Öğe A case report of primary tuberculosis of the tongue(Modestum Ltd, 2017) Korkmaz, Celalettin; Daye, Munise; Teke, Turgut; Toy, Hatice; Hasal, Eda; Yavsan, Durdu MehmetAt present, the rate of extrapulmonary tuberculosis is gradually increasing among all tuberculosis cases. The secondary tuberculosis of oral cavity represents 0.2-1.5% of all cases with extrapulmonary tuberculosis. Although incidence of oral lesions secondary to pulmonary tuberculosis ranges from 0.4% to 1.5%, primary tuberculosis of the tongue occurs very rare, and it is published only as isolated case reports. A male patient presented to our dermatology clinic with complaints of painful ulcerated white plaques on the tip of his tongue lasting for three months, and an excisional biopsy showed granulomatous inflammation with caseification necrosis. Pulmonary and systemic examination showed no evidence of pulmonary or other extra-pulmonary tuberculosis, and he was initiated therapy for diagnosis of primary tuberculosis of the tongue. We found that the lesion completely resolved during the follow up visit after 1 month. Primary tuberculosis of the tongue should be for differential diagnosis of resistant painful oral lesions.Öğe Cavitary pulmonary lesion due to systemic lupus erythematosus: An unusual manifestation(Professional Medical Publications, 2011) Maden, Emin; Teke, Turgut; Uzun, KursatSystemic lupus erythematosus (SLE) is an autoimmune disease of unknown pathogenesis. In this abstract we report a 34 years old female SLE case with cavitary pulmonary lesion as the pulmonary manifestation. In literature, SLE with cavitary pulmonary lesion is reported very rarely and thought to be due to infection or pulmonary embolism. Our case had no signs of infection and no finding of pulmonary embolism, the lesion resolved by steroid therapy and without antibiotic administration. In conclusion SLE should be thought in differential diagnosis of cavitary pulmonary lesions.Öğe Cigarette smoke and bleomycin-induced pulmonary oxidative stress in rats(Spandidos Publ Ltd, 2012) Teke, Turgut; Maden, Emin; Kiyici, Aysel; Korkmaz, Celalettin; Gok, Mehmet; Ozer, Faruk; Imecik, OktayBleomycin causes pulmonary fibrosis by increasing free oxygen radicals. Cigarette smoke is a strong oxidant which adversely affects pulmonary tissue. We evaluated the effects of cigarette smoke administered with intratracheal bleomycin on pulmonary tissue. We studied 3 groups of rats (n=10): one group received intratracheal saline and served as a control; one received intratracheal Neomycin (IT) (0.5 U/100 g body weight, single dose on the first day), and one group received intratracheal bleomycin (single dose on first day) and tobacco smoke (two times per day) (IT-S). After 4 weeks, the levels of malondialdehyde (MDA) and nitric oxide (NO) and the activities of superoxide dismutase (SOD) and xanthine oxidase (XO) were assayed in the homogenate of the lung tissue samples. The severity of interstitial fibrosis was assessed using the grading system described by Ashcroft. There was more intensive fibrosis in the IT and IT-S than in the control samples (P<0.001). The levels of MDA., NO and activity of XO were significantly increased (P<0.001, <0.002 and <0.002, respectively), and SOD activity (P<0.001) was significantly decreased in the IT group when compared to these values in the control group. The concentration of NO was significantly decreased (P<0.002), and SOD activity was significantly increased (P<0.05) in the lung tissue samples of the IT-S group. Theoretically, the combination of cigarette smoke and bleomycin may have a synergistic effect on oxidative lung injury. In conclusion, we showed that inhalation of cigarette smoke provides protection against oxidative stress in the lung tissue of rats with bleomycin-induced pulmonary fibrosis.