pulmonary amyloidosis in a patient with CVID.Öğe Effect of injectable medroxyprogesterone acetate and etonogestrel implants on GABA-A and serotonin receptors in white and gray matter of the brain: experimental study in rats(Informa Healthcare, 2014) Seven, Ali; Yuksel, Beril; Kilic, Sevtap; Esen, Hasan; Keskin, Ugur; Ulubay, Mustafa; Ozekinci, MuratThe aim of this study was to evaluate the time-dependent effect of progesterone-only contraceptives on the brain and to obtain an improved understanding of mood disorders experienced under this medication. A total of 66 Wistar albino rats were divided into three groups: etonogestrel (ENG) implant (group 1, n = 30); depot medroxyprogesterone acetate (MPA)-injectable (group 2, n = 30); and control (group 3, n = 6) groups. Groups 1 and 2 were each divided into five subgroups, which were examined every 10 d for up to 50 d after medication administration, to evaluate its time-dependent effect. There was no difference in terms of gamma-aminobutyric acid (GABA) and serotonin immunohistochemical staining in white and gray matter among the subgroups of group 1. In group 2, there was a significant decrease in serotonin receptor staining intensity in white and gray matter on day 50, when compared to the control group (p = 0.041). When the subgroups of group 2 were compared, there was a significant decrease in serotonin receptor staining intensity in white and gray matter on days 40 and 50 when compared to day 10. In conclusion, we showed that ENG and MPA have no effect on apoptosis and GABA-A receptors in the brain. We also showed that MPA has time-dependent effects on serotonin receptors, which may be a possible mechanism involved in mood disorders during long-term usage of injectable progesterone-only contraceptives.Öğe Effects of diclofenac sodium and octreotide on treatment of caerulein-induced acute pancreatitis in mice(E-Century Publishing Corp, 2015) Cakir, Ozlem Ozer; Esen, Hasan; Toker, Aysun; Ataseven, Huseyin; Demir, Ali; Polat, HakkiBackground: Research continues to develop novel therapeutic modalities that particularly focus on the pathogenesis of acute pancreatitis. This study aimed to assess the effects of diclofenac sodium and octreotide, alone or in combination, on pancreatic enzymes, pancreatic myeloperoxidase activity, histopathology and apoptosis of pancreas cells, using a model of experimentally induced acute pancreatitis. Objectives: We aimed to demonstrate effects of diclofenac sodium, octreotide and their combined use on pancreatic enzymes, activity of pancreatic myeloperoxidase (MPO) activity, histopathology and apoptosis of pancreas on treatment of caerulin-induced experimental acute pancreatitis. Materials and methods: Caerulin-induced acute pancreatitis model was created using a total of 58 male BALB-C mice of 25 gr in seven groups. Serum amylase, lipase levels and pancreatic myeloperoxidase activity were examined as well as apoptotic values in pancreatic acinar cells through TUNNEL method. Histopathology of pancreas was evaluated for presence of edema, hemorrhage, parenchymal necrosis, fat necrosis, leukocyte infiltration, and fibrosis. Results: In the diclofenac sodium group, apoptotic values in the pancreatic acinar cells were found to be statistically lower than in the acute pancreatitis group in terms of parenchymal necrosis and hemorrhage scores (P = 0.007, P = 0.002, and P = 0.052, respectively). No statistically significant differences were found in serum level of amylase, lipase, pancreatic myeloperoxidase activity and the other histopathological scores (P > 0.05). Conclusion: Diclofenac sodium, a cost-effective agent with a favorable side-effect profile, may represent a novel therapeutic agent for the treatment of acute pancreatitis. Findings of this study suggest a better efficacy for diclofenac sodium monotherapy as compared to octreotide alone or octreotide/diclofenac combination.