Our purpose was to present a case where vascular reconstruction was performed during multiple organ resections in a patient with locally advanced gastric cancer. An emergent laparotomy was performed on a patient who presented with upper gastrointestinal system hemorrhage; and he was diagnosed with locally advanced gastric cancer. Total gastrectomy, D II dissection, pancreaticoduodenectomy, transverse colectomy and end-to-end anastomosis were performed. Additionally, the superior mesenteric vein was partially excised and re-implanted to the portal vein. Neither recurrence nor distant metastasis was observed during the post-operative follow-up. Adjacent organ resections may be necessary for a curative treatment in locally advanced gastric cancer. In addition, partial resection and reconstruction may be required for extensive vascular invasion.Öğe Gastric necrosis and perforation caused by acute gastric dilatation(Aves, 2014) Gundes, Ebubekir; Kucukkartallar, Tevfik; Tekin, Ahmet; Cakir, MuratAcute gastric dilatation was first defined by Duplay in 1833. We herein present the case of a 55-year-old male patient diagnosed with gastric necrosis and perforation caused by acute gastric dilatation. Since the stomach has a rich blood circulation, necrosis and perforation are rarely seen. Clinically, more than 90% of cases have complaints of vomiting. The most useful method in revealing the diagnosis and aetiology is computerized tomography. Medical treatment is appropriate for cases where no necrosis and peritonitis findings are detected through endoscopy. The histopathological examination of the neuroendocrine carcinoma after laparoscopic cholecystectomy in a patient wanted to present the report. This is about the treatment of cancers, certain drugs used for chemotherapy after surgery. All the histological types of female breast cancer can be seen in male patients. Invasive papillary carcinoma is a rare breast cancer type. We hereby report the case of a 67-year-old male patient who underwent modified radical mastectomy because of invasive papillary carcinoma. The patient presented to our clinic with the complaint of a painless mass in his right breast. Tru-cut biopsy guided by imaging was performed. The pathological results of the biopsy were in line with invasive papillary carcinoma. We performed modified radical mastectomy on the patient. No recurrences were seen in the 24-month follow-up period.Öğe Magnetic Resonance Imaging Features of Idiopathic Granulomatous Mastitis: A Retrospective Analysis(Kowsar Publ, 2016) Poyraz, Necdet; Emlik, Ganime Dilek; Batur, Abdussamet; Gundes, Ebubekir; Keskin, SuatBackground: Granulomatous mastitis is a rare and benign inflammatory breast disease that may clinically and radiologically mimic breast cancer. Objectives: The aim of this study was to evaluate the features of idiopathic granulomatous mastitis (IGM) on breast magnetic resonance imaging (MRI) with mammographic and sonographic findings. Patients and Methods: A retrospective analysis was conducted on 20 patients with IGM who had been diagnosed by needle core or excisional biopsy at a single institution between 2006 and 2012. All of the patients underwent MRI for suspicious breast findings prior to biopsy. MRI examinations were performed on a 1.5 T scanner. The MRI findings were evaluated in accordance with the breast imaging-reporting and data system (BI-RADS) MRI lexicon established by the American College of Radiology. Results: MRI detected a total of 29 lesions in the 20 patients. Fourteen of these lesions were seen as mass enhancements, with the remaining 15 identified as non-mass enhancements (NMEs). The median size of all lesions was 3.6 cm (range, 0.7-6.7 cm). The most frequently observed features were masses with a round shape (9 out of 14, 64%), smooth contour (11 out of 14, 78%), and a rim enhancement pattern (10 out of 14, 71%). The most common features of the 15 NME lesions were segmental distribution (6 out of 15, 40%) and heterogeneous enhancement patterns (8 out of 15, 53%). The time-intensity curves of the dynamic studies showed benign type one signal intensity (persistent enhancement pattern) in the majority of lesions (10 out of 20, 50%). Conclusion: Our study suggests that breast MRI findings of IGM have a wide spectrum. Rim enhancement patterns are frequently seen on contrast enhanced images, but the imaging findings are nonspecific and cannot be used definitively to distinguish between benign and malignant lesions.