In the present study, it was aimed to assess the clinical features, pre-diagnosis and dermatologic diagnosis of patients consulted to the Dermatology Department from Pediatrics Department, and also to detect possible different approaches between the clinics. Methods: The list of dermatology consultations requested from Pediatrics Department between 1 September 2016 and 1 September 2017 were retrospectively evaluated by means of pre-diagnosis, dermatological diagnosis and unit consultations. Results: The mean age of patients was 7.4 +/- 5.3 years. Of the 628 patients, 52.1% were males, and 85% of the consultations were requested from outpatient clinics and 15% were from the inpatient clinics. Among all consultations 28.7% general pediatrics unit and 17.5% pediatric emergency unit required consultations. The most common diagnosis reported from Dermatology Department were eczema (28.2%), viral diseases (13.2%), parasitic diseases (8.4%) and fungal diseases (5.4%). Although, the ratio of an accurate pre-diagnosis was significantly higher in consultations requested from the inpatient clinics than those from outpatient clinics(p<0.001), 42.7% of the patients were referred to Dermatology Outpatient Clinic with an incorrect pre-diagnosis. Conclusion: Almost half of the patients, who were referred to Dermatology consultation, were diagnosed with incorrect pre-diagnosis. It was seen that pediatricians were difficult to recognize dermatoses. The awareness of Pediatric Dermatology Department should be raised for both specialties.Öğe Comparison of anti-TNF and IL-inhibitors treatments in patients with psoriasis in terms of response to routine laboratory parameter dynamics(Taylor & Francis Ltd, 2022) Ataseven, Arzu; Temiz, Selami Aykut; Eren, Gullu; Ozer, Ilkay; Dursun, RecepBackground Biological agent treatments represent a relatively new field and their effects on routine laboratory parameters are not fully known as there has been little research in the area. In our study, we aimed to evaluate the differences between the two main biological treatment groups and their effects on routine laboratory parameters. Methods Patients were enrolled when they had received treatment for more than six months with biological treatments between January 2013 and April 2020. The available data on routine laboratory parameters were collected by routine blood tests before the treatment, at three months, and at the final evaluation. Results When the changes in routine laboratory parameters were evaluated by treatment, it was found that the NLR and CRP values decreased statistically significantly in the anti-TNF group compared to the IL inhibitor group. In addition, strong suppression of these inflammation parameters means strong suppression of the immune system response. In addition, AST, ALT, and creatinine values were found to be statistically significantly higher in the anti-TNF group compared to the IL inhibitor group. Conclusions In our study, anti-TNF treatments are shown to be more effective in reducing inflammation parameters while IL antagonists are safer in terms of biochemical parameters.Öğe Cutaneous Mucinosis Case with Characteristic Lion Face Appearance(Aves, 2018) Temiz, Selami Aykut; Dursun, Recep; Ataseven, Arzu; Ozer, Ilkay; Findik, SiddikaCutaneous mucinosis includes a heterogeneous group of skin diseases characterized by the deposition of mucin in the interval of the dermis. Mucin is a protein ordinarily found as part of the dermal connective tissues. Mucin is a mucopolysaccharide produced by mast cells and fibroblasts and includes hyaluronic acid and sulfated glycosaminoglycans. Because hyaluronic acid holds water, in disease states where mucin production is increased, the dermal connective tissue swells, which is defined as myxedematous. Systemic mucin deposition may include systemic involvement; monoclonal gammopathy or paraproteinemia have been detected in the large majority (83.2%) of scleromyxedema cases. A 50-year-old female presented with pruritic, flesh-colored papules spread throughout the body. She also exhibited a characteristic lion face appearance. These lesions had first appeared 18 months before in the neck. The protein electrophoresis and bone marrow biopsy of the patient were normal. Histopathological examination revealed widespread mucin accumulation with alcian blue and colloidal iron stains in the papillary and reticular dermis. Granulomas were not observed with CD68 staining, and no accumulation was observed with amyloid staining. The final diagnosis was cutaneous mucinosis. In this paper we present the case of a patient with generalized primary cutaneous mucinosis without any systemic disease.