Öğe Evaluation of the effects of ankaferd blood stopper on rabbits with paranchyme damage: an experimental study(Baycinar Medical Publ-Baycinar Tibbi Yayincilik, 2013) Metin, Bayram; Altinok, Tamer; Menevse, Esma; Esen, HasanBackground: In this study, we aimed to evaluate the effects of Ankaferd Blood Stopper (ABS), a herbal extract used as a hemostatic agent in traditional Turkish medicine, on hemostasis, wound healing, air leakage and the efficiency over pleural adhesions in rabbits with pulmonary parenchymal damage. Methods: Sixteen New Zelland Albino type male and female rabbits with a mean weight of 2500 g were used. The rabbits were divided into two groups as study group and control group. One rabbit in each group died during anesthesia. We performed wedge resections in equal size to the left lower lobes of all rabbits. No additional procedure was performed on the control group, while the study group received 5 puffs (similar to 1 cc) of ABS to the resection area. Tube thoracostomy, which was performed in both groups, was terminated postoperatively at the sixth hours after drainage and air leakage follow-up. The rabbits were sacrificed with high dose anesthetic at the postoperative eight-day. Pleural inflammation and fibrosis in the hemithorax were examined macroscopically and microscopically. Biochemical measurements of tissue malondialdehyde (MDA) and hydroxyprolin (HPR) levels were done. Results: There was a statistically significant difference in postoperative air leakage and the amount of drainage between the groups (amount of hemorrhage p<0.05, air leakage p<0.001). A higher rate of macroscopic and microscopic fibrosis was observed in the study group. When both groups are compared according to tissue MDA levels, decreased MDA level was seen in the group which ABS was administered, where the difference was statistically non-significant. The function of Trx itself is regulated by thioredoxin reductase (TrxR). This study was designed to determine the expression of TrxR1 in meningioma tissues of different World Health Organization grades (grade I-III). Meningioma tissues were extracted from the histopathological specimens of 29 patients. These samples included seven histologically normal meningeal tissues that served as a control group and 12 grade I, 12 grade II and 5 grade III meningioma samples. TrxR1 expression was evaluated using quantitative reverse transcription polymerase chain reaction (qRT-PCR) and immunostaining. The proliferative and apoptotic indices of the specimens were investigated by Ki-67 immunostaining and TUNEL assay, respectively. TrxR1 expression, as assessed by qRT-PCR, increased significantly with meningioma grade (p < 0.001). The immunostaining intensity of TrxR1 increased significantly with meningioma grade (p < 0.001). Ki-67 index values increased significantly in accordance with grade progression (p < 0.001). The apoptotic index values were not significantly different in any group (p > 0.05). Trx system seems to be involved in the malignant progression of meningiomas. Further, large studies are required to elucidate the exact role of this system.Öğe Hipotermiye bağlı ölümlerin değerlendirilmesi(2013) Doğan, Kamil Hakan; Esen, Hasan; Şerafettin , Demirci; Gündoğmuş, Ümit NaciSistemik hipotermi terimi insan vücudunun 35° C’den daha aşağı düşecek kadar soğuduğunu belirtmek için kullanılır. Bu durum, vücudun ısı kaybının ısı yapımını aştığı zaman meydana gelir. Kaza sonucu meydana gelen hipoterminin en sık nedeni ısı derecesi düşük olan bir ortama maruz kalmadır. Hipotermi tehlikeli fizyolojik değişikliklere ve hatta ölüme neden olabilir. Bu çalış mada hipotermiye bağlı ölüm olgularının değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: 2000-2008 yılları arasında Konya Adli Tıp Şube Müdürlüğü tarafından ve Konya’ya bağlı ilçelerde adli tıp uzmanı tarafından otopsileri yapılan olgular retrospektif olarak taranmış ve ölümü hipotermiye bağlı olan olgular çalışma kapsamına alınmıştır. Bulgular: Çalışmaya dahil edilen dönemde 12 hipo termiye bağlı ölüm olgusu saptanmıştır. Olguların 11’i erkek, 1’i kadındır. Olguların yaşları 41 ile 85 arasında değişmekte olup, orta lama yaş 57.0±15.4 bulunmuştur. Ölümler en sık 5 olgu ile Ocak ayında meydana gelmiştir. Olguların 4’ü tarlada, 3’ü metruk binada bulunmuştur. 6 olguda psikiyatrik hastalık öyküsü mevcuttur. Histopatolojik incelemede 5 olguda enzimatik yağ nekrozu bulguları tespit edilmiştir. Sonuç: Hipotermiye bağlı ölümler, toplum sağlığı açısından önemli olduğu gibi, adli tıbbi açıdan da önemlidir. Bu tür olgularda, olay yeri incelemesi, olgunun tıbbi özgeçmişinin ayrıntılı bir şekilde incelenmesi, dikkatli bir şekilde yapılacak otopsi ve histopatolojik inceleme tanı konulmasını kolaylaştırır.