    Analysis of Consultation Cases Referred from Pediatrics Department to Dermatology Outpatient Clinic: Retrospective Study

    Objective: There are a few studies in the literature about dermatology consultations requested from Pediatrics Department. In the present study, it was aimed to assess the clinical features, pre-diagnosis and dermatologic diagnosis of patients consulted to the Dermatology Department from Pediatrics Department, and also to detect possible different approaches between the clinics. Methods: The list of dermatology consultations requested from Pediatrics Department between 1 September 2016 and 1 September 2017 were retrospectively evaluated by means of pre-diagnosis, dermatological diagnosis and unit consultations. Results: The mean age of patients was 7.4 +/- 5.3 years. Of the 628 patients, 52.1% were males, and 85% of the consultations were requested from outpatient clinics and 15% were from the inpatient clinics. Among all consultations 28.7% general pediatrics unit and 17.5% pediatric emergency unit required consultations. The most common diagnosis reported from Dermatology Department were eczema (28.2%), viral diseases (13.2%), parasitic diseases (8.4%) and fungal diseases (5.4%). Although, the ratio of an accurate pre-diagnosis was significantly higher in consultations requested from the inpatient clinics than those from outpatient clinics(p<0.001), 42.7% of the patients were referred to Dermatology Outpatient Clinic with an incorrect pre-diagnosis. Conclusion: Almost half of the patients, who were referred to Dermatology consultation, were diagnosed with incorrect pre-diagnosis. It was seen that pediatricians were difficult to recognize dermatoses. The awareness of Pediatric Dermatology Department should be raised for both specialties.

    Anaphylaxis in The Newborn with a Diagnosis of Cutaneous Mastocytosis: Case Report and Literature Review

    Mastocytosis is carachterized by excessive mast cell accumulation in the skin and/or internal organs. Mastocytosis is divided into two groups; systemic and cutaneous. Symptoms depend on excessive cytokine release and their local and systemic affects. Symptoms may be mild or cause life-threatening anaphylactic reactions. In the literature anaphylactic reactions reported in systemic disease of adult and childhood mastocytosis. We here present an anaphylaxis in the newborn with a diagnosis of cutaneous mastocytosis.

    Assessment of ELR, NLR, MPV, and CRP Levels in Patients With Acne Vulgaris

    BACKGROUND/AIMS: Acne vulgaris is a chronic inflammatory disease of the pilosebaceous unit. The relationship between some of the hematological parameters active in inflammation such as the neutrophil/lymphocyte ratio (NLR), the mean platelet volume (MPV), and C-reactive protein (CRP) has been demonstrated. In recent years, eosinophil cells have been shown to be associated with inflammation, and the eosinophil/lymphocyte ratio (ELR) has been shown to be an indicator of inflammation in some diseases. In this study, we aimed to evaluate the relationship between the ELR, NLR, MPV, and CRP levels, which are indicators of systemic inflammation, with acne severity parameters in patients with acne vulgaris. MATERIALS AND METHODS: A prospective study was planned with patients in our clinic who were diagnosed with acne vulgaris, who required routine blood parameters, and whose global acne score was calculated. The relationship between these parameters, which are an indicator of systemic inflammation and acne severity, was investigated. RESULTS: In our findings, MPV was found to be the strongest hematological marker for acne severity and inflammation. There was a significant difference between acne severity and ELR, and this difference was higher in the severe acne group. CONCLUSION: As far as we are aware, there has been no study on ELR levels in acne to date; this is the first study to evaluate this rate. Based on our findings, it is thought that ELR increases with the severity of acne and can be accepted as an indicator. In this context, we believe that there is a need for controlled studies involving the role of eosinophils in blood, tissue, and inflammation in the pathogenesis of acne. It is possible that the role of eosinophils in pathogenesis will explain itch-dominated acne patients and may also lead to the development of new targets for treatment.

