Hepatopulmoner sendrom oluşumunda östrojenin rolü ve anti-östrojen tedavinin etkinliği

Özet
HPS oluşumunda, östrojen hormonunun ve antiöstrojen tedavinin etkisini incelemektir. Durum Değerlendirmesi: HPSdaki hipoksinin nedeni perialveoler kapiller damar çapı genişlemeleridir. Bu damar genişlemeleri ile östrojen ilişkisi araştırılmamıştır. Yöntem: Deneysel çalışmamızda herbiri 10 rattan oluşan 6 grup oluşturuldu. 1: Sham-kontrol grubu, 2: Yalnız koledoğu bağlanan grup, 3: Koledok ligasyonu ile ooferektomi yapılan grup,4: Koledok ligasyonu ile eksojen östrojen verilen grup 5: Koledok ligasyonu ile Tamoxifen verilen grup, 6: Eksojen östrojen verilen grup. Koledok ligasyonu ile 5 hafta beklenerek siroz oluşturuldu. Ratların akciğer ve karaciğer dokuları histopatolojik olarak incelendi ve perialveoler damar çapları okülometre ile ölçüldü. Arteriyel kangazı ve biyokimyasal parametreler değerlendirildi. Çıkarımlar: Siroz oluşan 2. grupta kontrol grubuna göre, artan serum östrojen düzeyi ile beraber hipoksi ve perialveoler damar çaplarında artma, ilaveten östrojen verdiğimiz 4. grupta ise hipokside ve perialveoler damar çapında 2. gruba göre daha belirgin artış izlendi. Siroz oluşturulmuş ve ooferektomi ile östrojen düzeyi azaltılmış 3. grupta ve tamoksifen verdiğimiz 5. grupta ise hipokside ve perialveoler damar çapında istatistiksel olarak anlamlı düzelme saptandı. Sonuçlar: Östrojen hormonunun sirozda oluşan HPS' daki perialveoler kapiller damar genişlemelerinde ve hipokside rolü olduğunu ve anti östrojen tedavinin sirozda ortaya çıkan HPS gelişimine engel olabileceğini düşünmekteyiz.
Hepatopulmonary syndrome consists of a triad of liver dysfunction, intrapulmonary vascular dilatation, and hypoxemia. The loss of vascular tone in patients with intrapulmonary vascular dilatation is unexplained. Many studies in liver cirrhosis and pregnancy revealed that the cause of cutaneous vascular dilatation (spider nevi) is estrogen. In our study, we try to explain the effects of estrogen in intrapulmonary vascular dilatation. We used an animal model for the study of hepatopulmonary syndrome by ligating the common bile duct in rats and inducing biliary cirrhosis over 5 week period. Six groups each of which contains ten rats (l: Sham-control, 2:Common bile duct ligation, 3:Common bile duct ligation and oophorectomy, 4:Common bile duct ligation and exogenous estrogen, 5:Common bile duct ligation and tamoxifen, 6: Exogenous estrogen groups) were studied. Arterial blood gases, hepatic biochemical and histological features, and oculometric measurements of intrapulmonary vascular dilatation were assessed. In common bile duct ligated rats we obtained biliary cirrhosis, hypoxemia and intrapulmonary vascular dilatation. Compare with only common bile duct ligation group, degree of hypoxemia and intrapulmonary vasculary dilatation were more evident in cirrhosis and exogenous estrogen administrated group however those were less in cirrhosis plus tamoxifen and cirrhosis plus oophorectomy groups. We consider that estrogen has a potential role in intrapulmonary vasculary dilatation and hypoxemia in hepatopulmonary syndrome and anti- estrogen therapy may prevent the development of intrapulmonary vascular dilatation in cirrhosis.