Öğe Cutaneous Vasculitis after Radiotherapy(Aves, 2019) Temiz, Selami Aykut; Ozer, Ilkay; Kanyilmaz, Gul; Ataseven, Arzu; Oltulu, PembeCutaneous vasculitis is a large heterogeneous group of diseases, where blood vessels are targeted by immunological and inflammatory reactions, which are the primary causes of this condition. Infections, medications, systemic collagenosis, chronic diseases, and malignancies are the secondary factors that cause cutaneous vasculitis. Hemangiomas are the most common primary benign tumors of the spinal cordand are rarely symptomatic. The most commonly manifestedsymptom is pain, but in rare cases, cutaneous vasculitis may lead to paraparesis and paralysis. Radiotherapy (RT) is a safe and effective treatment for symptomatic spinal cord hemangiomas. A 44-year-old male patient was admitted to our dermatology polyclinic with a complaint of a bilateral rash on both legs that had lasted for 1 week. The medical history of the patient included no disease other than a sacral hemangioma with symptomatic pain, for which the patient had been treated with 4500 cGy curative radiotherapy 1 month previously. In our case, it was thought that cutaneous vasculitis was caused by the radiotherapy without any other triggering factor. A skin biopsy was taken to arrive at a definite diagnosis, and in the histopathological examination, abundant amounts of extra-red blood cells and lymphocytes were observed, along with endothelial profiling in superficial vessels; all of which are findings consistent with vasculitis. The patient was diagnosed with cutaneous vasculitis, both clinically and histopathologically. To the best of our knowledge, radiotherapy as a cause of vasculitis has been the subject of very few studies in the literature to date. In this regard, the present report describes a case of cutaneous vasculitis as a possible immune-related side effect of RT.Öğe The Evaluation of 2D:4D Finger Ratios in Seborrheic Dermatitis Cases(Erciyes Univ Sch Medicine, 2021) Ozer, Ilkay; Temiz, Selami Aykut; Ozer, Sumeyye; Ataseven, Arzu; Dursun, RecepObjective: Seborrheic dermatitis is common in the 14-50 age group, especially the adolescent age group. It was determined that the ratio of finger lengths did not change in the adult period after being determined in the intrauterine period. At the same time, the ratio of the index finger length to the ring finger length (2D:4D) is opposite to that of the serum testosterone. Based on this two basic knowledge, the intrauterine hormonal effect that an individual has been exposed to at any age can be determined. In our study, it was aimed to investigate the relationship between 2D:4D finger length ratio, which is an indicator of intrauterine androgen exposure, and seborrheic dermatitis, in which androgens are accused in etiology. Materials and Methods: Our study was planned as a descriptive and cross-sectional study among patients who applied to the dermatology outpatient clinic. Seborrheic dermatitis cases and age- and gender-matched healthy volunteers who applied to the dermatology outpatient clinic without seborrheic dermatitis were included in the study. Results: In the evaluation made separately for both sexes; while it was observed that both females and males with seborrheic dermatitis group had a lower 2D:4D finger length ratio compared to the control group, this was only significant in the left 2D:4D finger length ratio in males. Conclusion: To the best of our knowledge, there is no study in both sexes that evaluate the relationship between seborrheic dermatitis and 2D:4D finger length ratio in the literature. To assess accompanying gallstone in CSU patients and to evaluate laboratory test results. We retrospectivelyscanned the files of the 108 patients with CSU who had been diagnosed CSU in September 2017-September 2019. Accompanied gallstone cases by detecting transabdominal ultrasound (TUS) or were 11 