Öğe Computed tomography findings in pulmonary involvement of wegener's granulomatosis: Pictorial review(Turkish Assoc Tuberculosis & Thorax, 2015) Poyraz, Necdet; Korkmaz, Celalettin; Yavsan, Durdu Mehmet; Keskin, Suat; Teke, Turgut; Odev, KemalWegener's granulomatosis is a necrotising granulomatous vasculitis which has a variable manifestations in the chest that are best described on computed tomography. Imaging findings may include masses or nodules, which may cavitate; consolidations and ground-glass opacities. Wegener's granulomatosis can mimic pneumonia, malignancy, and noninfectious inflammatory diseases. The purpose of this pictorial essay is to demonstrate the characteristic computed tomography findings of pulmonary Wegener's granulomatosis.Öğe Effciency of Silver Coated Urinary Catheter in Catheter-Associated Urinary Tract Infection in Critical Care Unit(Aves, 2010) Teke, Turgut; Yavuz, Zuhal; Atalay, Huseyin; Maden, Emin; Solak, Yalcin; Uzun, Kursatwith an incidence of 40%. The incidence of urosepsis occurs in approximately 16% of the ICU patient populations. The purpose of this study was to determine the efficacy of a silver coated urinary catheter in prevention of catheter-associated UTI. Material and Methods: We investigated 21 patients (mean age with 71.4 +/- 9.0 year and 14 M, 7 F) foley silicon urinary catheter and 20 patients (mean age with 67.6 +/- 8.7 year and 15 M, 5 F) with silver coated urinary catheter. In all patients, foley urinary catheters were changed with silver coated catheter in critical care unit. Results: The most common cause of infection was Klebsiella (33%). The susceptibility of gram negative microorganisms against antimicrobial agents were amicacin, piperacillin-tazobactam and carbapenem. The cost of antibiotic was higher in foley catheter group than silver coated group (p<0.001). Conclusion: Silver coated urinary catheter is expensive, it is cost effect according to urinary infection and antibiotic cost.Öğe The effect of acute kidney injury on the success of non-invasive ventilation in COPD patients with hypercapnic respiratory failure(Kuwait Medical Assoc, 2020) Ince, Mahmut Sami; Teke, Turgut; Karagoz, Ali; Yucel, Fatih; Demirbas, Soner; Korkmaz, CelalettinObjective: To investigate the effect of acute kidney injury (AKI) on the success of noninvasive ventilation in chronic obstructive pulmonary disease (COPD) patients with hypercapnic respiratory failure Design: Retrospective observational cohort study Setting: Faculty of Meram Medicine, Necmettin Erbakan University, Konya, Turkey Subjects: This clinical study included 55 patients with decompensated COPD: 29 patients with AKI and 26 patients without AKI. Intervention: Demographical data, APACHE II scores, urea and creatinine values before non-invasive mechanical ventilation (NIMV) administration were recorded. Main outcome measures: The effect of AKI on the success of NIMV Results: There was no significant difference between sex, age and baseline respiratory rate of groups. There were statistically significant differences in baseline mean APACHE II score (21.4 +/- 5.3 vs 18.3 +/- 4.6, p=0.028), baseline mean pH (7.23 +/- 0.1 vs 7.30 +/- 0.1, p=0.001), urea (113.7 +/- 43.6 vs 45.2 +/- 13.0, p<0.001) and creatinine values (3.1 +/- 2.0 vs 0.9 +/- 0.3, p<0.001) of Group 1 and Group 2. Logistic regression analysis showed that none of these variable values tested have any effect on NIMV outcomes. Of the baseline variables tested, age (OR: 0.85; 95% CI: 0.69 to 1.07), sex (OR: 1.29; 95% CI: 0.89 to 1.89), baseline respiratory rate (OR: 1.02; 95% CI: 0.92 to 1.14), APACHE II score (OR: 1.03; 95% CI: 0.87 to 1.23), and AKI (OR: 0.79; 95% CI: 0.15 to 4.18) were not related to the outcome of NIMV in the logistic regression. Conclusions: We determined that AKI did not affect the outcome of NIMV in decompensated COPD patients.