Öğe Changes in cerebral oxygen saturation with the Trendelenburg position and increased intraabdominal pressure in laparocopic rectal surgery(Turkish Surgical Assoc, 2022) Alkan, Selman; Cakir, Murat; Senturk, Mustafa; Varman, Alper; Buyukbezirci, Gulcin; Yildirim, Mehmet Aykut; Bicer, MehmetObjective: Position changes and increased intra-abdominal pressure in laparoscopic interventions lead to some physiopathological changes. There is no definite information in the literature regarding cerebral oxygen saturation in patients undergoing colorectal surgery. Our aim was to investigate whether there is oxygen saturation change in the brain tissue in pneumoperitoneum and the Trendelenburg position during laparoscopic rectal surgery. Material and Methods: Cerebral oxygen saturation was measured in 35 patients who underwent laparoscopic rectal surgery in the Trendelenburg position. Measurements were made under general anesthesia in the pneumoperitoneum and the Trendelenburg position. Results: The values that are statistically affected by the position are systolic blood pressure, mean arterial blood pressure and cerebral oxygen saturation. The Trendelenburg position does not disturb the cerebral oxygen saturation and it causes an increase in saturation. After pneumoperitoneum occurred, changes in systolic blood pressure, mean arterial blood pressure and brain oxygen saturation were detected. Cerebral oxygen saturation increases with the formation of pneumoperitoneum. Conclusion: The Trendelenburg position and increased intraabdominal pressure during laparoscopic rectal surgery do not impair brain oxygen saturation.Öğe Emergency and elective approaches to femoral hernias(Bayrakol Medical Publisher, 2023) Varman, Alper; Alkan, SelmanAim: Femoral hernias often occur with incarceration, resulting in obstruction and strangulation. They usually require emergency surgical intervention and sometimes bowel resection may be required. The aim of this study is to compare the surgical results of femoral hernia cases in emergency or elective conditions.Material and Methods: The medical files of 38 patients who underwent surgery in January 2015-March 2022 with the diagnosis of femoral hernia at the general surgery service at Necmettin Erbakan University Meram Medical Faculty Hospital were retrospectively examined. Demographic data, party-relevant information, surgical technique, content of sac, length of hospital stay, recurrence and complications according to the last polyclinic were retrieved from the patient files and electronic database.Results: The gender distribution of the cases was 81.6% (n=31) females and 11.5% (n=7) males. The mean age was 45.74 +/- 16.11 years. The complication rate among the groups was significantly higher in those undergoing emergency surgery (p=0.014). No significant difference was found between the two groups in terms of the side of hernia (p=0.671).Discussion: Femoral hernia is more common in women. There are different repair techniques. More extensive prospective research is needed to compare surgical techniques and outcomes.Öğe Functional status, depression, and quality of life in female patients with postmastectomy lymphedema(Springer Wien, 2024) Varman, Busra; Yilmaz, Halim; Eryilmaz, Mehmet Ali; Mustu, Yurdagul Bahran; Varman, AlperBackgroundThis study aimed to evaluate QOL in female patients who had developed lymphedema (LE) following unilateral breast cancer surgery and investigate its relationship to shoulder pain, sleep and depression.MethodsA total of 69 female patients who had undergone unilateral mastectomy because of breast cancer and had been diagnosed with LE and 69 healthy controls were included in the study. While the QOL of participants was evaluated by the Short Form-36 (SF-36), depression levels were determined by the Beck Depression Inventory (BDI) and quality of sleep was evaluated by the Pittsburgh Sleep Quality Index (PSQI). Shoulder pain on the operated side in the patient group was appraised by the visual analogue scale (VAS).ResultsThis study revealed that LE patients had increased levels of depression, sleep disorders, and arm pain, while a decrease was found in the level of upper extremity functions and QOL. Therefore QOL, depression, and sleep disorders should be investigated in detail alongside upper extremity problems in the diagnosis and follow-up of patients with postmastectomy LE.ConclusionPostmastectomy LE causes chronic pain and depression and impairs patients' functionality and QOL.