    Assessment of knowledge, attitudes, and behavior about skin care and moisturizers in patients presenting to dermatology outpatient clinics

    Background and Design: Skin care includes the use of cleaning and cosmetic products (nourishing, moisturizing, sunscreen, etc.) that are specially prepared for different skin types. In our study, we aimed to determine the knowledge and behaviors about skin care and moisturizers in patients attending dermatology outpatient clinics. Materials and Methods: Patients, who attended our dermatology outpatient clinic for any reason between March 2013 and May 2013, were requested to fill out a questionnaire containing questions about their knowledge of skin care products and moisturizers and how to use them, using behaviors, where they get knowledge about them, what are the things they consider when choosing them, whom they ask for advice for these products, where they buy these products, and what are the things they consider when buying them. Results: The study involved 984 patients. 608 (61.8%) were female, 376 (38.2%) were male. 474 (48.2%) patients defined skin care as cleaning, (34.2%) 337 patients as lack of skin disease. 303 (30.8%) patients knew one skin care product, 213 (21.6%) - two skin care products, 284 (28.9%) - more products, and 184 (18.7%) patients had no idea. 528 (53.7%) patients were using skin products, 200 of them were using regularly and 328 were using sometimes. 456 (46.3%) patients were not using any skin care product. 648 (65.9%) subjects stated that they get information from dermatologists, 154 (15.7%) - via internet, 106 (10.8%) - via advertisements, 37 (3.8%) - from neighbors, and 39 (4%) subjects stated that they get information from pharmacy when choosing skin care products. 300 (30.5%) patients were regularly using moisturizers, 340 (34.6%) patients were using sometimes, 344 (35%) were not using moisturizers. Conclusion: Our patients did not have the behavior of using skin care products regularly. Our society needs to be informed about regular skin care, and choosing the right moisturizers, buying these products from appropriate places as well as using these products correctly.

    Amaç: Allerjik kontakt dermatit (AKD), allerjen madde ile önceden duyarlanmış kişilerde tekrarlayan temaslarda gelişen tip 4 aşırı duyarlılık yanıtıdır. Etiyolojide rol oynayabilecek allerjenleri saptamak için deri yama testinin yapılması ve bu sayede neden olabilecek faktörlerin ortadan kaldırılması ve hastaları yaşam kalitesini arttırılması amaçlanmaktadır. Yöntem: Çalışmada Ocak 2006-Ocak 2011 tarihleri arasında Deri ve Zührevi Hastalıkları Polikliniği’nde allerjik kontakt dermatit tanısı alan 1038 hasta değerlendirildi. Serilere göre en sık karşılaşılan allerjenler; yaş, cinsiyet ve meslek özellikleri kaydedildi. Bulgular: Çalışmamızda en sık karşılaşılan allerjenler sırasıyla standart seride nikel, gıda serisinde sodyum alginat (E401), kozmetik seride ise setilstearilalkol şeklinde bulunmuştur. Sonuç: Allerjik kontakt dermatit tanısı konan hastaların şikayetleri, anamnezleri, hobileri, meslekleri sorgulanmalı; uygun seçilmiş maddeler ile yama testi yapılmasına özen gösterilmelidir.

    Bullous disorders in Konya: A study of 93 cases

    Background and Design: Bullous diseases are a group of disorders caused by specific antibodies against tissue adhesion molecules in the skin and mucosa or by genetic inheritance. There are limited detailed epidemiological studies about bullous disorders in Turkey. In this study, we evaluated the demographic, clinical and immunopathological findings as well as course of the disease and follow-up properties in patients with bullous disorders in the 'Province of Konya. Materials and Methods: Ninety-three patients, who were followed up between 2006 and 2012, were included in our study: Since most of the patients were in pemphigus and bullous pemphigoid groups, we investigated data on those patients in more detail. Results: 53.8% of patients were female and 46.2% were male. The mean age was 52.9. 37.6% of patients had pemphigus vulgaris, - 34.4% bullous pemphigoid, - 15%hereditary epidermolysis bullosa, - 4.3% dermatitis herpetiformis, - 4.3% pemphigus foliaceus, - 2.2% pemphigus vegetans, - 1.1% Hailey-Hailey disease, and 1.1% of patients had cicatricial pemphigoid. The mean age of onset of pemhigus vulgaris was 45.9 years. Pemhigus vulgaris, which was most frequently observed in females, started at oral mucosa and the time of diagnosis was longer than in patients in the other geographic regions. The mean age of onset of bullous pemphigoid was 70 years and it was mostly observed in females. In 12.5 of cases, the initial manifestations occured the oral mucosa. None of the patients had a concomitant autoimmune disorder. Twenty-five cases were treated with systemic corticosteroids, 41 cases with systemic corticosteroids and adjuvant drugs; 11 patients received only adjuvant therapy, and 16 patients were administered only local steroids. Conclusion: We report the demographical, clinical and immunopathological, findings as well as course of the disease and follow-up properties in patients with bullous disorders in the Province of Konya.