Öğe Effects of comprehensive and intensive pulmonary rehabilitation and nutritional support on quality of life and functional status in patients with chronic obstructive pulmonary disease(Sage Publications Ltd, 2020) Korkmaz, Celalettin; Demirbas, Soner; Vatansev, Hulya; Yildirmn, Elif; Teke, Turgut; Zamani, AdilObjective To investigate the effects of pulmonary rehabilitation (PR) and combined nutritional support therapy on quality of life (QoL) and functional status in patients with chronic obstructive pulmonary disease (COPD). Methods This pre-and post-intervention prospective exploratory study involved 64 patients with stable stage three to four COPD. Oral nutritional support and personalized diet were combined with an intense and comprehensive PR program. Baseline and 8-week follow-up scores were compared for the 6-minute walk test (6MWT), incremental shuttle walking test (ISWT), St. George's Respiratory Questionnaire (SGRQ), pulmonary function tests (PFT), PImax-PEmax, arterial blood gas (ABG), respiratory rate (RR), handgrip strength, Borg and modified Medical Research Council dyspnoea scale scores and fat-free mass index. Results Significant improvements were found in functional status (6MWT: 86.72 m, ISWT: 76.24 m), QoL (SGRQ total: 13.86), PFT, ABG, RR, dyspnoea, upper extremity muscle strength and hand-body composition. Conclusion Nutritional support with comprehensive and intensive PR can significantly improve physical performance, QoL, dyspnoea and body composition in COPD. The improvement in QoL was greater than that reported in previous studies. Because two modalities were combined in this study, future randomized controlled studies are needed to confirm the extent and contribution of these modalities to the outcomes.Öğe Effects of oral/enteral nutrition alone versus plus pantoprazole on gastrointestinal bleeding in critically ill patients with low risk factor: a multicenter, randomized controlled trial(Tubitak Scientific & Technological Research Council Turkey, 2020) Gundogan, Kursat; Karakoc, Emre; Teke, Turgut; Zerman, Avsar; Esmaoglu, Aliye; Temel, Sahin; Guven, MuhammetBackground/aim: Critically ill patients are at risk of developing gastrointestinal (GI) bleeding due to stress causing mucosal damage. Aim of the study was to determine the effect of oral/enteral nutrition with or without concomitant pantoprazole on upper GI bleeding in low risk critically ill patients. Materials and methods: This was a prospective, randomized, open-label, multicenter study conducted with intensive care unit (ICU) patients receiving oral/enteral nutritional support. Patients were randomly assigned into two groups including intervention group (received oral/EN plus pantoprazole) and control group (received only oral/EN). Results: A total of 300 patients (intervention group: 152, control group: 148) participated in the study. Overall, 226 (75%) patients were fed by orally and 74(25%) patients fed by enteral tube feeding. Median duration of nutritional support 4 (range: 2-33) days. Overt upper GI bleeding was noted only in one patient (0.65%) who was in the intervention group. The overall length of ICU stay of 4 (2-105) days, while ICU stay was significantly longer in the intervention group than in the control group (P = 0.006). Conclusions: Our findings seems to indicate that in patients who are at low risk for GI bleeding and under oral/enteral nutritional support, the use of PPIs may not reduce the risk of bleeding, however these results are imprecise because of low event (GI bleeding) rate and limited power.Öğe Gluteal region muscle metastasis from squamous cell lung carcinoma(Modestum Ltd, 2017) Korkmaz, Celalettin; Yolacan, Ramazan; Teke, Turgut; Yavsan, Durdu MehmetLung cancer is known as a disease leading to metastases to all types of organs. Although covering a large part of the body and of much blood building-up, skeletal muscles are rare metastatic areas. In autopsies, less than 1% of malignant cancers that are spread hematogenously is known to lead to metastasis. A 51-year-old male patient diagnosed with squamous cell lung cancer and known to be in regression via screening after the administration of 32-day curative radiotheraphy and 6-cycle chemotherapy, the patient was radiologically followed-up. Four months later, he was admitted to outpatient clinic with the complaint of left hip pain. Magnetic resonance imaging revealed a lesion of nodular, necrotic metastatic mass within left gluteus muscles. As a result of tru-cut biopsy performed for the solid lesion, metastasis of carcinoma was detected. As also in our case, metastatic muscle disease should be suspected in patients with muscle pain.