Sağlık hizmetlerinde sayısal uçurumun e-nabız sistemi ve e-sağlık okuryazarlığı ile birlikte incelenmesi

Özet
Sayısal uçurum, Bilgi ve İletişim Teknolojileri (BİT)’ne erişimde, kullanımda ve elde edilen
faydalarda karşılaşılan eşitsizlik durumu olarak tanımlanmaktadır. Sağlık hizmetlerinde sayısal uçurum;
sağlık teknolojilerine erişimde, kullanımda ve kullanım becerileri sonucunda elde edilen faydalardaki
farklılıklardır. Bu farklılıklar internet kullanımında, internetin aktif kullanımında, sağlık hizmetlerine
yönelik internet kullanımında, sosyo-demografik değişkenler arasındaki farklılıklarda, sağlık
teknolojileri kullanımda ve sağlık teknolojileri kullanımından elde edilen faydalarda ortaya
çıkabilmektedir. Bu doğrultuda tez çalışmasının amacı, T.C. Sağlık Bakanlığı tarafından 2015 yılında
hayata geçirilen e-Nabız sistemi hakkında katılımcıların bilgi, kullanım ve yeterlilik düzeylerini
belirlemek ve bilgi ve kullanım düzeyleri ile ilişkili olan sosyo-demografik değişkenlerin neler
olduğunu ortaya koymaktır. Bu sayede e-Nabız sistemi üzerinden sağlık hizmetlerinde sayısal uçurum
konusuda incelenmiştir.
Araştırmanın evrenini Konya ilinde yaşayan 18 yaş ve üzeri vatandaşlar oluşturmaktadır.
Örneklem, yapılan hesaplama sonucunda 385 olarak bulunmuş ve 457 katılımcı ile çalışma
gerçekleştirilmiştir. Araştırmada kullanılan anket soruları literatür taraması sonucu oluşturulmuştur.
Elde edilen veriler analiz edilirken, tanımlayıcı istatistikler ve iki kategorik veri arasındaki ilişkiyi
incelemek için ki-kare testi kullanılmıştır. Ayrıca değişkenlerin ilişki düzeyini tespit etmek için
Contingency Coefficient, Cramer’s V değerleri incelenmiştir.
Araştırmada, katılımcıların büyük çoğunluğunun (%74,2) e-Nabız sistemi hakkında bilgi
sahibi olmasına rağmen, katılımcılar arasında birinci seviye sayısal uçurum (erişim uçurumu)
yaşandığının bir göstergesi olarak, kayda değer oranda katılımcının (%26,8) e-Nabız sistemi hakkında
bilgi sahibi olmadıkları tespit edilmiştir. Aynı zamanda katılımcıların %32,6’sının e-Nabız sistemine
nasıl erişim sağlayacaklarını tam olarak bilmedikleri bulunmuştur. Katılımcıların e-Nabız sistemi
hakkında bilgi sahibi olmalarında internet kaynaklarının (%29,8) daha etkili olduğu tespit edilmiştir.
Bu durum ayrıca sağlık hizmetlerinde görülen sayısal uçurumun genel sayısal uçurumdan farklı
olmadığını göstermektedir. Araştırmada, katılımcıların yarıdan fazlasının (%56,7) e-Nabız sistemini
kullanmadıkları ve bundan dolayı katılımcılar arasında ikinci seviye sayısal uçurumun (kullanım
uçurumu) var olduğu tespit edilmiştir. Ayrıca katılımcıların önemli bir kısmı (%38,1) e-Nabız
sisteminin kullanabilmek için gerekli olan bilgiye tam olarak sahip olmadıklarını belirtmişlerdir. ENabız sistemini kullanan katılımcıların e-Nabız sisteminin sunmuş olduğu hizmetlerde yeterlilik
düzeylerinin farklılık gösterdiği tespit edilmiştir. Kullanımdan elde edilecek faydaların eşit bir şekilde
dağılmasını engelleyecek olan bu durum katılımcılar arasında üçüncü seviye sayısal uçurumun (yetenek
uçurumu) yaşandığını göstermektedir.
Araştırma neticesinde, sağlık hizmetlerinde sayısal uçurum yaşandığı ve özellikle de ikinci
seviye sayısal uçurumun ön plana çıktığı görülmüştür. Katılımcıların arasında e-Nabız sistemi bilgi ve
kullanım düzeylerinde görülen farklılıklarda eğitim, sağlık bilgisine erişmek için internet kullanımı,
sağlık alanındaki teknolojik gelişmeleri takip etme durumu ve e-Sağlık becerilerinin (online sağlık
bilgisine doğru kaynaktan erişebilme ve internetten erişilen sağlık bilgilerinin güvenilirliğini ayırt
edebilme) etkili olduğu sonucuna varılmıştır.
Digital divide is defined as the inequality encountered in accessing, using and benefiting from Information and Communication Technologies (ICT). Digital divide in health care; these are the differences in access to health technologies, usage and benefits obtained as a result of usage skills. These differences can arise in internet use, active use of the internet, internet use for health services, differences between socio-demographic variables, health technology use and benefits obtained from health technology use. Accordingly, the aim of the thesis study is to determine the knowledge, usage and adequacy levels of the participants about the e-Pulse system launched by the T.C. Ministry of Health in 2015 and to reveal the socio-demographic variables associated with their knowledge and usage levels. The population of the study is composed of citizens aged 18 and over living in Konya. As a result of the calculation, the sample was found as 385 and 457 participants were studied. The questionnaire questions used in the research were created as a result of the literature review. While analyzing the data obtained, chi-square test was used to examine the relationship between descriptive statistics and two categorical data. In addition, Contingency Coefficient and Cramer's V values were examined to determine the level of relationship of the variables. In this way, the digital divide in health services has been examined through the e-Pulse system In the research, although the majority of the participants (74.2%) had information about the ePulse system, as an indication that there is a first level digital divide (access divide) among the participants, it has been determined that a significant proportion of the participants (26.8%) do not know about the e-Pulse system. It was also found that 32.6% of the participants did not know exactly how to access the e-Pulse system. It was determined that internet resources (29.8%) were more effective for the participants to have information about the e-Pulse system. This also shows that the digital divide seen in health care is not different from the general digital divide. In the study, it was determined that more than half (56.7%) of the participants did not use the e-Pulse system and therefore there was a second level digital divide (usage gap) among the participants. In addition, a significant part of the participants (38.1%) stated that they did not have the information required to use the e-Pulse system. It has been determined that the participants who use the e-Pulse system have different levels of proficiency in the services provided by the e-Pulse system. This situation, which will prevent the benefits from being distributed evenly, shows that there is a third level digital divide (skill gap) among the participants. As a result of the research, it has been observed that there is a digital gap in health services and especially the second level digital gap comes to the fore. It was concluded that education, use of internet to access health information, following the technological developments in the field of health and e-Health skills were effective in the differences in e-Pulse system knowledge and usage levels among the participants.