    A Case of Nodular Cystic Acne Treated with Systemic Dapsone

    Dapsone is an aniline derivative from synthetic sulfones. The mechanism of action of dapsone is obscure in inflammatory diseases; however, it is suggested to inhibit neutrophil chemotaxis and lysosomal enzymes. Acne vulgaris (AV) is a chronic inflammatory disorder of the pilo-sebaceous unit. Herein we report a pediatric patient who was started systemic isotretionin due to severe nodular cystic AV but could not continue due to elevated liver enzymes and well response to systemic dapsone treatment. We consider that per-oral dapsone treatment may be a well-tolerated and effective treatment option in patients with moderate/severe, frequently recurring AV which is irresponsive to conventional therapies and may a good alternative to isotretionin particularly in pediatric cases.

    A case report of primary tuberculosis of the tongue

    At present, the rate of extrapulmonary tuberculosis is gradually increasing among all tuberculosis cases. The secondary tuberculosis of oral cavity represents 0.2-1.5% of all cases with extrapulmonary tuberculosis. Although incidence of oral lesions secondary to pulmonary tuberculosis ranges from 0.4% to 1.5%, primary tuberculosis of the tongue occurs very rare, and it is published only as isolated case reports. A male patient presented to our dermatology clinic with complaints of painful ulcerated white plaques on the tip of his tongue lasting for three months, and an excisional biopsy showed granulomatous inflammation with caseification necrosis. Pulmonary and systemic examination showed no evidence of pulmonary or other extra-pulmonary tuberculosis, and he was initiated therapy for diagnosis of primary tuberculosis of the tongue. We found that the lesion completely resolved during the follow up visit after 1 month. Primary tuberculosis of the tongue should be for differential diagnosis of resistant painful oral lesions.

    Çocukluk çağı alopesi areatalı olgularımızın geriye dönük değerlendirilmesi

    Amaç: Alopesi areata (AA)lı olguların yaklaşık %20si çocuklardır. Çocukluk çağında görülen alopesi areata ile ilgili sınırlı bilgi bulunmaktadır. Kliniğimizde takip edilen AAlı çocuk olguların demografik özelliklerini, hastalık tedavilerini ve takiplerini incelemeyi amaçladık. Gereç ve Yöntem: Yaşları 16 ve altında olan 110 AA hastasına ait veriler geriye dönük olarak incelendi. Yaş, cinsiyet, hastalık başlangıç yaşı, hastalık süresi, AA tipi ve başlangıç yeri, tırnak tutulumu, eşlik eden dermatoz veya sistemik hastalık varlığı, laboratuvar testleri, AA tedavileri ve takip edildikleri döneme ait bilgiler değerlendirildi. Bulgular: Olguların % 46,4ü kadın, %53,6sı erkekti. Yaş ortalamaları 10,35 yıldı. Hastalık başlangıç yaşı 8,65 yıldı. AA başlangıç yeri % 83,6 olguda scalp, % 5,4 olguda kaş, %5,4 olguda vücut kılları, %2,7 olguda kirpik, %2,7 olguda kaş ve kirpikti. Hastalık tutulum tipi %73,4 olguda AA, %19 olguda alopesi totalis, %5,4 olguda alopesi üniversalis, %1,8 olguda ofiyazisti. Tırnak tutulumu %36,3 olguda mevcuttu. Nevüs flammeus %2,7 olguda mevcuttu. Hastalık süresi ortalama 17,02 aydı. Eşlik eden dermatoz %2,7 olguda vitiligo, %6,3 olguda atopik dermatitti. Eşlik eden sistemik hastalık %1,8 olguda otoimmün tiroidit, %2,7 olguda Down sendromuydu. Tiroid otoantikorları olguların %0,9unda yüksekti. Olguların %4,5inde hipotiroidi, %0,9unda hipertiroidi saptandı. Olguların %24,5ine topikal kortikosteroid losyon, %30,9una antralin, %0,9una minoksidil %2lik losyon, %0,9una kalsipotriol losyon, %1,8ine topikal kalsinörin inhibitörü, %10una intralezyonel kortikosteroid tedavisi verildi. Topikal tedaviye dirençli %15,4 olguda sistemik kortikosteroid ve PUVA tedavisine geçildi. Olguların %14,5inde topikal tedaviler değişik kombinasyonlarda kullanıldı. Olguların %34,5inde remisyon gözlendi. Ortalama remisyon süresi 12,2 aydı. Olguların %26,3ünde ortalama 2 kez nüks gözlendi. Sonuç: Sonuçlarımızın, ülkemizdeki AAlı çocuk olguların epidemiyolojik verilerine katkıda bulunacağını düşünüyoruz. (Türkderm 2013; 47: 158- 60)

    Dapsone for the treatment of acne vulgaris: do the risks outweigh the benefits?

