Öğe 18-45 Yaş Grubu Kadınlarda, Trichomonas Vaginalis Ve Diğer Mikroorganizmaların Vajinal Akıntı Örneklerinden Mikroskobik Olarak İncelenmesi(2012) Keşli, Recep; Pektaş, Bayram; Özdemir, Mehmet; Günenç, Oğuzhan; Coşkun, Erkan; Baykan, Mahmut; Baysal, BülentAmaç: Trichomonas vaginalis vajinal mukozada nekrotik ülserler ve yüzeysel defektlerle beraber köpüklü, yeşilimsi kötü kokulu akıntı ile karekterize olan trichomoniasise neden olan bir protozoondur. Trichomoniasis insandan insana seksüel temasla taşınır ve hemen hemen dünyanın her tarafında görülür. Bu çalışmanın amacı 18-45 yaş grubu vajinal akıntı şikayeti olan ve 1 Eylül-15 Aralık 2003 tarihleri arasında Konya Sosyal Sigortalar Kurumu Hastanesi Kadın Hastalıkları ve Doğum Polikliniği'ne başvuran 70 kadında Trichomonas vaginalis sıklığının belirlenmesidir. Yöntemler: Örnekler bir spekulum ve steril pamuklu eküvyonlu çubuklar yardımı ile vajen posterior forniksinden alındı. Bütün örnekler ışık mikroskobu altında nativ preparat, Gram ve Giemsa boyama metodları ile incelendi. Bulgular: Yetmiş örneğin 6'sı (%9) Trichomonas vaginalis, 9'u (%13) Gardnerella vaginalis biri Mobilincus spp. ve 11'i (%16) Candida spp. pozitif bulundu. Sonuç: Ttrichomoniasis kesin tanısı kültür metodu ile yapılmasına rağmen direkt mikroskobi ile vajinal smear incelemesinin enfeksiyonun tanısında önemli bir role sahip olduğu söylenebilir. Direkt mikroskobik inceleme trichomoniasis tedavisine başlamaya karar vermekte yardımcı olacaktır.Öğe Akciğer iskemi reperfüzyonunun tetiklediği böbrek hasarında karnosol ön koşullaması(2014) Apilioğulları, Burhan; Keşli, Recep; Esme, Hıdır; Yüceaktaş, Ali; Yoldaş, BanuAmaç: Bu deneysel çalışmada, akciğer iskemi/reperfüzyonu (İ/R) ile indüklenen böbrek hasarı üzerine karnosolun koruyucu etkisi araştırıldı.Çalışma planı: Yirmi dört Yeni Zelanda beyaz tavşanı üç gruba randomize edildi. Grup IR; akciğer İ/R grubu (60 dk. iskemi-60 dakika reperfüzyon), grup CIR; akciğer İ/R'si öncesi karnasol bolus enjeksiyonu ve grup S; plasebo grubu (pulmoner hilus klemplenmedi). Böbrek dokusu miyeloperoksidaz (MPO) ve interselüler adezyon molekülü -1 (ICAM-1) düzeyleri değerlendirildi.Bul gu lar: Böbrek dokusu MPO ve ICAM-1 düzeyleri grup IR'de, grup CIR ve grup S ile karşılaştırıldığında, anlamlı olarak daha yüksek idi (sırasıyla, p0.021 ve p0.0001). Grup CIR ve grup S arasında ise ölçülen parametrelerde istatistiksel olarak anlamlı fark tespit edilmedi.So nuç: Çalışma sonuçlarımız akciğer İ/R'sine bağlı böbrek hasarında aktif nötrofil sekestrasyonu ile ilişkili MPO ve ICAM-1 düzeylerinin arttığını ve karnosolun bu hasarda koruyucu etkisi olabileceğini göstermektedirÖğe Association of Severity of Coronary Lesion with Markers of Acute Infection and Inflammation in Patients with Acute Coronary Syndrome(2016) Koçak, Sedat; Dündar, Zerrin Defne; Harmankaya, Atıf; Erdemir, Esma; Keşli, Recep; Alibasic, HayrudinAim: Inflammation and some infectious agents play a key role in acute coronary syndrome (ACS) caused by atherosclerosis. The purpose of this study was to assess the effects of inflammatory markers and the positivity of Chlamydia pneumoniae (CP), Helicobacter pylori (HP), and Cytomegalovirus (CMV) on the level of atherosclerosis in patients with ACS. Materials and Methods: Patients (57) that were referred to the emergency unit with classic angina symptoms or angina equivalent symptoms and were determined to have critical lesions in the coronary angiography (<70% stenosis, coronary artery disease (CAD) severity assessed by the Gensini score) were compared with 27 ACS patients who had no critical lesions in terms of procalcitonin (PCT), tumor necrosis factor-alpha (TNF-?), interleukin-2 receptor (IL2r), interleukin-6 (IL-6), and interleukin-10 (IL-10) levels and positivity of CP, HP, and CMV. Also, the two groups of ACS patients were compared in terms of cytokine levels measured at hours 0 and 48. Results: No significant association was found between the degree of the coronary lesion and the inflammatory and infectious agents. However, in patients with critical coronary lesions, as markers of inflammatory agents, the levels of IL-6 were significantly lower and levels of IL-10 were significantly higher (p>0.001 and p0.030, respectively) at hour 48 than originally found at hour 0. Conclusion: There is no association between the severity of coronary lesions and cytokine levels and positivity of infectious agents in ACS since the levels of proinflammatory cytokines in ACS are higher than those in atherosclerosis. The changes in cytokine levels at hour 48 were found to be significant.Öğe The prevalence of infectious agents in patients with systemic sclerosis(2015) Bilgin, Hüseyin; Kocabaş, Hilal; Keşli, RecepBackground/aim: Systemic sclerosis (SSc) is an autoimmune disease characterized by microvascular injury, excessive extracellular matrix deposition, and fibrosis in the skin and internal organs. Bacterial and viral infectious agents have been suspected to be contributing factors in the development and progression of the pathologic features of SSc. Materials and methods: In this study, 30 SSc patients who were admitted to the rheumatology unit of the Konya Training and Research Hospital and 30 healthy controls were included. The presence of 9 different antibodies (IgM and IgG) against Helicobacter pylori, cytomegalovirus (CMV), EpsteinBarr virus (EBV), and parvovirus B19 were investigated in sera samples obtained from the 60 participants using an enzyme-linked immunosorbent assay method. The characteristics of current and past infections with H. pylori, CMV, EBV, and parvovirus B19 were evaluated by determining the seropositivity of the tested bacterial and viral agents. Results: The prevalences of H. pylori, CMV, EBV, and parvovirus B19 were determined to be higher in patients with SSc than in the control group. Conclusion: SSc is associated with a higher rate of certain infections, which deserves further investigation in order to assess the role of infections in disease etiology/pathogenesis.
