Öğe Characteristics of the cornea in patients with pseudoexfoliation syndrome(Consel Brasil Oftalmologia, 2015) Oltulu, Refik; Satirtav, Gunhal; Kayitmazbatir, Emine Tinkir; Bitirgen, Gulfidan; Ozkagnici, Ahmet; Karaibrahimoglu, AdnanPurpose: To quantify the morphological alterations in corneal nerve fibers and cells in patients with pseudoexfoliation syndrome (PEX) and their relationship with the presence of hyperreflective endothelial deposits observed using in vivo confocal microscopy. Methods: One eye each of 37 patients with PEX and 20 age-matched healthy control subjects was evaluated by in vivo corneal confocal microscopy. Patients with PEX were further classified into two groups: those with and without hyperreflective endothelial deposits. We evaluated the densities of basal epithelial cells, anterior and posterior stromal keratocytes, and endothelial cells and structure of sub-basal nerve fibers. Results: The mean anterior and posterior stromal keratocyte and endothelial cell densities and corneal sub-basal nerve plexus variables were significantly lower in patients with PEX compared with those in healthy control subjects. The mean basal epithelial cell density did not significantly differ. Conclusion: Eyes with PEX presented decreased corneal sub-basal nerve plexus variables and cell densities in all corneas, except for the mean basal epithelial cell density. Further, a trend of lower corneal sub-basal nerve plexus measurements in patients with hyperreflective endothelial deposits compared with those without endothelial deposits was observed.Öğe Circular analyses of dates on patients with gastric carcinoma(Taylor & Francis Ltd, 2021) Karaibrahimoglu, Adnan; Ayhan, Seren; Karaagac, Mustafa; Artac, MehmetDates have great importance in cancer diseases. However, the date variables themselves are not analyzed. This study aims to evaluate the descriptive statistics of diagnosis, operation, and last examination dates in gastric carcinoma patients by circular analysis methods. Totally 502 gastric carcinoma patients were enrolled in the study. The mean month of diagnosis date was found in nearly November (similar to 10.86) for females and May (similar to 5.17) for male patients. The mean month of operation date was found March (similar to 3.24) for females, and July & August (similar to 7.79) for males. The mean month of the last examination date was found as February & March (similar to 2.61) for females, and May (similar to 4.85) for males. Moreover, the mean day of the week for diagnosis date was found Thursday (similar to 5.50) for both female and male patients. The fitting of distributions of all variables was checked, also, according to von Mises, Rayleigh, and Kuiper's tests. When the days and months were analyzed by classical descriptive statistics, the results were obtained completely different from the circular analyses results. Therefore, the dates and times should be analyzed in certain diseases to give an idea for physicians.Öğe Cleft Lip Nose Correction Combining Open Rhinoplasty With the Dibbel Technique(Lippincott Williams & Wilkins, 2015) Gundeslioglu, Ayse Ozlem; Altuntas, Zeynep; Inan, Irfan; Bilgen, Fatma; Jasharllari, Lorenc; Karaibrahimoglu, AdnanCleft lip nose rhinoplasty is a challenging procedure because of the different presentation and severity of the deformity. Due to this presentation type, there is still no standard procedure correcting all the components of the deformity although a number of techniques have been published in literature. In this study, the effectiveness of the combination of open rhinoplasty and the Dibbel technique with nasal sill augmentation was evaluated. We hereby report our experience with 7 patients who had unilateral cleft lip nose deformity with slumped lower lateral cartilage and underprojected and deformed dome, operated on between September 2010 and April 2013 by 1 surgeon. The mean age of the patients at the time of surgery was 24.5 years (18-38 years) and the patients were followed up for an average of 18.5 months (6-31 months). All patients were operated on with open rhinoplasty and Dibbel technique combination with nasal sill augmentation. Frontal, lateral, oblique, and basilar photographs were obtained preoperatively and postoperatively for each patient. Nasal projection, columella height, nasolabial angle, nasal sill symmetry, and base width were measured on the photographs for comparison of preoperative