Öğe Appendix in recurrent hernia sac (Amyand's hernia)(Aves Press Ltd, 2017) Tasci, Halil Ibrahim; Erikoglu, MehmetAmyand's hernia refers to the condition in which an inflamed, perforated, or normal appendix is present in the hernia sac. It accounts for 1% of all inguinal hernias and its preoperative diagnosis in patients is difficult. A 67-year-old male patient who underwent right-sided inguinal hernia repair with a mesh approximately 2 years before at another healthcare center was referred to our clinic with complaints of pain in his right inguinal area and swelling, which became evident while coughing or straining, for approximately 2 months. A surgical procedure to repair his recurrent inguinal hernia was planned, and appendiceal tissue was observed in the hernia sac during the procedure. Appendectomy was not performed because the appendix was observed to be normal. Tension-free mesh hernioplasty was performed. The patient was discharged on postoperative day 1, without any complications. Amyand's hernia is generally intraoperatively diagnosed and is a rare form of hernia with no specific symptoms. In this case report, we showed that despite the patient undergoing a surgical procedure, the appendix was present in the hernia sac because of recurrence, and we aimed at reviewing Amyand's hernia with the literature.Öğe A Case of Ectopic Parathyroid Adenoma at an Unexpected Location(Cukurova Univ, Fac Medicine, 2015) Tasci, Halil Ibrahim; Erikoglu, MehmetEctopic parathyroid tissue can be seen at any location from the mandibula to the mediastinum. The main problem in parathyroid surgery are determination of these ectopic parathyroid tissues' location and their resection. About 1-2% of patients with hyperparathyroidism have a deep mediastinal located parathyroid and transsternal or transthoracic approach should have to be employed most of the time. A 63-year-old female patient, presenting with widespread joint pain and fatigue complaints going on for about a year, was diagnosed with parathyroid adenoma in the mediastinal area as shown by the results of analyses conducted upon the detection of high levels of calcium and parathormone. Accordingly she was taken into surgery with a cervical incision and adenoma in a posterior localization in the carotid artery sheath on the upper mediastinum was seen. The patient, whose calcium, phosphor, and parathormone levels returned to normal levels following the surgery, was discharged with no problems on post-op day 2. Consequently, parathyroid is a tissue whose atypical localization is quite frequent. Nevertheless, atypical localized parathyroid surgeries can be performed by experienced surgeons without having to resort to a secondary procedure with minimally invasive methods with the contribution of localization studies before and/or during the procedure.Öğe A Case Of Sporadic Mesenteric Fibromatosis Mimicking Pancreatic Mass(Cukurova Univ, Fac Medicine, 2015) Tasci, Halil Ibrahim; Cakir, Murat; Esen, Haci HasanThe term abdominal fibromatosis refers to sporadic, pelvic, and mesenteric lesions and to all the fibromatosis lesions seen in Gardner's syndrome. Sporadic fibromatosis, however, is very rarer and literature offers a limited number of cases. The 14-year-old female patient presented to our clinic with complaints of indefinite abdominal pain in the epigastric area for the last 2 months, dyspeptic problems, and vomiting after eating. Upon the patient's gastroscopy revealed a mass lesion pressuring the stomach, endoscopic ultrasonography was performed. A hyperechoic mass lesion of 9x5 cm thought to have originated from the pancreatic tail was detected. The mass was surgically excised. Although mesenteric fibromatosis shows the characteristics of a benign tumor pathologically, it is extremely aggressive clinically and has a very high rate of recurrence. These patients should be treated like they have malign tumors and surgeons should perform surgical resection as wide as possible.Öğe Our clinical experiences in patients with de novo and secondary metastatic breast cancers(Bayrakol Medical Publisher, 2023) Tasci, Halil Ibrahim; Alkan, Selman; Varman, AlperAim: While breast cancer may be de novo, that is, metastatic at the time of diagnosis, a local tumor at initial diagnosis may metastasize to other regions over the course of treatment and follow-up. This study aimed to reveal the clinical, pathological, and survival differences in patients with de novo metastatic and secondary metastatic breast cancer who were followed up and treated in our clinic, and to investigate factors that may have an effect on survival.Material and Methods: The data of female patients who were diagnosed with metastatic breast cancer, treated, and