Psöriasis hastalarında obezite ilişkili melanokortin 4 reseptör gen mutasyonunun araştırılması ve bunun hastalığın başlangıç dönemi ve hastalık şiddeti ile ilişkisinin incelenmesi

Özet
Bu çalışmada psöriasis vulgarisli hastalarda obezite ile ilişkilendirilmiş melanokortin 4 reseptör gen mutasyonu olup olmadığı araştırılarak, bu mutasyonun psöriasisin başlangıç dönemi ve eşlik eden ek hastalıklar ile ilişkisinin değerlendirilmesi amaçlandı. Gereç ve Yöntem Deri ve Zührevi Hastalıkları polikiliniğine başvuran, histopatolojik ve/veya klinik olarak psöriasis vulgaris tanısı konmuş olan 101 hasta çalışmaya dahil edilerek hastalarda melanokortin reseptör geni mutasyon analizi yapıldı. Çalışmada yer alan psöriasis hastalarının dermografik özellikleri, ek sistemik hastalıkları, bel ve kalça çevresi, vücut ağırlığı, boy uzunluğu, vücut kitle indeksi değerleri kaydedildi. Bulgular Çalışmaya katılan 101psöriasis vulgaris hastasının 50'si kadın 51'i erkek idi. Yaş ortalamaları 41,99±12,60 idi. Hastaların medyan VKI değeri 28,2±5,27 bulundu. Çalışmada yer alan 101 hastanın 6'sında melanokortin 4 reseptör gen mutasyonu saptandı. Mutasyon saptanan hastaların 5(%83)'i erken başlangıçlı, 1(%17)'i geç başlangıçlıpsöriasis grubundaydı. Erken ve geç başlangıçlı hastalar arasında mutasyon görülme sıklığı anlamlı düzeyde farklı bulunmadı (p=0,84). Geç başlangıçlı psöriasis hastalarında diyabet, hipertansiyon ve hiperlipidemi görülme sıklığı erken başlangıçlı gruba göre anlamlı düzeyde yüksek bulundu (sırası ile p=0,001, p=0,01, p=0,039) Sonuç Sonuçlarımız melanokortin 4 reseptör mutasyonunun görülme sıklığının erken ve geç başlangıçlı hastalar arasında farklı olmadığını gösterdi. Melanortin 4 reseptör gen mutasyonun psöriasis gelişimde ve progresyonunda rolünün ortaya konabilmesi için daha fazla çalışmaya ihtiyaç vardır.
In this study, we aimed to investigate whether there is a melanocortin 4 receptor gene mutation associated with obesity in patients with psoriasis vulgaris, and to evaluate the association of this mutation with the onset period of psoriasis and its associated comorbidities. Materials and Methods A total of 101 patients with histopathological and / or clinical diagnosis of psoriasis vulgaris were included in the study and melanocortin receptor gene mutation analysis was performed. The dermographic features, additional systemic diseases, waist and hip circumference, body weight, height, body mass index values were recorded. Results Of the 101 patients with psoriasis vulgaris, 50 were female and 51 were male. The mean age was 41,99 ± 12,60. Median BMI was 28.2 ± 5.27. Melanocortin 4 receptor gene mutation was detected in 6 of 101 patients. Five (83%) of the patients with mutations were in early onset and 1 (17%) were in the late onset psoriasis group. The frequency of mutation was not significantly different between early and late onset patients (p=0,84). The incidence of diabetes, hyperlipidemia, hypertension was significantly higher in patients with late-onset psoriasis than in the early-onset psoriasis (p=0,001, p=0,01, p=0,039 respectively). Conclusion Our results showed that the incidence of melanocortin 4 receptor mutation was not different between early and late onset patients. Further studies are needed to demonstrate the role of melanortin 4 receptor gene mutation in the development and progression of psoriasis.