Öğe Konya Selçuk Üniversitesi öğrencileri arasında astım ve astım semptom prevalansı(2005) Teke, Turgut; Özer, FarukBu çalışma, Selçuk Üniversitesi öğrencilerinde, astım ve astım benzeri semptomların yaygınlığının ve astım gelişimini etkileyen risk faktörlerinin belirlenmesi amacıyla bir anket çalışması olarak gerçekleştirilmiştir. Çalışma, 2003-2004 eğitim-öğretim yılında Konya kent merkezindeki fakülte ve 4 yıllık yüksekokulların 1, sınıfında öğrenim gören 8110 öğrencinin 4504'üne 2003 Kasım- Aralık aylarında, ECRHS protokolüne dayanılarak hazırlanan yazılı anket uygulaması ile yapıldı. Çalışmaya alınan 4433 öğrencinin %52.4'ü erkek, %47.6'sı kız ve genel yaş ortalaması 19.2±1.5 idi. Çalışmada: current astım %1.6, kümülatif astım %2.7, astım benzeri semptom %38 olarak bulunmuştur. Son 12 aydaki vizing, vizing ile birlikte nefes darlığı ve nezle-grip olmadan olan vizing prevaiansiarı sırasıyla %20.5, %9.5, %7.8 olarak bulunurken gece uykudan göğüste sıkışma hissi, nefes darlığı, öksürük atağı ile uyanma oranları ise sırasıyla %1 1.1, %8.5 ve %19 olarak bulunmuştur. Son 12 ayda astım atağı geçirme ve halen astım İçin tedavi kullanma oranları ise %1.3 ve %0.8 bulunmuştur. Allerjik rinit oranı ise %1 1 bulunmuştur. Bu çalışmada, cinsiyet, allerjik rinit, ailesel atopi, yaşanılan bölge ve sigara gibi risk faktörlerinin astım prevalansı üzerine etkileri de araştırılmıştır. Önceki çalışmalarla uyumlu olarak ailesel atopi, allerjik rinit ve sigara faktörlerinin astım gelişiminde risk faktörleri olarak rol oynayabileceği belirlenmiştir. Cinsiyetin ise astım gelişimine fazla etkisinin olmadığı tespit edilmiştir. Sonuç olarak çalışmamız, Türkiye'de erişkin populasyonda bildirilen oranlarla karşılaştırıldığında üniversite öğrencilerinde astım sıklığının daha düşük olduğunu göstermiştir. Öte yandan bu çalışmada saptanan bulguların ülkemiz üniversite öğrencilerinde yapılan diğer çalışmalarda bildirilen verilerle uyumlu olduğu gözlenmiştir.Öğe Myasthenia gravis with acute respiratory failure in the emergency department(Kare Publ, 2016) Kozak, Hasan Huseyin; Uca, Ali Ulvi; Teke, Turgut; Altas, Mustafa; Karatas, EmineAcute respiratory failure (ARF) is defined as a sudden malfunction in the ability of respiratory system to maintain adequate gas exchange. Acute hypercapnic respiratory failure develops as a result of ventilation deficiency and it is defined as an increase of PaCO2 above 45 mmHg. Myasthenia Gravis (MG) is a sporadically developing auto- immune deficiency where the neuro-muscular transmission is affected and it is one of the important reasons for neurologically-induced respiratory distress. Here, we report a case of a 75-year-old male patient previously undiagnosed MG, who presented with ARF. MG is not a common entity that we encounter daily. Patients on occasions may present to the emergency department because of acute exacerbation. Though most of them were known cases, we should be aware of some unrecognized cases and should consider MG as a differential diagnosis for patients with acute respiratory failure. Production and hosting by Elsevier B.V. on behalf of the Owner.