Öğe Mesenteric Cyst in 22 Cases(Springer, 2021) Yavuz, Yusuf; Varman, Alper; Senturk, Uyesi Mustafa; Kafadar, Mehmet TolgaBackground The aim of this study is to retrospectively and multi-centerly examine the clinicopathological features of patients who were operated with the diagnosis of mesenteric cyst, which is a rare cause of intra-abdominal mass in the last 9 years, in the light of the literature. Methods The patients were operated due to mesenteric cysts in the general surgery clinics of two Training and Research Hospital between 2010 and 2019; age, gender, preoperative clinical findings, computed tomography (CT) findings, localization of the mass, surgical procedure, morbidity, histopathological results, and follow-up period status were analyzed and reported electronically. Results The patients generally applied with the complaints of abdominal pain, palpable abdominal mass, and abdominal distention. Fourteen (63.7%) of the cases were male and 8 (36.3%) were female. The mesenteric cysts were located in the small intestine in 18 cases and colon in 4 cases. In the histopathological examination of the surgical materials, simple cyst was detected in 17 cases, lymphangioma in 4 cases, colon adenocarcinoma, and simple cyst in 1 case. In the postoperative period, superficial surgical site infection developed in 3 patients and morbidity developed in 1 patient due to anastomotic leakage. Conclusion Although mesenteric cysts are rarely seen, although most of them are not histopathologically malignant, they can reach large sizes and require extensive surgical operation, and related morbidities can be seen. Therefore, mesenteric cysts should be followed carefully in the postoperative period as well as during the diagnosis and surgical treatment process.Öğe Our clinical experiences in patients with de novo and secondary metastatic breast cancers(Bayrakol Medical Publisher, 2023) Tasci, Halil Ibrahim; Alkan, Selman; Varman, AlperAim: While breast cancer may be de novo, that is, metastatic at the time of diagnosis, a local tumor at initial diagnosis may metastasize to other regions over the course of treatment and follow-up. This study aimed to reveal the clinical, pathological, and survival differences in patients with de novo metastatic and secondary metastatic breast cancer who were followed up and treated in our clinic, and to investigate factors that may have an effect on survival.Material and Methods: The data of female patients who were diagnosed with metastatic breast cancer, treated, and followed up in our clinic between January 2000 and May 2021 were retrospectively analyzed. Patients were divided into two groups: the de novo metastatic disease group (Group 1) and the secondary metastatic disease group (Group 2). Clinical and pathological data of the groups were analyzed.Results: Patients with de novo metastatic disease most commonly had bone metastases. But in the secondary metastatic disease group, the most common metastasis type was multiorgan metastases. The rate of triple negativity was significantly lower in Group 1. When the groups were analyzed in terms of survival, the median time was 77 +/- 10.89 months in the de novo metastatic group and 66 +/- 10.15 months in the secondary metastatic group (p=0.05).Discussion: Our study demonstrated that de novo metastatic breast cancers had a better prognosis than secondary metastatic breast cancers even though they tend to metastasize early.Öğe Surprises after laparoscopic cholecystectomy: Incidentally detected gallbladder malignancies(Bayrakol Medical Publisher, 2023) Varman, AlperAim: Accidental gallbladder cancers have started to increase all over the world with the spread of laparoscopic cholecystectomy. The aim of this study is to evaluate gallbladder malignancies in patients who were not diagnosed with preoperative malignancies undergoing laparoscopic cholecystectomy for benign reasons in our clinic, accompanied by literature. Material and Methods: Cases of laparoscopic cholecystectomy performed in our clinic between 2010 and 2019 with the preliminary diagnosis of gallstones, chronic cholecystitis, delayed acute cholecystitis or benign gallbladder polyps were retrospectively reviewed. Demographic data such as age, gender and comorbidities of the patients were recorded. Patients with preoperative gallbladder or liver malignancy were excluded from the study. Results: Incidental gallbladder malignancy was detected in 11 of 5100 cases in the histopathological examination of cholecystectomy material. Nine cases were recalled and reoperated, and then transferred to oncology for chemotherapy. Two cases were followed nonoperatively. Discussion: Cholecystectomy operation is one of the most performed surgeries in a number of general surgery clinics and is mostly performed for benign reasons. For this reason, the process of following up and evaluating the histopathology results after the operation can be neglected. Today, with the widespread use of laparoscopic cholecystectomy, incidental gallbladder malignancies have increased. For this reason, pathology outcome follow-up after cholecystectomy or laparoscopic cholecystectomy operation is important.
