Öğe Approach to Traumatic Diaphragm Injuries: Single Center Experience(Taiwan Soc Emergency Medicine, 2021) Senturk, Mustafa; Cakir, Murat; Akbulut, Muhammed Ali; Yesildag, KerimBackground: Traumatic diaphragm injuries are rare. After blunt trauma, injuries occur with a 1-7% rate. This rate increases up to 15% in penetrating injuries. Diagnosis may be difficult and imaging tests may be misleading. The misdiagnosis may lead to herniated abdominal organs towards the intrathoracic cavity, with a mortality rate between 30% and 60%. This study was designed to investigate the data of patients operated for diaphragmatic injury in our clinic. Methods: The files of trauma patients admitted to our hospital between 2014-2019 were reviewed retrospectively. The files of cases with diaphragmatic injury were examined. Patients were evaluated in terms of age, sex, type of injury, localization, additional organ injuries, grade, surgical method, and mortality. Values were statistically analyzed. Results: A total of 20 patients were included in the study. Of the patients, 7 were female and 13 were male. The mean age was 32.7 (17-52) years. Of the cases, 10 were with stab wounds while 4 were incar traffic accidents and 6 were gunshot wounds. Although all patients had thoracoabdominal injuries, 4 patients had additional pelvic injuries. Of the cases, 14 were accompanied with lung injury while 5 with intestinal, 4 with the spleen, 3 with liver, and 2 with cardiovascular injury. Patients with intestinal resection performed had a longer hospitalization period than others. When the severity of the injury was evaluated, grade 3 injuries were detected most commonly, whereas 3 patients had grade 1, 2, and 4 injuries. Grade 5 injury was detected in 1 patient. Primary repair was performed in 18 patients and mesh repair was performed in 2 patients. In three cases, the repair was performed with a thoracic way. One patient died on the postoperative first day. Conclusions: Diaphragmatic injuries that may be missed during imaging may be damaged with many organs. Diaphragmatic injuries should be kept in mind in the upper abdominal and thoracic injuries.Öğe The role of torque teno virus (TTV) viremia in sarcoidosis etiology(Kuwait Medical Assoc, 2019) Teke, Turgut; Feyzioglu, Bahadir; Tosun, Mustafa; Dogan, Metin; Yesildag, Kerim; Ozdemir, MehmetObjective: To investigate a possible role of torque teno virus (TTV) in the etiology of sarcoidosis Design: Controlled prospective study Setting: Faculty of Meram Medicine, Necmettin Erbakan University, Konya, Turkey Subjects: This cross-sectional, clinical study included 37 sarcoidosis patients and 9 healthy control patients. Intervention: TTV DNA quantification was performed by real-time PCR by the hybridization probe system, and viral load was interpreted by means of the crossing point (CP) value in sera of subjects. Main outcome measures: The prevalence and viral load of TTV Results: TTV DNA was detected in the majority of both patients and healthy controls. The prevalence was 59.5% in patients and 55.6% in controls. This difference was not statistically significant. However, CP values were statistically different in the patient group from the control group (p < 0.001); namely, the viral load of TTV was higher in the patient group than in the healthy control group. Conclusions: Overall, these findings demonstrate for the first time that TTV plays a role in the etiology of sarcoidosis. It is likely that the role of TTV in the pathogenesis of sarcoidosis is an aberrant immune response, driven by viral antigens.Öğe Use of Tobacco and Tobacco Products By Public Employees in Konya Province and the Support Status of the Tobacco Control Law(Turkish Green Crescent Soc, 2021) Kutlu, Ruhusen; Marakoglu, Kamile; Demirbas, Nur; Yesildag, Kerim; Cavdarci, FerahnurThis study aimed to evaluate the frequency of the use of tobacco and tobacco products among public employees in Konya province and support for the tobacco control law. This cross-sectional, analytical study was carried out on 4,874 public employees. The participants were surveyed about the use of tobacco and tobacco products, the tobacco control law, and smoking ban in dosed areas. Of the participants, 59.7% were current and/or occasional users of various tobacco products. Specifically, 36.5% of the participants were current smokers. Men smoked 2272 times more than women (p<0.001) (Odds ratio 2.272, confidence interval 2.003-2.576). The highest frequency of smoking (54.4%) was found among the provincial police department personnel. Of the participants, 93.0% supported the ban on smoking in closed areas, and 28.3% supported increasing its coverage. The frequency of smoking among the personnel working in public institutions in our province was found to be higher than the national average. Despite this, almost all the personnel supported and implemented the smoking ban in dosed areas. In-service training on smoking and its harmful effects should be increased, and those who plan to quit smoking should be supported.