patients. Nine patients had also had cholecystectomy history. We seperated these 20 patients as Group A than the rest. WBC count is 9.38 +/- 2.37 in Group A and 8.03 +/- 2.04 in Group B. Platelet count was also differ between two groups significantly (Group A = 328.70 +/- 85.62 x 10(3)/mm(3) vs. Group B = 287.12 +/- 72.77 x 10(3)/mm(3)). Neutrophil count was more in Group A (5.87 +/- 1.65) than Group B (4.76 +/- 1.75). Gallstone history of family was significantly higher in Group A (n = 12 positive/8 negative) than Group B. These findings were significant statistically p < .05. CSU can be associated with inflammation and gallstones. It is needed to perform further investigations to detect these relations and pathways.Öğe Histopathological comparison of pathergy positive and negative areas of newly diagnosed Behcet patients(Wiley-Hindawi, 2021) Temiz, Selami Aykut; Balevi, Sukru; Oltulu, Pembe; Ozer, IlkayBackground Behcet's disease is an inflammatory vasculitis, and pathergy reaction is a compulsory element of the diagnostic criteria. In the literature, there is no study comparing the histopathology of negative pathergy tests and positive pathergy tests in newly diagnosed Behcet's patients. The primary goal was to investigate the frequency of vasculitis, which is an important finding of Behcet's disease, in pathergy histopathology. Methods The histopathology of patients with positive and negative pathergy tests revealed vasculitis or vasculopathy. In histopathology, neutrophils, lymphocytes, eosinophils, and mast cells in the perivascular area were counted and recorded as a percentage. Results Of the 88 Behcet's patients who participated in the study, 45 were female and 43 were male. The rate of vasculitis in the histopathology of those who were positive for the pathergy test was not statistically different than those of those with negative pathergy tests (P = .14). In our study, although the relationship between the male gender and uveitis was significant at the border, and the relationship between pathergy positivity and uveitis was not found to be significant, a significant relationship was found between vasculitis in histopathology and uveitis (P = .023). Conclusions Based on the results of our study, we think that a pathergy test of histopathology will contribute to the clinical diagnostic evaluation in cases with suspicious clinical pathergy tests and especially in cases with negative pathergy tests.Öğe The Importance of Preoperative Dermoscopy in Basal Cell Carcinoma Due to a Case of Nodular Cystic BCC(Erciyes Univ Sch Medicine, 2019) Temiz, Selami Aykut; Ozer, Ilkay; Ataseven, Arzu; Dursun, Recep; Findik, SiddikaBasal cell carcinoma (BCC) is the most widespread malignancy among all malignancies. In this report, we describe a patient who was referred for surgery with the diagnosis of cystic BCC by preoperative dermoscopy. Various laser and light systems, which have advantages and disadvantages, including the pro-yellow laser are used for the therapy of these disorders. Aim: The goal of the present study was to investigate the effectiveness and feasibility of the 577 nm pro-yellow laser for a broad range of indications including erythematelangiectatic rosacea, facial erythema, post-acne erythema, facial telangiectasis, hemangioma, genital angiokeratoma, and port wine stain nevus. Methods: A total of 98 patients (25 male, 73 female) older than 15 years who were treated with the pro-yellow laser for vascular disorders at the cosmetology unit between 2017 and 2019 were retrospectively included in the study. Results: The mean rate of recovery was 100% in genital angiokeratoma, 94.4% in spider angioma, 83.3% in facial telangiectasis, 74.8% in erythematotelangiectatic rosacea, 72% in facial erythema + facial telangiectasis, and 68.3% in facial erythema. Over 60% of improvement was observed in most patients with vascular disorders. There was no significant link between the Fitzpatrick skin type and treatment success. Treatment success was significantly low in cases with nasal involvement. The second-to-fourth digit (2D:4D) ratio, which is an indicator of prenatal sex hormone balance, has been studied in various diseases that are affected by hormones. A total of 369 individuals comprising 172 patients with psoriasis and 197 healthy volunteers