Öğe Adaptation of the Childhood Anxiety Sensitivity Index for Use in Turkey(Turkiye Sinir Ve Ruh Sagligi Dernegi, 2015) Yilmaz, Savas; Zinnur Kilic, EmineObjective: Anxiety sensitivity (AS) refers to the fear of anxiety-related sensations. The aim of the present study is to investigate the validity and reliability of Childhood Anxiety Sensitivity Index (CAST) and adapting it into Turkish. Method: The study group consisted of 328 healthy individuals whose age between 8-15 years old. All subjects included in the study filled out the Childhood Anxiety Sensitivity Index (CAST) and the State-Trait Anxiety Inventories for Children (STAI-C). Results: The CAST was found to be significantly related to both trait and state anxiety scores (r=0.42 and r=0.32, respectively). It had a high internal consistency (Cronbach a= 0.74) and good test-retest reliability (r= 0.77, p< 0.001). Explorotary factor analyses showed that the CAST is comprised of 6 lower-order factors. However, CAST subfactors was established according to literature as a three factor model: physical, cognitive and social. Conclusion: According to the present study, the CAST Turkish version was shown to be a valid and a reliable scale.Öğe Are Problem Based Scenarios Supporting the Positive Affect and Empathy of Medical Students?(Elsevier Science Bv, 2013) Karaoglu, Nazan; Pekcan, Sevgi; Yilmaz, SavasThis cross-sectional study aims to determine the effect of a Problem Based Learning (PBL) scenario on positive-negative affects and empathic tendency of first year medical students. The Positive and Negative Affect Schedule (PANAS), Empathic Tendency Scale (ETS) was applied before and after the PBL sessions. The PA, NA and ETS Scale scores in pre and post tests were 30.29 +/- 7.50 and 32.04 +/- 6.50 (p=0.02); 18.03 +/- 5.94 and 17.31 +/- 6.36 (p=0.38), and 69.06 +/- 9.28 and 69.21 +/- 9.04 (p=0.81), respectively. This scenario significantly increased positive affect of medical students. There was also an increase in empathic tendency and a decrease in negative affect of males. (C) 2013 The Authors. Methods: Fifty adolescents (12-18 years old) with type 1 diabetes mellitus and 50 healthy adolescents and their parents were recruited for the study. Clinical interviews with the diabetic adolescents were performed using Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version (K-SADS-PL). Both groups completed the Depression Scale for Children, State-Trait Anxiety Inventory, and Health Related Quality of Life Scale for Children, while their parents completed the Parental Attitude Research Instrument, The Coping Strategy Indicator, and Health Related Quality of Life Scale for Children-Parent Form. Results: The psychological disorder ratio in diabetic adolescents was 68%. No significant difference was found regarding perceptions of quality of life between the diabetic group and control group. However, diabetic adolescents with psychological disorder had reduced perception of quality of life than those without psychological disorder. Among parental attitudes, an authoritarian attitude was found to be more common in the diabetic group. It was found that among coping strategies, parents in the diabetic group use avoidance more commonly. Conclusion: In the present study, a high rate of psychopathology was detected among adolescents with type 1 DM. In addition, no clear impairment in quality of life was reported in patients with type 1 DM; however, there was worsening in the perception of quality of life in the presence of psychiatric disorders accompanying diabetes. It was found that parents of diabetic children use inappropriate coping strategies and negative parental attitudes more often than those of healthy controls.Öğe Association of Reactive-Proactive Aggression and Anxiety Sensitivity with Internalizing and Externalizing Symptoms in Children with Attention-Deficit/Hyperactivity Disorder(Springer, 2017) Bilgic, Ayhan; Tufan, Ali Evren; Yilmaz, Savas; Ozcan, Ozlem; Ozmen, Sevgi; Oztop, Didem; Turkoglu, SerhatThis study evaluates the associations among the symptoms of anxiety, depression, and disruptive behavioral disorders (DBD) in the context of their relationships with reactive-proactive aggression and anxiety sensitivity in children with attention-deficit/hyperactivity disorder (ADHD). The sample consisted of 342 treatment-naive children with ADHD. The severity of ADHD and DBD symptoms were assessed via parent-and teacher-rated inventories. Anxiety sensitivity, reactive-proactive aggression and severity of anxiety and depression symp-toms of children were evaluated by self-report inventories. According to structural equation modeling, depression and anxiety scores had a relation with the DBD scores through reactive-proactive aggression. Results also showed a negative relation of the total scores of anxiety sensitivity on DBD scores, while conduct disorder scores had a positive relation with anxiety scores. This study suggests that examining the relations of reactive-proactive aggression and anxiety sensitivity with internalizing and externalizing disorders could be useful for understanding the link among these disorders in ADHD.