    Dapsone is a 4,4'-diamino diphenyl sulfone compound and an aniline derivative from synthetic sulphones. Sulphonamides were first used in humans as antimicrobial agents to treat streptococcal infections. Dapsone derived from sulphonamides was first used in the treatment of leprosy in 1940. Today, Dapsone treatment is among the treatment options for many dermatological diseases. Acne vulgaris is a chronic inflammatory disease, which causes scar formation and changed pigmentation. Acne affects 85% of teenagers, but can occur at any age and can last into adulthood and even lifelong. Through its antimicrobial, anti-inflammatory, and antioxidant effects, dapsone treatment (local or systemic) can also be used in the treatment of acne. Dapsone treatment can cause a variety of side effects that can be categorized as pharmacological, dose-related, allergic, or idiosyncratic reactions. In this review article, the risks and benefits of using dapsone treatment in acne vulgaris will be evaluated in light of the literature.

    Dermatoloji polikliniğine başvuran olguların deri bakımı ve nemlendiricilerle ilgili bilgi, tutum ve davranışlarının değerlendirilmesi

    Amaç: Sağlıklı derinin devamı için deri tiplerine özel hazırlanmış temizleme ve kozmetik ürünlerinin (besleyici, nemlendirici, güneş koruyucu gibi) kullanılması ve/veya uygulanması deri bakımı olarak isimlendirilir. Çalışmamızda dermatoloji polikliniğine başvuran olguların deri bakımı hakkında bilgi seviyesini, deri bakımı ürünü kullanma alışkanlıklarını, nemlendiriciler hakkında bilgi seviyeleri ve bu ürünleri kullanma alışkanlıklarını değerlendirmeyi amaçladık. Gereç ve Yöntem: Mart-Mayıs 2013 tarihleri arasında herhangi bir nedenle dermatoloji polikliniğine başvuran olgulara deri bakım ürünleri ve nemlendiricileri, kullanma alışkanlıkları, deri bakımı ile ilgili bilgileri nereden edindikleri, ürün seçiminde dikkat ettikleri noktalar, ürünleri kime danışarak ve nereden aldıkları, alırken neye dikkat ettiklerini sorgulayan anket doldurtuldu. Bulgular: Araştırmaya katılan 984 olgunun 608i (%61,8) kadın, 376sı (%38,2) erkekti. Çalışmamızda 474 olgu (%48,2) deri bakımı deyince temizlik anladığını, 337 olgu (%34,2) ise deri hastalığının olmaması olduğunu belirtti. Üç yüz üç (%30,8) olgu bir ürün, 213 (%21,6) iki ürün, 284 (%28,9) üç ve daha fazla ürün biliyorken, 184 (%18,7) olgu deri bakım ürünlerini bilmiyordu. Deri bakım ürünlerini 200ü düzenli ve 328i bazen olmak üzere 528 (%53,7) olgu kullanmakta, 456 (%46,3) olgu da kullanmamaktaydı. Olgularımız deri bakım ürünlerini seçerken; 648 (%65,9) olgu dermatoloğundan, 154ü (%15,7) internetten, 106sı (%10,8) reklamlardan, 37si (%3,8) komşusundan, 39u (%4) eczacısından yardım almaktaydı. Çalışmamızda; olguların 300ü (%30,5) nemlendiricileri kullanırken, 340ı (%34,6) bazen kullanmaktaydı, 344 (%35) olgu da nemlendirici kullanmadığını belirtti. Sonuç: Düzenli deri bakım ürünü kullanma alışkanlığı olgularımızın çoğunda bulunmamaktadır. Toplumun düzenli deri bakımı ve nemlendiricilerin doğru seçimi, uygun yerlerden alımı ve kullanılmasıyla ilgili bilgilendirilmesine ihtiyaç duyulmaktadır.