followed up in our clinic between January 2000 and May 2021 were retrospectively analyzed. Patients were divided into two groups: the de novo metastatic disease group (Group 1) and the secondary metastatic disease group (Group 2). Clinical and pathological data of the groups were analyzed.Results: Patients with de novo metastatic disease most commonly had bone metastases. But in the secondary metastatic disease group, the most common metastasis type was multiorgan metastases. The rate of triple negativity was significantly lower in Group 1. When the groups were analyzed in terms of survival, the median time was 77 +/- 10.89 months in the de novo metastatic group and 66 +/- 10.15 months in the secondary metastatic group (p=0.05).Discussion: Our study demonstrated that de novo metastatic breast cancers had a better prognosis than secondary metastatic breast cancers even though they tend to metastasize early.Öğe Our Primary Colorectal Lymphoma Experiences(Galenos Yayincilik, 2014) Gundes, Ebubekir; Cakir, Murat; Kucukkartallar, Tevfik; Tasci, Halil Ibrahim; Tekin, Ahmet; Tekin, SakirAim: In this paper, we aimed to present our surgical approach to ten patients who underwent surgery for primary colonic lymphoma and the features and clinical presentations of the tumors. Methods: We retrospectively analyzed the data of ten patients who underwent surgery in our clinic for primary colorectal lymphoma between January 2006 and December 2012. Results: The median age of the patients at the time of diagnosis was 66 (50-84) years. The most frequent presenting complaints were abdominal pain and change in bowel habits. The most frequent physical examination findings were abdominal distention and widespread tenderness. Three patients underwent emergency surgery subsequent to acute abdomen diagnosis. During the surgery, it was observed that one patient had ileocecal invagination, one patient had a mass that caused cecal perforation, and one other had an obstructive mass in the sigmoid colon. The most common localizations were the cecum (60%), sigmoid colon (30%), and the rectum (10%), respectively. The most frequent type was B- cell lymphoma (n=8). Conclusion: Primary colonic lymphoma is a rare condition. It is still yet uncertain whether surgery or chemotherapy should be advised first.Öğe Retroperitoneal extra-adrenal paraganglioma: case report(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2016) Gundes, Ebubekir; Aksoy, Faruk; Tasci, Halil Ibrahim; Yilmaz, OsmanParaganglioma (PG), also known as extra-adrenal pheochromocytoma, is a rare neuroectodermal tumor. The incidence of extra-adrenal paraganglioma is between 00.1-0.1%. Paragangliomas originating from the retroperitoneum are generally functional. 40-50% of them are malignant and they generally have a slow progression. Surgical resection and radiotherapy are the preferred methods for the local treatment of these tumors. A case with a tumor localized in the retroperitoneal space that was totally resected by surgical method was evaluated.Öğe Sporadic mesenteric fibromatosis: a single center experience(Yerkure Tanitim & Yayincilik Hizmetleri A S, 2017) Tasci, Halil Ibrahim; Cakir, Murat; Senturk, MustafaObjective: Our aim with this study is to present the surgical approach of our clinic, the characteristics of the tumor, clinical presentation and long-term follow-up results with the literature in the cases of spontaneous mesenteric fibromatosis. Material and Methods: The data collected from 8 cases of patients, who had underwent surgery because of sporadic mesenteric fibromatosis at Necmettin Erbakan University Medical School's Department of General Surgery between January 2005 and December 2014, were retrospectively evaluated. Results: The mean age of the patients was 46.7 +/- 19.9. Four (50%) of the patients were taken into emergency surgery having been diagnosed with ileus, 2 (25%) patients were ascertained to have mesenteric mass upon complaints of obscure abdominal pain, 1 (12.5%) patient underwent surgery on the detection of a mass in the meso of the small intestines during a surgical procedure performed by urologists because of a benign cause and in 1 (12.5%) patient's case, the pathology results of the piece, which was removed after a surgical procedure performed because of a mass that might have been originated from the distal pancreas, was reported to be mesenteric fibromatosis. Only 1 (12.5%) patient had recurrence and necessitated re-operation following an average of 33.5 +/- 34.5 months of follow-up. Conclusion: The fundamental principle in the treatment of this clinically aggressive tumor with a high recurrence rate, although it has the pathological characteristics of a benign tumor, is wide surgical resection.