Öğe Histopathological Evaluation of Platelet-Rich Plasma Effect in Acute Tympanic Membrane Perforation(Springer India, 2022) Cobden, Serap Bulut; Ozturk, Kayhan; Duman, Selcuk; Esen, Hasan; Aktan, Tahsin Murad; Avunduk, Mustafa Cihat; Elsurer, CagdasPlatelet-rich plasma (PRP) is a reliable and has low side-effect profile and has beneficial effects on wound healing. Its investigatory effects on wound-healing process were shown on various tissues. This study aims to investigate PRP's local application effects to perforated rat TM in terms of healing and histopatological outcomes. Twenty-two Wistar rats were used in the study. The rats' ears were examined with a pediatric endoscope (2.7 mm, 0 degrees), and the TM posterior quadrant of their right ear was perforated with a 20-gauge needle. After this procedure, the rats were divided into two equal groups. A spongel with PRP was applied on the perforated TM in the first group, and spongel with standard saline solution was applied on the second group. Following the sacrifice, the middle air bullas were carefully dissected and removed for histopathological examination. Hematoxylin eosin (for fibroblasts, lymphocyte, collagen fibers) and immunohistochemical staining were done for epithelial growth factor receptor (EGFR), fibroblast growth factor receptor (FGFR1), and vascular endothelial growth factor (VEGF) staining for histopathologic examinations. There was not a significant difference between the two groups for lymphocyte. There was a significant difference between control and study groups for collagen and EGFR (P < 0.05). Although the mean value of FGF- and VEGF-positive cells was higher in the study group than in the control group, the difference was not significant (P > 0.05). PRP is an effective autologous material for the healing process of acute TM perforations in a rat model, as demonstrated in the present study. We think that the use of PRP for acute TM perforations can have a positive effect on the healing process by increasing the level of growth factors, especially EGFR. In addition, an increase in collagen can also have a positive effect on healing.Öğe How do endoscopic bladder tumor resection techniques affect pathology practice? EAU Section of Uro-Technology (ESUT) and Uropathology (ESUP) survey(Springer, 2023) Guven, Selcuk; Colecchia, Maurizio; Oltulu, Pembe; Bonfante, Giulia; Enikeev, Dmitry; Esen, Hasan; Herrmann, ThomasPurpose We aimed to examine how different endoscopic bladder tumor resection techniques affect pathologists' clinical practice patterns. Methods An online survey including 28 questions clustered in four main sections was prepared by the ESUT ERBT Working Group and released to the pathologists working in the institutions of experts of the ESUT Board and the working groups and experts in the uropathology working group. A descriptive analysis was performed using the collected data. Results Sixty-eight pathologists from 23 countries responded to the survey. 37.3% of the participants stated that they always report the T1 sub-staging. Of those who gave sub-staging, 61.3% used T1a, b. 85.2% think that en bloc samples provide spatial orientation faster than piecemeal samples, and 60% think en bloc samples are timesaving during an inspection. 55.7% stated that whether the tissue sample is en bloc or piecemeal is essential. 57.4% think en bloc sample reduces turnaround time and is cost-effective for 44.1%. A large number of pathologists find that the pathology examination of piecemeal samples has a longer learning curve. Conclusion The survey shows that pathologists think that they can diagnose faster, accurately, and cost-effectively with ERBT samples, but they do not often encounter them in practice. Moreover, en bloc samples may be a better choice in pathology resident training. Surgically resected human GBM samples from 20 patients who underwent surgery at our institute were extracted from the histopathological specimens and divided into two groups. A total of 10 samples from each type of GBM (World Health Organization grade IV, intratumoral hemorrhage-positive or -negative) were included in each group. VEGF, bFGF and TrxR1 expression was analyzed using immunohistochemistry and the results were compared between groups. VEGF and bFGF immunoreactivity was significantly higher in GBMs containing intratumoral hemorrhage. Furthermore, VEGF, bFGF and TrxR1 immunointensity was significantly higher in GBMs containing intratumoral hemorrhage. Thus, the present study demonstrated a higher VEGF, bFGF and TrxR1 expression in GBMs contain intratumoral hemorrhage, indicatiogn a role of VEGF, bFGF and TrxR1 expression in the promotion of tumoral angiogenesis and tumoral growth by complex mechanisms that require further elucidation.