Özet
Bağışıklık sistemi ile ilgili hastalıklara bağlı olarak oral mikrobiyal kompozisyondaki değişiklik
diş çürüklerine neden olan önemli bir nedendir. Bu çalışmanın amacı, 16S rRNA tabanlı metagenomik
yaklaşımla primer immün yetmezlik hastalığı (PİYH) olan çocukların diş çürüğü mikrobiyotasını araştırmak
ve sonuçları sağlıklı deneklerden elde edilen sonuçlarla karşılaştırmaktır. Bu doğrultuda, diş çürüklü ve
primer immün yetmezliği olan on beş çocuktan oluşan bir hasta grubu ile diş çürüklü on beş sağlıklı
çocuktan oluşan bir kontrol grubu çalışmaya dahil edilmiştir. Çalışma deneklerinin DMFT indeksi, tükürük
akış hızı ve tamponlama kapasitesi değerlendirilmiştir. Daha sonra, diş çürüklerinden ekstrakte edilen
DNA'lar, 16S rRNA geninin V3-V4 hiper değişken bölgesini hedefleyen yüksek verimli Illumina dizileme
teknolojisi ile incelenmiştir. Dizileme sonucu elde edilen okumaların kalitesi, Genomes OnLine Database
(GOLD) ve Trimmomatic aracı kullanılarak kontrol edilerek ve geliştirilmiştir. Taksonomik profil
oluşturma için, okumalar, Ribosomal Database Project (RDP) sınıflandırıcısı kullanılarak Greengenes
veritabanına dayalı olarak hedef organizmalara göre hizalanmıştır. Sonuçlara göre iki grubun DMFT skoru,
tükürük akış hızı ve tamponlama kapasitesi benzer bulunmuştur. Buna ek olarak, akış hızı ve tamponlama
kapasitesi, %95 güvenle tür bolluğu ile hiçbir korelasyon göstermemiştir. Metagenomik analiz, tüm
numunelerde 2440 bakteri suşunun tanımlanmasıyla sonuçlanmıştır. PİYH ve kontrol gruplarının çürük
mikrobiyotası, PİYH grubunda daha fazla bulunan Prevotella melaninogenica CP002122 ve Prevotella
salivae AB108826 dışında önemli bir farklılık göstermemiştir. Her bir PİYH alt grubunun tür bolluğu da
kontrol grubuyla karşılaştırılmıştır. Actinomyces naeslundii, Lautropia mirabiils ve Prophyromonas
endodontalis kontrol grubunda immün disregülasyon grubuna göre anlamlı derecede yüksek bulunmuştur.
Kombine immün yetmezliği olan çocuklar ile kontrol grubu arasında anlamlı bir fark bulunmamıştır. Ancak,
Parascardovia denticolens, Scardovia wiggsiae, Atopobium parvulum, Prevotella nigrescens ve Prevotella histicola kombine immün yetmezlik tanısı konulan ve kök hücre nakli tedavisi alan çocuklarda kontrol
grubuna göre daha yüksek bulunmuştur. Streptococcus mutans bolluğu kontrol grubunda antikor eksikliği
olan çocuklara göre daha yüksek bulunurken, Lactobacillus fermentum ve Macellibacteroides fermentans
antikor eksikliği olan çocuklarda daha yüksek bulunmuştur. Sonuç olarak, PİYH ve kontrol gruplarının
çürük mikrobiyotasındaki farklılıklar, bakteriyel kompozisyonun değişmesinde bağışıklık sisteminin
rolünün olduğunu ortaya koymuştur. Bakteri kompozisyonundaki bu değişiklik, bakterilerin daha
karyojenik veya patojenik olmaları için uygun ortam sağlayabileceği öngörülmüştür.
The alteration in oral microbial composition due to immune system-related diseases is an important reason to cause dental caries. The purpose of this study was to investigate caries microbiota of children with primary immunodeficiency disorder (PID) by 16S rRNA-based metagenomic approach and compare the results to those obtained from healthy subjects. In this regard, a group of fifteen primary immunodeficiency patients with caries and a group of fifteen healthy children with caries as a control group were enrolled. DMFT index, saliva flow rate and buffering capacity of each study subject were assessed before metagenomic analyses. Then, DNAs extracted from tooth decays were examined by high-throughput sequencing targeting the V3–V4 hypervariable region of the 16S rRNA gene using Illumina sequencing. The quality of readings was checked and improved using Genomes OnLine Database (GOLD) and Trimmomatic tool. For taxonomic profiling, readings were aligned to target organisms based on the Greengenes database using the Ribosomal Database Project (RDP) Classifier. According to the results, DMFT score, saliva flow rate and buffering capacity of the two groups were found to be similar. Moreover, flow rate and buffering capacity had no correlation with species abundance with 95% confidence. Metagenomic analysis resulted in identification of 2440 bacterial strains in all samples. The caries microbiota of PID and control groups showed no significant differences except for Prevotella melaninogenica CP002122 and Prevotella salivae AB108826 which were more abundant in PID group. The species abundance of each PID subgroups was also compared to control group. Actinomyces naeslundii, Lautropia mirabiils and Prophyromonas endodontalis were found to be significantly higher in control group than immune dysregulation group. No significant differences were found between children with combined immunodeficiency and control group. However, Parascardovia denticolens, Scardovia wiggsiae, Atopobium parvulum, Prevotella nigrescens, and Prevotella histicola were higher in children diagnosed with combined immunodeficiency and had stem-cell transplantation therapy than control group. The abundance of Streptococcus mutans was higher in control group than children with antibody deficiency however Lactobacillus fermentum and Macellibacteroides fermentans were higher in children with antibody deficiency. In conclusion, the differences in caries microbiota between PID and control groups indicate the role of the immune system in altering the bacterial composition. This alteration in bacterial composition may provide the bacteria with the suitable environment to be more cariogenic or pathogenic.