and postoperative results. All patients' medial and lateral cantus distances were used for photographic standardization. The results demonstrated that there was a statistically significant increase in nasal projection (2.13 +/- 0.28mm preoperatively versus 2.31 +/- 0.08mm postoperatively; P = 0.018), columella height (1.07 +/- 0.25mm preoperatively versus 1.21 +/- 0.18mm postoperatively; P = 0.028), nostril apex height (1.11 +/- 0.15mm preoperatively versus 1.22 +/- 0.11mm postoperatively, P< 0.028), nasolabial angle (77.71 +/- 8.74mm preoperatively versus 91.33 +/- 6.49mm postoperatively; P< 0.05), and nasal sill symmetry (0.42 +/- 0.15mm preoperatively versus 0.27 +/- 0.07mm postoperatively; P< 0.05), and a significant decrease of alar width (2.35 +/- 0.44mm versus 2.16 +/- 0.32mm postoperatively; P = 0.018) on the affected side in response to surgery. The results of this study demonstrated that the Dibbel technique and open rhinoplasty combination with nasal sill augmentation is an effective and safe method for the correction of cleft lip nose deformity in respect to nasal symmetry.Öğe Comparison of the Perioperative and Postoperative Effects of Levobupivacaine and of Levobupivacaine plus Adrenaline in Pediatric Tonsillectomy: A Double-Blind Randomized Study(Hindawi Ltd, 2017) Cicekci, Faruk; Sizer, Cigdem; Atici, Sait Selcuk; Arican, Sule; Karaibrahimoglu, Adnan; Kara, InciObjectives. We aimed to assess the effects of levobupivacaine and of levobupivacaine + adrenaline administered during pediatric tonsillectomy on the postoperative period. Methods. A total of 90 patients between the ages of five and twelve were divided randomly into two groups before tonsillectomy: levobupivacaine only (0.5%) 0.4 mg.kg(-1) or levobupivacaine (0.5%) 0.4 mg.kg(-1) + adrenaline (1 :200.000) administered by means of peritonsillar infiltration. Primary outcomes were postoperative pain scores recorded at various intervals until 24 hours postoperatively. Secondary outcomes included postoperative nausea and vomiting (PONV), time to first oral intake, time to the first administration of analgesics and total consumption of analgesics, and the amount of bleeding for all children. Results. In both groups, patients had the same postoperative pain scores and PONV rates, and equal amounts of analgesics were consumed up to 24 hours postoperatively. The two groups also had the same time until first oral intake, recovery time and time to the first analgesic request, and amount of bleeding. Conclusions. Methods: The patients in this study were analyzed in 2 groups: group 1 receiving riboflavin solution containing 20% dextran and group 2 receiving dextran-free riboflavin solution. Corneal thickness measurements were obtained with ultrasonic pachymetry at 6 different time points: preoperatively and at 0, 15, 30, 45, and 60 minutes after epithelial removal and initiation of ultraviolet A irradiation. Results: Twenty-seven eyes of 27 patients with progressive keratoconus were included in the study. In group 1 (n = 13), the mean preoperative thinnest pachymetric readings were 469.3 +/- 8.5 mu m, which decreased to 423.8 +/- 8.9 mu m after deepithelization and further decreased to 385.9 +/- 10.9 mm at 15 minutes. The corneal thickness assumed a steady course at the 30-, 45-, and 60-minute measurements (378.9 +/- 9.8 mu m, 384.5 +/- 10.5 mu m, and 396.7 +/- 9.4 mu m, respectively). The initial change was statistically significant (P < 0.01). In group 2 (n = 14), the corneal thickness decreased from 459.4 +/- 3.8 mu m to 414.7 +/- 2.6 mu m with deepithelization, after which it increased steadily with the initiation of the dextran-free riboflavin solution. The final measurement in group 2 was 474.1 +/- 9.4 mu mm; the increase at each time point was statistically significant when compared with the previous measurement (P < 0.01). Conclusions: Using isoosmolar riboflavin solution without dextran causes a steady increase in the corneal thickness during the cross-linking procedure, as opposed to riboflavin with dextran. This result might be beneficial in broadening the spectrum of cross-linking indications in patients with thin corneas.