Öğe New Biomarkers Used in the Diagnosis of Tuberculosis-Related Pleural Effusions(Georg Thieme Verlag Kg, 2020) Teke, TurgutTuberculosis-related pleural effusion (TPE) is reported in 12 to 38% of thoracic tuberculosis (TB) cases in the pediatric population. In TPE, the pleural fluid bacilli load is very low, generally resulting in negative acid-fast bacill (AFB) staining and Mycobacterium culture. In the pleural fluid, AFB stain positivity is reported in <20%, and Mycobacterium tuberculosis positive culture in 18 to 38%. In childhood, this ratio is even lower. Also, pleural effusion (PE) mycobacterial culture gives late results (2-8 weeks). Therefore, TPE is diagnosed with pleura biopsy and pleural liquid examination. However, pleura biopsy is a more invasive operation and the diagnosis rates vary. The clinical and laboratory findings are not typical; it is very hard to distinguish between TPE and another cause of exudative PE. This situation makes it essential for research on reliable new biomarkers that can provide fast and accurate diagnosis of TPE. Adenosine deaminase (ADA) activity being above 40 to 50 U/L (despite varying depending on age) strongly suggests TPE. Other new biomarkers featured in TPE diagnosis are interferon-gamma, cytokines, and nucleic acid multiplication tests. However, the results of ADA and other potential biomarkers shown to be beneficial in TPE diagnosis should always be interpreted with clinical and microbiological findings because no biomarker can provide information about M. tuberculosis culture and drug resistance. Especially in countries with high resistance against TB drugs, performing pleura biopsy, culture and drug resistance tests are important with regard to diagnosis and therapy planning.Öğe Noninvasive Ventilation for Severe Acute Asthmatic Attacks(2015) Teke, Turgut; Yavşan, Mehmet; Uzun, KürşatAstım, bronşial düz kas kontraksiyonu, hava yolu inflamasyonu ve sekresyonlarda artış sonucu oluşan reverzibıl hava yolu obstrüksiyonu ile karakterize kronik bir hastalıktır. Astım hastalarının çoğunda ataklar medikal tedavi ile kolayca kontrol altına alınabilir. Ancak çok küçük bir hasta grubunda astım atakları ölümcül olabilmektedir. Bu hasta grubunda şiddetli astım atakları standart tedaviye dirençlidir ve bu hastaların bir kısmında mekanik ventilasyon tedavisine ihtiyaç olmaktadır. Astımlı hastalar invaziv mekanik ventilasyonun olağan komplikasyonlarına ilave olarak başka komplikasyonlar için de risk altındadırlar. Bu hastalarda yeterli ventilasyonu sağlamak yüksek inspiratuar basınçlarda dahi zor olabilmekte ve sıklıkla hasta-ventilator asenkronisi gelişmektedir. Noninvaziv ventilasyon (NIV) son 20 yıldır akut solunum yetmezliği tedavisinde giderek artan bir şekilde kullanılmaktadır. Günümüzde NIV'in KOAH alevlenmesi ve akut akciğer ödeminin standart ilk seçenek tedavisi olduğu kabul edilmektedir. Bilgi ve tecrübelerdeki artış NIV'in daha önce kontrendike olduğu düşünülen astımda da kullanılabileceği konusunu gündeme getirmiştir. Bu derlemede şiddetli akut astım ataklarında NIV'nin kullanımı ile ilgili güncel bilgileri gözden geçirilmesi amaçlanmıştır. Günümüzde şiddetli astım ataklarında NIV kullanımını tavsiye etmek için yeterli veriler olmamakla birlikte bazı ilginç ve ümit verici sonuçlar vardır. Sonuç olarak çelişkileri ortadan kaldırabilmek için respiratuar asidozu ve hiperkapnisi olan hastaların dâhil edildiği iyi dizayn edilmiş yeni çalışmalara ihtiyaç vardır.Öğe The presence of Torque teno virus in chronic obstructive pulmonary disease(E-Century Publishing Corp, 2014) Feyzioglu, Bahadir; Teke, Turgut; Ozdemir, Mehmet; Karaibrahimoglu, Adnan; Dogan, Metin; Yavsan, MehmetTorque Teno Virus (TTV) has been identified as transfusion-transmitted virus in humans, initially. Although TTV viremia is extremely common in the general population worldwide, there is no direct causal evidence linking TTV infection to specific clinical manifestations. Our hypothesis was that TTV might play a role in Chronic obstructive pulmonary disease (COPD) by inducing inflammatory mechanisms previously identified. The study was conducted on 57 COPD patients and 39 healthy control groups. COPD patient groups included: the patients (n:20) with exacerbation needed noninvasive ventilation, the patients (n: 19) who received