were included in the study. Patients with psoriasis were divided into type-1 and type-2, according to age of onset and family history. The 2D:4D ratio of both hands was recorded for all participants. Females in the psoriasis group had a lower 2D:4D ratio in both hands compared with those in the control group, with no significant difference. Males in the psoriasis group had a higher 2D:4D ratio for both hands compared with those in the control group (P= .009 andP < .001 for the right and left hands, respectively). Further, male patients with type-1 psoriasis had a lower 2D:4D ratio compared to those with type-2 psoriasis. Our results suggest that an alteration of the estrogen-testosterone balance due to prenatal testosterone activity is an independent predisposing factor for psoriasis in males.Öğe The Knowledge, Attitude and Behaviours of Women above 18 Years Old about Genital Warts, Cervical Cancer and Human Papilloma Virus Vaccination(Wolters Kluwer Medknow Publications, 2016) Kurtipek, Gulcan Saylam; Cihan, Fatma Goksin; Ataseven, Arzu; Ozer, Ilkay; Turhan, Zeynep CanObjective: This study aims to evaluate the knowledge and attitudes of women on genital warts, cervical cancer and human papilloma virus (HPV). Methods: Women aged 18 years old and over, admitting dermatology outpatient clinics of Konya Training and Research Hospital for any reason, were included in this cross sectional descriptive study. A 19-question survey was administered to 543 women to evaluate their knowledge and attitudes on genital warts, cervical cancer, smear test, protection methods, HPV vaccination and education. The collected data were analyzed by SPSS 21.0. Results: Only 16.6% (n=90) of the participants had information about genital HPV, among them only 38.9% (n=35) had information about the treatment of genital warts. While 61.1% of women having knowledge about genital HPV (n=55) knew about cervical cancer, 56.7% (n=51) knew genital warts might cause cervical cancer. 25.8% (n=140) knew of vaginal smear test while 16.9% (n=92) had a smear test before. Only 26.7% of women with genital HPV knowledge (n=24) knew prevention methods of genital warts. 40.0% of them (n=36) believed that condoms can protect them from genital HPV. 68% (n=369) had never heard of HPV vaccination. Conclusion: The study results showed that information level of the focus group is quite low about genital warts, cervical cancer, protection methods, screening methods, and HPV vaccination.Öğe Management of patients with Behcet's disease during theCOVID-19 pandemic(Wiley, 2020) Dursun, Recep; Temiz, Selami A.; Ozer, Ilkay; Daye, Munise; Ataseven, ArzuBehcet's disease is a disease characterized by chronic inflammatory vasculitis. In the clinical symptoms of Behcet's disease, many immunosuppressive and immunomodulatory drugs are used. The suspicion that drugs used in chronic disease treatments such as Behcet's disease during pandemic will increase the risk of transmission of COVID-19 disease, and that the disease may progress more lethally in these patients after the infection caused clinicians to worry. As far as we know, there is no study in the literature about the management of patients with Behcet's disease in the pandemic period. Fifty-four patients with Behcet's disease who were admitted to the dermatology outpatient clinic between 11 March and July 14, 2020 were retrospectively analyzed. In this pandemic period, 44 of 54 patients were recommended to continue their previous treatment. While the dose of medication used by 7 patients was reduced, it was decided to change the treatment of 3 patients. No life-threatening activation was observed. None of the patients developed COVID-19 disease. This article is important in terms of being the first study in the literature examining the treatment of patients with Behcet's disease during the COVID-19. In this period, we know that the treatment practices in chronic diseases change frequently daily, and in this respect, we hope that our study will provide a perspective to other dermatology clinics in terms of the treatment of Behcet's disease during the pandemic.Öğe Paradoxical plaque psoriasis due to secukinumab treatment in an ankylosing spondylitis patient(Wiley, 2020) Durmaz, Koray; Ataseven, Arzu; Ozer, Ilkay[Abstract Not Availabe]