    The effect of isotretinoin therapy on oxidative damage in rats

    Isotretinoin is prescribed in many dermatologic disorders, but mostly in acne. There is limited research about oxidative stress induced by isotretinoin and its effects on the liver tissue, muscle tissue, and blood. In this study, oxidative damage of isotretinoin on the liver, muscles, and blood in rats at the therapeutic dosage for humans, is evaluated. Thirty, 2-months-old Wistar albino rats were randomly divided into four groups. Isotretinoin was administered at the human equivalent low dose of 7.5 mg/kg by gavage. Blood, liver, and skeletal muscle samples were taken from the animals under anesthesia. Oxidative stress and antioxidant defense markers such as Malondialdehyde (MDA), Protein carbonyl (PC), 8-OHDG (8-hydroxy-deoxyguanosine), SOD (Superoxide dismutase), GSH(Glutathione), GPX (glutathione peroxidase), NO (Nitric Oxide) levels, Aspartate aminotransferase (AST), Alanine aminotransferase (ALT), and creatine kinase (CK) levels were measured. There were significant differences between the ALT values of the control group and the third month of isotretinoin treatment group. Oxidative stress markers such as 8-OHDG, PC, GSH, GPX, and NO values significantly differed in month 3. SOD was significantly lower in the treatment groups compared to the control group. Our study supports that the levels of oxidative markers are increasing with the isotretinoin treatment so this may flare acne. GPX levels increased at the muscle tissue level, and may be responsible for the myopathy that is seen in acne patients. Addition of antioxidants to isotretinoin treatment may be beneficial in reducing oxidative damage.

    Evaluation of bowel habits in patients with acne vulgaris

    Purpose To evaluate the bowel habits of patients with acne vulgaris. Method In this case-control study, socio-demographical characteristics of the participants (age, gender, marital status, educational status, profession, economic status, smoking-alcohol habits, chronic diseases, previous operations, people they live with and diet) were questioned and the global acne grading scores of the patients were calculated. Bristol Stool Scale, Rome III IBS Criteria, International Physical Activity Questionnaire-Short Form, Hospital Anxiety Depression and KADF (dietary fibre information) scales were completed. Results The patient group consisted of 102 participants and the control group consisted of 104 participants. The mean age was 20.9 +/- 3.9 years old in the case group and 21.8 +/- 5.0 years old in the control group. Patient and control groups were similar in terms of age and gender. Smoking rates were significantly higher in the patient group (P = .035). The amount of coffee consumed/day was significantly higher in the patient group (P = .040). According to the global acne grading scores, 55.9% (n = 57) had medium, 39.2% (n = 40) had mild, 3.9% (n = 4) had severe and 1% (n = 1) had very severe acne. Anxiety scores were found to be significantly higher in the patient group (P = .005). When the case and control groups were compared for IBS presence, no significant difference was found (P = .317). Also, IBS was not related to acne severity (P = .162). Conclusion Further large sample sized studies are needed on this subject, as there is strong evidence about brain-gut-skin axis existence.

    Evaluation of dermatology consultations in the era of COVID-19

    It has been reported that dermatology practices may be a vector for SARS-CoV-2 transmission and elective cases should be postponed during the pandemic period. In this context, studies on the change of patient profile in Dermatology outpatient clinic have been conducted. However, there was no study in the literature about dermatology consultations during the pandemic period. One hundred and forty-seven dermatology consultation cases in the era of COVID-19 pandemic between March 11, 2020 and May 4, 2020 were retrospectively evaluated. Twenty-four patients (16.3%) had suspicion and signs of COVID during consultation (fever, cough, shortness of breath, etc.). Nine (37.5%) of these patients also had accompanying COVID-19 skin lesions (two urticarial lesions, two livedo and necrosis, two maculopapular eruption, two vesicular rashes, one pseudo-chilblain). The number of cases that were suspected to have COVID-19 was statistically significantly higher in consultations requested by the emergency department and intensive care unit, while there were no suspected cases in outpatient clinic consultations (P = .001). Two (1.4%) of these patients were diagnosed with COVID-19 confirmed by PCR within 2 weeks. We acknowledge that daily practice changes frequently during this period, but still our study provides a perspective to other dermatology clinics in terms of the requested dermatology consultations during the pandemic.
