Öğe Anxiety, Mood, and Personality Disorders in Patients with Benign Paroxysmal Positional Vertigo(2018) Kozak, Hasan Hüseyin; Dündar, Mehmet Akif; Uca, Ali Ulvi; Uğuz, Faruk; Turgut, Keziban; Altaş, Mustafa; Aziz, Suhayb Kuria; Tekin, GoncaIntroduction: This study presents the current prevalence of anxiety,mood, and personality disorders as well as factors associated with theexistence of psychiatric disorders in patients with benign paroxysmalpositional vertigo (BPPV).Methods: The study sample comprised 46 patients with BPPV and 74control subjects. Anxiety and mood disorders were ascertained via theStructured Clinical Interview for the Diagnostic and Statistical Manual(DSM) of Mental Disorders, Fourth Edition/Clinical Version. Personalitydisorders were diagnosed via the Structured Clinical Interview for DSM,Revised Third Edition, Personality Disorders.Results: Of the 46 patients, 18 (39.1%) had at least one mood or anxietydisorder and 13 (28.3%) had at least one personality disorder. The mostcommon Axis I and Axis II disorders in the patient group were majordepression in 8 (17.4%) and obsessive–compulsive personality disorderin 10 (21.7%) patients, respectively. Yeterli araştırma olmamasına rağmen, şimdiye kadar elde edilen veriler, obsesif-kompulsif belirtiler ve uyku şikayetlerinin şiddeti arasındaki ilişkiyi desteklemektedir. Bu çalışmada, obsesif-kompulsif bozukluk (OKB) olan hastalarda seçici serotonin geri alım inhibitörleri (SSRIlar) ne adjuvan tedavi olarak eklenen parlak ışık tedavisinin etkinliği (BRT) araştırıldı. Yöntem: Necmettin Erbakan Üniversitesi Psikiyatri Kliniği'ne OKB ile başvuran 49 gönüllü hasta çalışmaya dahil edildi. Obsesif-kompulsif belirtiler, tedavi öncesi ve tedavinin 10. ve 30. gününde Padua Envanteri-Revize (PE-R) ile değerlendirildi. Padua Envanteri-Revize skoruna göre ile randomizasyon ile iki gruba ayrılan hastalardan bir gruba yalnız SSRI verilirken diğer gruba SSRI tedavisine ek olarak parlak ışık tedavisi uygulandı. Uzunlamasına latent sınıf analizi kullanarak, hastalar tedaviye yanıt veren ve tedaviye dirençli hastalar olmak üzere iki gruba ayrıldı. Bulgular: PE-R puanları temelinde, 34 hasta (% 69.4) tedaviye yanıt veren ve 15 hasta (% 30.1) tedaviye dirençli grupta sınıflandırıldı. Tedaviye yanıt veren grupta depresif belirtiler, uyku yakınmaları ve obsesif inançlar üzerinde önemli bir iyileşme olduğu gözlendi. Sonuç: SSRI tedavisine adjuvan olarak eklenen parlak ışık tedavisinin etkinliğinin değerlendirilmesi için daha büyük örneklemli çalışmalara ihtiyaç vardır.Öğe Personality disorders in pateints with multiple sclerosis: Prevalence and association with depressive and anxiety disorders and clinical features(Asean Neurological Assoc, 2016) Uca, Ali Ulvi; Uguz, Faruk; Kozak, Hasan Huseyin; Turgut, Keziban; Tekin, Gonca; Altas, Mustafa; Akpinar, ZehraObjective: This study examines the current prevalence of mood, anxiety and personality disorders in patients with multiple sclerosis (MS), the impact of personality disorders on clinical features of MS and the existence of depressive and anxiety disorders. Methods: The study sample comprised of 55 patients with relapsing-remitting MS and 56 control subjects. Axis I diagnoses including mood and anxiety disorders and personality disorders were ascertained by means of the Structured Clinical Interview for the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition / Clinical Version and the Structured Clinical Interview for DSM, Revised Third Edition Personality Disorders, respectively. The Expanded Disability Status Scale (EDSS) was used to determine degree of disability due to MS. Results: The prevalence of any mood, any anxiety and any personality disorders in patients with MS were 40.0%, 38.2% and 45.5%, respectively, which are significantly higher than the control subjects. Major depression, generalized anxiety disorder, avoidant personality disorder and obsessive-compulsive personality disorder were more frequent in patients with MS compared to the controls. In comparison to MS patients without personality disorders, MS patients with personality disorders had a more frequent occurence of any mood disorder, any anxiety disorder, major depression, posttraumatic stress disorders and MS attacks, and higher scoresof EDSS. Conclusions: Mood, anxiety and personality disorders are frequently seen in patients with MS. The results also suggest that personality disorders may negatively affect the existence of mood and anxiety disorders as well as the clinical course of MS in the patients.
