Transrektal prostat biyopsisinde lokal anestezi: randomize vaka kontrollü çalışma

Özet
Bu çalışmada , inrarektal lidokain jel ve intrarektal lidokain jel ileperiprostatik alana enjekte edilen uzun etkili lokal anestetiklerin transrektalultrasonografi eşliğinde yapılan prostat biyopsisi sırasında ve sonrasında etkinliğinideğerlendirdik.Metotlar: Mayıs 2006-Ağustos 2006 tarihleri arasınada transrektal prostat biyopsisiyapılan toplam 100 hasta, her biri 25 kişiden oluşmak üzere 4 gruba randomize edildi.Biyopsi öncesi; grup 1'e lokal anestezi verilmedi, grup 2'ye intrarektal %2 lidokain jel,grup 3'e intrarektal %2 lidokain jel ve prostat tabanı ile vezikülo seminalis bileşkesine%0.25 bupivakain, grup 4'e intrarektal %2 lidokain jel ve grup 3'teki aynı periprostatikalana %0.25 ropivakain verildi. Biyopsi sırasında ve sonrasındaki ağrı seviyesi 10skalalı vizüel analog scale (VAS) ile değerlendirildi.Sonuçlar: Grup 1 ve 2 arasında ağrı skorlarında fark yoktu. Grup 3 ve 4'tekihastalar biyopsi sırasında (VAS1) ve biyopsiden 1 saat sonraki (VAS2) skorlarıaçısından kontrol ve intrarektal lidokain jel grubuna göre belirgin düşük VASskorlarına sahipti.Sonuç: İntrarektal lidokain jel ile periprostatik uzun etkili lokal anestetik ajanlarınkombinasyonu prostat biyosisi sırasındaki ağrıda belirgin azalma sağladı, buna karşılıkintrarektal lidokain jel uygulanımı ağrıda istatistiksel olarak anlamlı azalma sağlamadı.
In this study, we assessed the efficacy of intrarecal lidocain gel only andcombination of lidocaine gel with long acting anesthetic agents which injected ofperiprostatic area during and after transrectal ultrasound-guided prostate biopsy.Methods: A hundred men undergoing trnsrectal prostate biopsy from May 2006 toAugust 2006 were randomized into 4 groups of 25 patients each. Before the biopsy;group 1 received no local anesthesia, group 2 received %2 lidocaine gel intrarectally,group 3 received intrarectal lidocaine gel and %0.25 bupivacaine injected junction ofthe seminal vesicles and the base of prostate, group 4 received lidocain gel and injectedof %0.25 ropivacain in same location of group 3. Pain level during and after the biopsywas assessed using a 10-point linear visual analog scale (VAS).Results: There were no differences in pain score between group 1 and 2. Patients ingroup 3 and 4 had significantly lower VAS scores than those in control and intrarectallidocaine gel groups during (VAS1) and one hour after biopsy (VAS2).Conclusion: Combination of intrarectal lidocain gel and periprostatic long actinganesthetic agents is significantly reduces pain during biopsy, whereas intrarectallidocaine gel administration did not istatical significant reduce on pain.