only medical treatment, and the invited patients (n: 18) for outpatient control. Serum samples were collected from patients and voluntary blood donors. TTV DNA quantification was carried out with a real time PCR by the hybridization probe system and viral load was interpreted through the crossing point value. TTV DNA was detected in the majority of both patients and healthy control groups. The prevalence was 94.4% (17/18) in patients for outpatient control, 94.7% (18/19) in patients who received only medical treatment, 100% (20/20) in patients with exacerbation needed noninvasive ventilation and 84.6% (33/39) in healthy controls. This difference was not statistically significant. However, CP values was statistically different in all the patient groups from the control group. TTV DNA prevalence was higher in patients than healthy individuals. More interesting thing, viral load was highest in the patients with exacerbation needed noninvasive ventilation. As a result, TTV may be associated with COPD and the severity of it.Öğe The role of torque teno virus (TTV) viremia in sarcoidosis etiology(Kuwait Medical Assoc, 2019) Teke, Turgut; Feyzioglu, Bahadir; Tosun, Mustafa; Dogan, Metin; Yesildag, Kerim; Ozdemir, MehmetObjective: To investigate a possible role of torque teno virus (TTV) in the etiology of sarcoidosis Design: Controlled prospective study Setting: Faculty of Meram Medicine, Necmettin Erbakan University, Konya, Turkey Subjects: This cross-sectional, clinical study included 37 sarcoidosis patients and 9 healthy control patients. Intervention: TTV DNA quantification was performed by real-time PCR by the hybridization probe system, and viral load was interpreted by means of the crossing point (CP) value in sera of subjects. Main outcome measures: The prevalence and viral load of TTV Results: TTV DNA was detected in the majority of both patients and healthy controls. The prevalence was 59.5% in patients and 55.6% in controls. This difference was not statistically significant. However, CP values were statistically different in the patient group from the control group (p < 0.001); namely, the viral load of TTV was higher in the patient group than in the healthy control group. Conclusions: Overall, these findings demonstrate for the first time that TTV plays a role in the etiology of sarcoidosis. The purpose of this study was toanalyze the impact of cancer symptoms on insomnia and the prevalence of sleep-related problems reported by the patients with lung cancer in Turkey. Materials and Methods: Assesment of Palliative Care in Lung Cancer in Turkey (ASPECT) study, a prospective multicenter study conducted in Turkey with the participation of 26 centers and included all patients with lung cancer, was re-evaluated in terms of sleep problems, insomnia and possible association with the cancer symptoms. Demographic characteristics of patients and information about disease were recorded for each patient by physicians via face-to-face interviews, and using hospital records. Patients who have difficulty initiating or maintaining sleep (DIMS) is associated with daytime sleepiness/fatigue were diagnosed as having insomnia. Daytime sleepiness, fatigue and lung cancer symptoms were recorded and graded using the Edmonton Symptom Assessment Scale. Results: Among 1245 cases, 48.4% reported DIMS, 60.8% reported daytime sleepiness and 82.1% reported fatigue. The prevalence of insomnia was 44.7%. Female gender, patients with stage 3-4 disease, patients with metastases, with comorbidities, and with weight loss 5 kg had higher rates of insomnia. Also, patients with insomnia had significantly higher rates of pain, nausea, dyspnea, and anxiety. Multivariate logistic regression analysis showed that patients with moderate to severe pain and dyspnea and severe anxiety had 2-3 times higher rates of insomnia. Conclusion: In conclusion, our results showed a clear association between sleep disturbances and cancer symptoms.