Öğe Effect of OROS methylphenidate on encopresis in children with ADHD: a retrospective chart review(Springer, 2013) Yilmaz, Savas; Bilgic, Ayhan; Herguner, Sabri[Abstract Not Availabe]Öğe Effect of OROS Methylphenidate on Encopresis in Children with Attention-Deficit/Hyperactivity Disorder(Mary Ann Liebert, Inc, 2014) Yilmaz, Savas; Bilgic, Ayhan; Herguner, SabriObjectives: Although encopresis shows a high rate of comorbidity in patients with attention-deficit/hyperactivity disorder (ADHD), the etiologic origin of this relationship and the effect of ADHD drugs on encopresis are unclear. In this chart review, we explored the effect of OROS long-acting methylphenidate (MPH) treatment on encopresis in children with ADHD. We also evaluated the relationship between the clinical variables of ADHD and encopresis. Methods: The sample consisted of 21 children and adolescents (20 boys and 1 girl) with encopresis and coexisting ADHD 7-15 years of age. Their clinical characteristics and baseline (visit 1) and end of the second months' (visit 2) Conners' Parent Rating Scale (CPRS) subscores were recorded. Retrospective clinician determinations were made using the Clinical Global Impressions-Severity subscale (CGI-S) for encopresis severity and the Clinical Global Impressions-Improvement subscale (CGI-I) for encopresis response. Results: According to the CGI-I, 14 subjects (71.4 %) showed much or very much improvement in their encopresis at the second visit. All of the CPRS scores showed a significant reduction during the second visit. No association was found between the CGI-I score and the changes in any of the CPRS scores. Baseline oppositional defiant disorder (ODD) and conduct disorder (CD) scores were correlated with the CGI-S score; however, no association was found between core ADHD symptom severity and the CGI-S score. With regard to the encopresis outcome, the baseline CD score was negatively correlated with the CGI-I score, and the baseline ODD score was prone to show a negative correlation with the CGI-I score. Conclusions: These results suggest that coexisting behavioral problems may be a vulnerability factor based on the severity of encopresis, and that MPH treatment may have a positive effect on encopresis in children and adolescents with ADHD.Öğe The Effectiveness of Methylphenidate in the Treatment of Encopresis Independent from Attention-Deficit Hyperactivity Disorder Symptoms(Korean Neuropsychiatric Assoc, 2015) Akca, Omer Faruk; Yilmaz, SavasSeveral medications are reported to be effective in treatment of encopresis. To assess the effects of attention-deficit/hyperactivity disorder (ADHD) symptom dimensions on Internet addiction (IA) aft er controlling for Internet usage features among high school students. Methods. This study consisted of 640 students (331 females and 309 males) ranging from 14 to 19 years of age. The Internet Addiction Scale, the Conners-Wells 'Adolescent Self-Report Scale-Short Form, and a personal information form were completed by the participants. Statistical analyses were conducted for both sexes and the total sample. Results. According to the logistic regression analysis, attention deficit and playing online games were significant predictors of IA in both sexes. Other predictors of IA included behavioral problems for females, total weekly Internet usage time, and lifelong total Internet use for males. Hyperactivity and other Internet usage features did not predict IA. Conclusion. Method: The sample consisted of 542 treatment-naive children with ADHD and their biological parents. Children were assessed via both parent- and teacher-rated behavioral disorder scales. Parental affective temperament and ADHD symptoms were measured by self-report inventories. The relationships between psychiatric variables were evaluated using structural equation modeling. Results: According to parent-rated behavioral disorder scales, paternal cyclothymic and maternal irritable temperaments were associated with ODD scores, and maternal depressive temperament was associated with CD scores. In terms of teacher-rated behavioral disorder scales, maternal anxious temperament was associated with ODD scores, and paternal cyclothymic and maternal depressive temperaments were associated with CD scores. Conclusion: These results suggest that certain parental affective temperaments are related to an increase in symptoms of disruptive behavioral disorders in children with ADHD.