Obsesif kompulsif bozukluk tanılı hastalarda çocukluk çağıdikkat eksikliği hiperaktivite bozukluğu varlığının bilişsel işlevlerüzerine etkisinin incelenmesi

Özet
Amaç: Yapılan güncel çalışmalar Obsesif Kompulsif Bozukluk (OKB) hastalarında bilişsel
işlev bozukluklarında tutarsız sonuçlar saptamıştır. Bilişsel işlevlerdeki bozulma OKB’nin
çekirdek semptomlar arasında yer almasa da bu bozulmaların hastalık şiddetini ve işlevselliği
büyük ölçüde etkilediği ile ilgili araştırmalar mevcuttur. OKB hastalarında bilişsel işlev
bozukluklarındaki heterojen sonuçlar yeterince araştırılmamıştır. Araştırmamızın amacı,
çocukluk çağı döneminde Dikkat Eksikliği ve Hiperaktivite Bozukluğu tanısı olan ve olmayan
erişkin Obsesif Kompulsif Bozukluk hastalarında bilişsel işlevleri karşılaştırmak ve
farklılıkları tespit etmektir.
Gereç ve Yöntem: Araştırmaya DSM-5 tanı ölçütlerine göre Obsesif Kompulsif Bozukluk
tanısıyla takip edilen ve çocukluk döneminde DEHB olan 35 hasta, çocukluk döneminde
DEHB olmayan 44 hasta ve 40 sağlıklı dahil ediliştir. Araştırmamız gözlemsel bir vakakontrol çalışmasıdır. Tüm hastalara ve sağlıklılara araştırmacı tarafından hazırlanan
sosyodemografik veri formu, Yale-Brown Obsesyon Kompulsiyon Ölçeği (YBOKÖ),
Hamilton Depresyon Ölçeği, Hamilton Anksiyete Ölçeği uygulanmıştır. Öz bildirim ölçekleri
olan Wender-Utah Ölçeği (WUDO), Barratt Dürtüsellik Ölçeği-11 (BDÖ-11) ve Erişkin
Dikkat Eksikliği Hiperaktivite Değerlendirme Ölçeği (EDEHDÖ)'nin katılımcılar tarafından
doldurulması istenmiştir. Daha sonra tüm hastalar ve sağlıklı gönüllülere klinisyen eşliğinde
Cambridge Nörofizyolojik Değerlendirme Test Bataryası (CANTAB) uygulanmıştır.
Bulgular: Çocukluk çağında DEHB olmayan hasta grubunda ailede OKB öyküsü oranı ve
psikiyatri servis yatış öyküsü oranı çocukluk çağında DEHB olan hasta grubundan yüksek
bulundu. Çocukluk çağında DEHB olan hasta grubunda kendine zarar verme davranışı
öyküsü, antipsikotik kullanımı ve Barratt dürtüsellik alt ölçekleri ve toplam puanı çocukluk
çağında DEHB olmayan hasta grubundan yüksek bulundu. Çocukluk çağı DEHB olmayan
hasta grubunda karar verme kalitesi skoru sağlıklı gruba göre daha düşük saptandı. Çocukluk
çağı DEHB olmayan hasta grubunda eğitim düzeyi düştükçe ve tanı alma yaşı arttıkça karar
verme kalitesinin ve görsel uzamsal belleğin belirgin kötüleştiği; tanı alma yaşı arttıkça karar
verme sürelerinin belirgin uzadığı saptandı. Çocukluk çağında DEHB olan ve olmayan OKB
hasta gruplarının yanıt inhibisyonu bilişsel işlevinde farklı alt alanlarda (hata yapma oranı,
süre, demene kaçırma gibi) farklı performans sergiledikleri saptandı. OKB hastalarında Stop
Sign Testi'nde durdurma yön hataları ve yap yön hatalarının DEHB tanısını yordadığı
saptandı. Çocuklukta DEHB öyküsünün OKB hastalarındaki bilişsel esnekliğin bozulmasına
neden olduğu saptandı.
Sonuç: Çalışmamızda çocukluk çağında DEHB olan ve olmayan OKB hastalarının klinik
özellik ve bilişsel işlev bozuklukları açısından belirgin farklılıklar sergilediği saptandı.
Bulgularımızın doğrulanması ve konu ile ilgili daha ayrıntılı bilgi edinilebilmesi için daha
geniş örneklemli, komorbit psikiyatrik hastalıkların dışlandığı, benzer tedavi alan hastalardan
oluşan çalışmaların yapılması gereklidir.
Aim: Recent studies have found inconsistent results in cognitive dysfunction in patients with Obsessive Compulsive Disorder (OCD). Although deterioration in cognitive functions is not among the core symptoms of OCD, there are studies on how these impairments greatly affect disease severity and functionality. The heterogeneous consequences of cognitive dysfunction in OCD patients have not been adequately studied. The aim of our study is to compare cognitive functions and determine the differences in adult patients with Obsessive Compulsive Disorder with and without Attention Deficit and Hyperactivity Disorder in childhood. Materials and Methods: 35 patients with childhood ADHD and 44 patients without ADHD in childhood, who were followed up with the diagnosis of Obsessive Compulsive Disorder according to DSM-5 diagnostic criteria,and 40 healthy, were included in the study. Our research is an observational case-control study. Sociodemographic data form prepared by the researcher, Yale-Brown Obsession Compulsion Scale (YBOCS), Hamilton Depression Scale, and Hamilton Anxiety Scale were applied to all patients and healthy people. Participants were asked to fill in self-report scales such as Wender-Utah Scale (WUDO), Barratt Impulsivity Scale-11 (BDI-11), and Adult Attention Deficit Hyperactivity Rating Scale (ASRS). Then, the Cambridge Neurophysiological Assessment Test Battery (CANTAB) was administered to all patients and healthy volunteers under the supervision of a clinician. Results: The rate of family history of OCD and history of psychiatric service hospitalization was found to be higher in the patient group without ADHD in childhood than in the patient group with childhood ADHD. History of self-harming behavior, use of antipsychotics, and Barratt impulsivity subscales and total scores were found to be higher in the patient group with childhood ADHD than in the group of patients without childhood ADHD. In the group of patients without childhood ADHD decision-making quality score was found to be lower than the healthy group. In the group of patients without childhood ADHD decision-making quality and visual-spatial memory deteriorate significantly as education level decreases and age at diagnosis increases; It was found that as the age at diagnosis increased, the decisionmaking times were significantly longer. It was determined that OCD patient groups with and without childhood ADHD had different performances in response inhibition cognitive function in different sub-domains (such as error rate, duration, and avoidance). In OCD patients, stopping directional errors and directional errors in the Stop Sign Test were found to predict the diagnosis of ADHD. It was determined that the history of ADHD in childhood caused cognitive flexibility deterioration in OCD patients. Conclusion: In our study, it was determined that OCD patients with and without ADHD in childhood showed significant differences in terms of clinical features and cognitive dysfunctions. In order to confirm our findings and obtain more detailed information on the subject, it may be recommended to conduct studies with larger samples, consisting of patients receiving similar treatment and excluding comorbid psychiatric diseases.