Öğe Our Primary Colorectal Lymphoma Experiences(Galenos Yayincilik, 2014) Gundes, Ebubekir; Cakir, Murat; Kucukkartallar, Tevfik; Tasci, Halil Ibrahim; Tekin, Ahmet; Tekin, SakirAim: In this paper, we aimed to present our surgical approach to ten patients who underwent surgery for primary colonic lymphoma and the features and clinical presentations of the tumors. Methods: We retrospectively analyzed the data of ten patients who underwent surgery in our clinic for primary colorectal lymphoma between January 2006 and December 2012. Results: The median age of the patients at the time of diagnosis was 66 (50-84) years. The most frequent presenting complaints were abdominal pain and change in bowel habits. The most frequent physical examination findings were abdominal distention and widespread tenderness. Three patients underwent emergency surgery subsequent to acute abdomen diagnosis. During the surgery, it was observed that one patient had ileocecal invagination, one patient had a mass that caused cecal perforation, and one other had an obstructive mass in the sigmoid colon. The most common localizations were the cecum (60%), sigmoid colon (30%), and the rectum (10%), respectively. The most frequent type was B- cell lymphoma (n=8). Conclusion: Primary colonic lymphoma is a rare condition. It is still yet uncertain whether surgery or chemotherapy should be advised first.Öğe Relationship of preoperative neutrophil lymphocyte ratio with prognosis in gastrointestinal stromal tumors(Turkish Surgical Assoc, 2015) Kargin, Suleyman; Cakir, Murat; Gundes, Ebubekir; Yavuz, Yusuf; Esen, Haci Hasan; Iyisoy, M. Sinan; Kokbudak, NaileObjective: Gastrointestinal stromal tumors are believed to play a role in immunosuppression with their local effect. In some cancers with associated inflammation, an increased pretreatment neutrophil lymphocyte ratio is considered as an indicator of poor prognosis. The aim of our study was to assess the relationship of increased blood neutrophil lymphocyte ratio with disease prognosis in patients with gastrointestinal stromal tumors. Material and Methods: The data of 78 patients who underwent surgery with the diagnosis of primary gastrointestinal stromal tumor in our clinic were evaluated retrospectively. The preoperative neutrophil lymphocyte ratio in the peripheral blood was determined. The neutrophil lymphocyte ratio and its relationship with tumor risk and prognosis were compared. The data were evaluated by Pearson's correlation analysis and the Welch ANOVA test. Results: The preoperative neutrophil lymphocyte ratio was significantly increased in the high-risk groups (p<0.05). An increased preoperative neutrophil lymphocyte ratio was associated with shorter survival (r=-0.32, p=0.009). In addition, an increase in the neutrophil lymphocyte ratio was associated with an increase in the mitotic activity of the tumor (r=-0.364, p=0.025). Conclusion: The preoperative neutrophil lymphocyte ratio in gastrointestinal stromal tumors can be used as an indicator of high-risk tumors and poor prognosis.Öğe Retroperitoneal extra-adrenal paraganglioma: case report(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2016) Gundes, Ebubekir; Aksoy, Faruk; Tasci, Halil Ibrahim; Yilmaz, OsmanParaganglioma (PG), also known as extra-adrenal pheochromocytoma, is a rare neuroectodermal tumor. The incidence of extra-adrenal paraganglioma is between 00.1-0.1%. Paragangliomas originating from the retroperitoneum are generally functional. 40-50% of them are malignant and they generally have a slow progression. Surgical resection and radiotherapy are the preferred methods for the local treatment of these tumors. A case with a tumor localized in the retroperitoneal space that was totally resected by surgical method was evaluated.Öğe SURGICAL TREATMENT OF PHYLLODES TUMORS OF THE BREAST: A SINGLE CENTER EXPERIENCE(Aves, 2013) Aksoy, Faruk; Gundes, Ebubekir; Vatansev, Celalettin; Emlik, Ganime Dilek; Dogan, SerhatObjective: Phyllodes tumor of the breast is a rare fibroepithelial tumor. In this study, our aim is to present the clinicopathological characteristics of our patients diagnosed with phyllodes tumor of the breast and our treatment approach to the cases. Materials and Methods: The medical records of a total of 9 patients who had been treated at our clinic between 2003 and 2011 for phyllodes tumor were analyzed retrospectively. Results: The median age of the patients was 38 (20-70) years. The presenting complaint of all patients was a palpable mass in the breast. Six of the patients were diagnosed by tru-cut biopsy, two were diagnosed through excisional biopsy, and one was diagnosed through fine-needle aspiration biopsy (FNAB). Median tumor size was 9.3 (4-20) cm. Local excision was performed on all but two patients so as to have the surgical border negative and no adjuvant additional treatment was initiated. Mastectomy was performed for one of the two patients and modified radical mastectomy was performed for the other. The follow-up period of the patients was an average of 43 (5-95) months. No breast recurrences were seen during the follow-up period. Conclusion: Pre-operative diagnosis is important for these cases in order to determine the surgical approach. It is difficult to differentiate these tumors from fibroadenoma by ultrasonography and mammography. It is possible to diagnose patients through tissue biopsy. In conclusion, a surgical approach with negative margins for all patients should be taken into consideration since phyllodes tumors are generally benign both clinically and pathologically.