Öğe Relationship Between Psychological Factors and Oral Health Status and Behaviours(Quintessence Publishing Co Inc, 2015) Alkan, Arzu; Cakmak, Omer; Yilmaz, Savas; Cebi, Taylan; Gurgan, CemPurpose: To examine oral health related behaviours in relation to anxiety, depression and attention deficit hyperactivity disorder (ADHD). Materials and Methods: This study included 364 subjects (195 female, age range 19-65 years; 169 male, age range 18-64 years). A psychological evaluation was performed using Beck's anxiety, Beck's depression and ADHD scales. A separate questionnaire covering oral health behaviours, including parafunctional habits, dental visits and toothbrushing frequency, was given to the subjects and completed before the clinical examination. The numbers of decayed, missing and filled teeth were recorded and the status of the subject's periodontal health was determined clinically using periodontal probes and panoramic radiographs. Results: There were significant differences in the gender, level of education, level of income, household size, parafunctional habits and number of missing teeth in those with anxiety and depression (p < 0.05) vs those without. Females and subjects with parafunctional habits and lower education and income level presented higher anxiety and depression scores. ADHD was associated negatively with household size and positively with dental visits and toothbrushing frequency (p < 0.05). The results of structural equation modeling (SEM) revealed that anxiety, depression and ADHD were indirectly related to missing teeth and periodontal status: subjects with higher depression and anxiety scores had more missing teeth and worse periodontal health. However, there was no association between psychological factors and decayed teeth or toothbrushing frequency. Conclusions: Anxiety, depression and ADHD can result in poor oral hygiene, thereby having indirect, adverse effects on the periodontal health status of affected individuals. Although the frequency of dental visits seemed to be related to anxiety and ADHD, the frequency of toothbrushing showed no such association.Öğe Sex Differences in the Effects of Anxiety and Anxiety Sensitivity on Visual Perception(Modestum Ltd, 2016) Yilmaz, Savas; Akca, Omer Faruk; Acikel, Burak; Bilgic, Ayhan; Kilic, Emine ZinnurObjective: Most previous studies investigating the relationship between visual perception and anxiety have found that anxiety-provoking stimuli are perceived as relatively larger in size. Thus, the present study used neutral stimuli to investigate the relationships among anxiety, anxiety sensitivity (AS), and visual perception in a group of male and female university students. Methods: The Visual Size Perception Assessment Test (V-SPAT), which requires subjects to define a neutral figure in a dichotomous manner (i.e., tall/short, large/small, wide/narrow, crowded/deserted) was administered to all participants (n:76). Additionally, the anxiety level and AS of each participant was determined using the Beck Anxiety Inventory (BAI) and the Anxiety Sensitivity Index-3 (ASI-3), respectively. Results: The BAI and ASI-3 total and cognitive scores of female participants were correlated with perceptions of deserted, whereas their ASI-3 social scores were correlated with perceptions of short. The ASI-3 cognitive scores of male participants were correlated with perceptions of crowded, and their ASI-3 physical scores were correlated with perceptions of tall. The present findings indicate that the visual perception of neutral objects is correlated with anxiety and AS. The common use of atypical antipsychotics in this age group may lead to various consequences, including movement disorders, such as tardive dyskinesia (TD). In this study, we aimed to present the diagnosis of TD due to long-term use of olanzapine and successful treatment with clozapine in an adolescent patient who has been followed with the diagnoses of moderate mental retardation and undifferentiated schizophrenia. It may be posited that female adolescents with chronic illnesses, receiving long-term antipsychotic medication, with affective and negative symptoms, mental retardation, and teeth infections should be monitored regularly for TD. Clozapine may be a choice in the treatment of tardive dyskinesia developing in adolescent patients.
