    ADIPOSE TISSUE DERIVED STEM CELLS AND THEIR USES IN PLASTIC SURGERY

    Stem cell treatments have become one of the most studied area in recent years and and it is seen that they will take the place of organ transplants in the future. Adipose tissue derived stem cells are also widely used by other branches outside of Plastic Surgery as they can easily be obtained in large quantities and effective as much as haemopoetic stem cells. Publications related to soft tissue augmentation, wound healing, and tissue engineering have been reported in plastic surgery practice. However, there are still some question marks about their reliability and lack of standardization for obtaining and using stem cells. The purpose of this review is to give knowledge about the concept of fat tissue derived stem cells, their properties, their mechanisms, also their uses in Plastic Surgery and discuss reliability of them.

    Axillary lipogranuloma mimicking carcinoma metastasis after silicone breast implant rupture: A case report

    Silicone-gel-filled breast implants have been widely used for breast augmentation and reconstruction after mastectomy. However, silicone implants have some well-known complications, such as implant rupture, which requires surgical intervention. Dissemination of silicone particles out of the implant causes a granulomatous reaction, a phenomenon known as silicone granuloma, in breast parenchyma as well as axillary, breast and chest wall lymph nodes, which mimics breast cancer metastasis. However, lipogranuloma after silicone breast implant rupture has not been reported in the literature, although it is a common complication after mineral oil or liquid silicone injection. We present a case report of an axillary lymphadenopathy resulting from lipogranuloma after silicone-gel-filled implant rupture. Review of the literature suggests that this is the first report of a lipogranuloma resulting from implant rupture. (C) 2012 British Association of Plastic, Reconstructive and Aesthetic Surgeons.     A Different Stabilization Technique of Autogenous Cartilage Grafts in Saddle Nose Deformity: Prevention Warping and Resorption

    Background: Saddle nose deformity (SND) is a collapse of the cartilage and bone structures forming the dorsal projection of the nose. After the use of autologous cartilage in the treatment of SND, resorption and warping continue to be a problem. Methods: This study presents results from 11 patients with SND types III and IV from 2005 to 2017 treated with autologous cartilage fixed with microplates and microscrews. The patients were monitored for at least 1 year. Resorption and warping were measured using postoperative photographs of the patients in the 3rd and 12th months after treatment. Results: No signs of resorption or warping were noted in the patients. Conclusions: Our findings indicate autologous cartilage use with microplates and microscrews is a viable, long-term treatment option for patients with SND types III and IV.

    Direct Cutaneous Metastasis of Lung Carcinoma: Case Report

    Cutaneous metastasis from lung carcinoma is a rare condition. While cutaneous metastasis is commonly seen in large cell carcinoma type of lung carcinomas, rarely seen in squamous cell carcinoma type. Usually, cutaneous metastasis occurs as a result of hematogenous spread. In this report, we presented a case of direct cutaneous metastasis in lobectomy incision site, which is rarely seen in squamous cell lung carcinoma.

    An frontalis sling operation using an autogenous en-bloc, fan-shaped tensor fascia lata graft for blepharoptosis

    This article presents a modification of the frontalis sling operation for severe blepharoptosis with poor levator function. The fascia or other suspension materials are usually used in strip form to avoid a mass under the skin and lid-crease obliteration in the frontal sling operations. However, the 'strip-sling technique' carries the risk of some complications including irregular eyelid contour, unstable fixation and loss of elevating power in the follow-up period. To overcome these complications and to enhance elevating power transmission between the tars and the frontal muscle, the autogenous tensor fascia lata graft was designed to be en bloc and fan-shaped. This technique was used in nine patients (12 eyelids) who presented with different aetiologies. They had an average follow-up of 13.5 months. Although all the patients achieved favourable results with good eyelid opening, the patients who had traumatic ptosis were more prone to complications including lagophtalmos and pulling away problem. This modification appears to increase the surface contact area between the frontalis muscle, the fascia lata graft and the tarsus. This increased contact enhances tarsus elevation in ptotic eyelids and is associated with satisfactory results. (C) 2012 British Association of Plastic, Reconstructive and Aesthetic Surgeons.     The better understanding of the biology of wound healing shows that platelets and especially the growth factors they contain play a main role on hemostasis and healing. Studies show that autologous platelet rich plasma increases wound healing. Its clinical applications are also increasing and recently its effects on tissue regeneration have been discussed. In this study, the different PRP concepts, and their effects on wound healing and clinical applications in plastic surgery were discussed.

    Reconstruction of Large Anterior Scalp Defects Using Advancement Flaps

    Reconstruction of the scalp may be challenging for reconstructive surgeons because of the special anatomic structure of the scalp and underlying skeleton. Anterior scalp defects especially deserve special care for pleasant hairline re-creation and redirection of hair follicles. Local transposition or rotation flaps are the most common reconstruction methods for moderate or large anterior scalp defects. However, currently available techniques usually require multiple stages for completion of the reconstruction without alopecia. In this study, we report our experience with unilateral or bilateral advancement flaps for moderate or large anterior scalp defect reconstructions. Eight patients who had anterior scalp defect of varying etiology were presented. The defect size ranged between 3 x 5 and 8 x 12 cm. The average size of the defect was 53. 3 cm(2). The defects were located on the frontoparietal area of the scalp in 3 patients and frontal area in 5 patients. Unilateral advancement flap was used in four cases. All the flaps survived except in one patient who had partial flap loss. Based on our experience, we suggest that large scalp defects located especially on the frontal or frontoparietal area can be reconstructed with unilateral or bilateral scalp advancement flaps in one stage without the need for multiple surgeries.

    Stahl Ear Correction Using the Third Crus Cartilage Flap

    Stahl ear is usually associated with the existence of a third crus that traverses the scapha. The absence of the superior crus of the antihelix, a broadened scapha, and unfolded, long helical rim may contribute to the formation of the deformity. The method presented in this article is a combined technique intended to recreate the absent superior crus using the third crus as a cartilage flap along with elimination of the third crus, reducing the scapha size, and correcting helical rim deformities by skin and cartilage excisions. One bilateral and three unilateral cases were operated on with this technique (five auricles). The first unilateral cases were performed without scapha reduction and helix correction which was used in the other three cases with successful results. This procedure can be used to correct Stahl ear variations in the case of an absent superior crus when a third crus is present, the scapha is of increased size, and the helix is long without a fold.