Öğe Investigation of the inflammatory biomarkers of metabolic syndrome in adolescents(Walter De Gruyter Gmbh, 2016) Can, Ummugulsum; Buyukinan, Muammer; Guzelant, Asuman; Ugur, Ayse; Karaibrahimoglu, Adnan; Yabanciun, SukriyeBackground: Metabolic syndrome (MetS) is a chronic and multifactorial syndrome characterized by a low-grade chronic inflammation, and a major risk factor for type 2 diabetes mellitus (T2DM) and cardiovascular disease (CVD). In our study, we aimed to investigate the serum levels of high sensitive C-reactive protein (hs-CRP), haptoglobin (Hp), alpha(2)-macroglobulin (alpha(2)-MG), platelet factor-4 (PF-4), fetuin-A, serum amyloid P (SAP) and alpha(1)-acid glycoprotein (AGP) in an adolescent population with MetS. Methods: This study was performed in 43 (18 males, 25 females) MetS adolescents between the ages of 13 and 17 years (14.70 +/- 1.15) and 43 lean controls were matched for age and sex. The serum levels of Hp, alpha(2)-MG, PF-4, fetuin-A, SAP and AGP were measured by using a multi-ELISA technique. Results: Serum Hp, fetuin-A (p < 0.01) and PF-4, hs-CRP, SAP, AGP (p < 0.001) values of the MetS subjects were significantly higher than those of the controls. No difference was found in serum alpha(2)-MG levels between the MetS and control groups (p = 0.184). We especially wanted to examine the contribution of eosinophil count (EC), platelet count (PC), and mean platelet volume (MPV) values, which are suspected predictive parameters for vascular endothelium damage and venoocclusive pathogenesis and etiopathogenesis, particularly in IP. Methods: A total of 40 IP patients fulfilled the study criteria. Forty healthy volunteers in a similar age group were included as the control group. Complete blood count values were compared between the two groups. Intergroup comparisons were performed using the Mann-Whitney U test, and the chi-square test was used to assess the relationship between categorical variables in the patient groups. The area under the curve was calculated by receiver operating characteristic (ROC) regression analyses. Epidemiological diagnosis percentages were calculated by finding cutoff values. Results: The IP group's high MPV, PC, and EC values compared to those of the control group were detected to be statistically significant (p<0.001, p=0,03, p=0.001, respectively). No statistically significant difference was observed between the two groups for other blood count parameters. Statistically significant values for IP were measured as MPV: positive predictive value: 84%; EC: positive predictive value: 71.4%; and PC: positive predictive value: 61.4%. Conclusions: High MPV, PC, and EC values are significant positive predictive factors in IP etiopathogenesis. No proof was detected for other blood count parameters playing an active role in IP etiopathogenesis.Öğe The presence of Torque teno virus in chronic obstructive pulmonary disease(E-Century Publishing Corp, 2014) Feyzioglu, Bahadir; Teke, Turgut; Ozdemir, Mehmet; Karaibrahimoglu, Adnan; Dogan, Metin; Yavsan, MehmetTorque Teno Virus (TTV) has been identified as transfusion-transmitted virus in humans, initially. Although TTV viremia is extremely common in the general population worldwide, there is no direct causal evidence linking TTV infection to specific clinical manifestations. Our hypothesis was that TTV might play a role in Chronic obstructive pulmonary disease (COPD) by inducing inflammatory mechanisms previously identified. The study was conducted on 57 COPD patients and 39 healthy control groups. COPD patient groups included: the patients (n:20) with exacerbation needed noninvasive ventilation, the patients (n: 19) who received only medical treatment, and the invited patients (n: 18) for outpatient control. Serum samples were collected from patients and voluntary blood donors. TTV DNA quantification was carried out with a real time PCR by the hybridization probe system and viral load was interpreted through the crossing point value. TTV DNA was detected in the majority of both patients and healthy control groups. The prevalence was 94.4% (17/18) in patients for outpatient control, 94.7% (18/19) in patients who received only medical treatment, 100% (20/20) in patients with exacerbation needed noninvasive ventilation and 84.6% (33/39) in healthy controls. This difference was not statistically significant. However, CP values was statistically different in all the patient groups from the control group. TTV DNA prevalence was higher in patients than healthy individuals. More interesting thing, viral load was highest in the patients with exacerbation needed noninvasive ventilation. As a result, TTV may be associated with COPD and the severity of it.