Öğe Low-Grade Fibromyxoid Sarcoma of Superior Turbinate in a Pediatric Patient(Lippincott Williams & Wilkins, 2015) Varsak, Yasin Kursad; Arbag, Hamdi; Yesildemir, Huri Sultan; Esen, Hasan[Abstract Not Availabe]Öğe Myxoma Arising from the Middle Ear: A Case Report(Aves, 2020) Ulku, Cagatay Han; Aydogdu, Demet; Erdem, Rukiye; Esen, HasanThe primary head and neck myxomas are rare, generally arising from the mandible, maxilla, and oral cavity. Other anatomical areas, such as cardiac myxomas, may also have metastases to the head and neck regions. The middle ear is an extremely rare location for myxomas. Myxoma slowly grows and is usually asymptomatic until it affects the surrounding structures. Surgical treatment is performed with a complete en bloc resection where possible. We report a case of a 42-year-old woman with myxoma arising from the right middle ear because of her tumor's rare anatomical region. Her main complaints were progressive fullness and loss of hearing which she felt for approximately 1 year on the right ear. High-resolution computed tomography (HRCT) revealed an isodense soft tissue mass localized in the right mastoid bone and the middle ear. The mass was totally removed by canal wall up tympanomastoidectomy. At the last follow-up examination on 36 months after the surgery, the patient was asymptomatic, and there were no signs of recurrence.Öğe New Clues in the Malignant Progression of Glioblastoma: Can the Thioredoxin System Play a Role?(2018) Erdi, Fatih; Kaya, Bülent; Esen, Hasan; Karataş, Yaşar; Fındık, Sıddıka; Keskin, Fatih; Feyzioğlu, Bahadır; Kalkan, ErdalAIm: To evaluate and compare the expression of thioredoxin reductase 1 (TrxR1) in primary and secondary glioblastoma samples. mATERIAl and mEThODS: Surgically resected human glioblastoma samples from 40 patients who underwent surgery at our institution were extracted from their histopathological specimens and divided into three groups. Ten histopathologically regular cerebral tissue samples, acquired from the non-neoplastic portion of the specimens, were assigned as the control group. Twenty specimens that included tumoral tissue from each type of glioblastoma (WHO grade IV, primary and secondary) were assigned as the primary and secondary glioblastoma groups. TrxR1 expression was analyzed by using both quantitative reverse transcription polymerase chain reaction (qRT-PCR) and immunohistochemistry. Isocitrate dehydrogenase 1 (IDH1) mutation was analyzed by immunohistochemistry. Ki-67 proliferative index and apoptosis were also analyzed by immunohistochemistry. The differences between the groups were statistically compared and the correlation between these parameters was analyzed. RESulTS: The expressions of TrxR1 and Ki-67 values were significantly higher in primary glioblastoma. IDH1 mutation was significantly higher in secondary glioblastoma. TrxR1 expression was found to be highly correlated with the Ki-67 index. The apoptotic index was similar between primary and secondary glioblastoma. CONCluSION: This study showed a high TrxR1 expression in primary glioblastoma which could indicate a role of the Trx system in promoting the malignant progression by some complex processes.Öğe Non-Caseating Granulamatus Nephritis: Rare Sign of Sarcoidosis(Turk Nefroloji Diyaliz Transplantasyon Dergisi, 2016) Gencer, Vedat; Guney, Ibrahim; Yazici, Raziye; Esen, Hasan; Aslan, Uysaler; Bolukbas, OzdenSarcoidosis is chronic multisystem granulomatous disease. It can rarely cause renal structural and functional damage. Renal impairment is mostly due to the calcium metabolism and associated with hypercalciuria, nephrolithiasis and nephrocalcinosis. Non-caseating granulomatous nephritis can also be seen in few patients. We diagnosed non-caseating granulomatous nephritis with renal biopsy in a 50-year-old female pulmonary sarcoidosis case. Steroid treatment was immediately started and there was a significant decrease in proteinuria and creatinine after a month. Proteinuria and serum creatinine increase are important in patients with sarcoidosis and treatment must be started immediately to prevent end-stage renal failure.
