Öğe Assessment of Functional Status and Quality of Life in Children with Spina Bifida(Baycinar Medical Publ-Baycinar Tibbi Yayincilik, 2014) Sahin, Nilay; Albayrak, Ilknur; Durmus, Bekir; Gungor, Tayfun; Turac Cingoz, HavvaObjective: The aim of this study was to determine functional status and QoL of children with spina bifida (SB) by using the Functional Independence Measure for Children (WeeFIM) and the Child Health Questionnaire PF-50 (CHQPF-50) and to compare the functional status data of pediatric SB patients with those of healthy children. Material and Methods: Forty children with SB and 40 healthy children aged between 36 and 143 months were enrolled in the study. Both pediatric SB patients and healthy children were divided into three age groups: Group 1: 36-71 months, Group 2: 72-107 months, and Group 3: 108-143 months. The WeeFIM and CHQPF-50 were completed for children with SB, whereas the WeeFIM was completed only for healthy children. Results: In both assessments, the total score and subscale scores were lower than normal values in children with SB. However, improvement was found in self-care; communication; social, emotional, and mental status; and family factors with increasing age. On the other hand, there was no improvement in physical score, transfer, mobility, and sphincter control with increasing age. Functional status of children with SB was significantly lower compared to healthy children. Conclusion: There was progress in self-care, communication, family factors, and social, emotional and mental status in children with SB with increasing age.Öğe Clinical characteristics, disease activity, functional status, and quality of life results of patients with psoriatic arthritis using biological and conventional synthetic disease-modifying antirheumatic drugs(Turkish League Against Rheumatism, 2021) Keskin, Yasar; Nas, Kemal; Kilic, Erkan; Sargin, Betul; Kasman, Sevtap Acer; Alkan, Hakan; Sahin, NilayObjectives: This study aims to compare the clinical characteristics, disease activity, and quality of life (QoL) of patients with psoriatic arthritis (PsA) who use biological and conventional synthetic disease-modifying antirheumatic drugs (DMARDs) in a nationwide cohort throughout Turkey. Patients and methods: A total of 961 patients (346 males, 615 females; mean age: 46.9+/-12.2 years; range, 18 to 81 years) with PsA according to the classification criteria for PsA were included in the study. The patients demographic and clinical characteristics, physical examination results, Disease Activity Score 28, Disease Activity Index for Psoriatic Arthritis and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI), Psoriasis Area and Severity Index, Bath Ankylosing Spondylitis Functional Index, Bath Ankylosing Spondylitis Metrology Index, Hospital Anxiety and Depression Scale, Health Assessment Questionnaire, Psoriatic Arthritis Quality of Life (PsAQoL), and Short Form-36 scores were all recorded. Results: Of the patients, 23% underwent biological DMARD (bDMARD) monotherapy, 42% underwent conventional synthetic DMARD (csDMARD) monotherapy, 10% underwent a csDMARD combination therapy, and 10% underwent a combination bDMARD and csDMARD treatment. The Visual Analog Scale (VAS pain), patient global assessment, physician global assessment, and BASDAI scores were found to be lower among patients using combination treatment of csDMARD and bDMARD, while the swollen joint count was found to be lower among patients using bDMARD. The PsAQoL score was found to be the lowest among patients not using any medication and the highest among those using bDMARD. Conclusion: In our study, patients with PsA were successfully treated with both csDMARD and bDMARD monotherapy. When the biological treatments used for PsA were compared with csDMARD, it was found that biological treatments had a positive effect on both disease activity and the QoL. Combinations of csDMARDs and bDMARDs were preferred in cases in which the disease activity was still high or increased. Because of the highest efficacy of the combined treatment, we highly suggest increasing the number of patients on combined treatment.Öğe Comparison of effectiveness of different exercise programs in treatment of failed back surgery syndrome: A randomized controlled trial(Ios Press, 2017) Karahan, Ali Yavuz; Sahin, Nilay; Baskent, AkinBACKGROUND: Failed back surgery syndrome (FBSS) is a condition characterized by persistent pain following back surgeries. OBJECTIVE: To determine the effects of different exercises programs in FBSS. METHODS: A single-blind, randomized, controlled trial designed. One hundred participants with failed back surgery syndrome were randomly assigned to three different exercises groups (Isokinetic, Dynamic lumbar stabilization (DLS), and home exercises (HE) groups) and a control group. Patients were evaluated before and after 8-week exercise program and follow-ups at the 3rd and 6th months after the exercise program. Finger-floor distance and lumbar Schober for lumbar mobility, visual analog scale (VAS) for pain, Modified Oswestry Disability Index, Beck Depression Inventory, fear-avoidance attitudes survey, and progressive-isoinertial weight lifting test were used as follow-up parameters. Lumbar muscle strength was assessed with the isokinetic dynamometer. RESULTS: VAS levels were decreased from 67.7 to 22.8 in isokinetic and from 68.7 to 25.0 in DLS and from 64.6 to 47.1 in HE groups at the end of the program (p < 0.05). Also, all of other follow-up parameters of the isokinetic and DLS exercise groups viewed significant improvements compared to the control group. CONCLUSIONS: According to our results either isokinetic or DLS exercises are more effective in FBSS patients.Öğe Compliance to home-based exercise therapy in elderly patients with knee osteoarthritis(Baycinar Medical Publ-Baycinar Tibbi Yayincilik, 2016) Ay, Saime; Evcik, Deniz; Gokce-Kutsal, Yesim; Toraman, Fusun; Okumus, Muesser; Eyigor, Sibel; Sahin, NilayObjectives: This study aims to evaluate the compliance to home-based exercise regimen by elderly patients with osteoarthritis of the knee. Patients and methods: Between May 2012 and May 2013 year, a total of 72 patients (54 females, 18 males) over 65 years old with knee osteoarthritis admitted to the Physical Medicine and Rehabilitation outpatient clinic were included in the study. The patients were given a home-based exercise program. They were recommended to perform the exercises 10 times twice a day, for a month. We evaluated the patients' demographic data, comorbid diseases, whether they exercised previously and compliance to exercise. Results: All patients' home exercise program compliance was 62.5%. Males were more compliant than female patients and high education and comorbidity were also associated with poor compliance. Conclusion: The compliance of the geriatric patients to home-based exercise program is difficult and multiple variables affect the compliance.Öğe Isokinetic strength of the trunk flexors and extensors in ankylosing spondylitis(Ios Press, 2013) Durmus, Bekir; Sahin, Nilay; Baysal, Ozlem; Ersoy, Yuksel; Baysal, Tamer; Koca, Tuba Tulay; Altay, ZuhalBACKGROUND: Objective measurement of trunk muscles strength in ankylosing spondylitis (AS) may provide clinical information that can be used to evaluate functional disability. OBJECTIVE: To compare the isokinetic strength of trunk muscles in AS patients with healthy subjects. METHODS: Thirty-four patients with AS and thirty-one healthy subjects were enrolled. Patients with spinal ankylosis and bridged syndesmophytes in the lumbar and dorsal vertebrae were excluded. Trunk flexor and extensor strength was measured at: 60 and 90 degrees/sec. RESULTS: When compared with the control group, AS patients demonstrated 38% and 24% decrease in the extensor and flexor strength, respectively (p < 0.05). In addition, the flexor/extensor strength ratio was 1.0 and 0.8 in the patients and control subjects, respectively.
