Preoperatif hasta anksiyete düzeyleri ile sağlık okuryazarlığı arasındaki ilişkinin değerlendirilmesi

Özet
Giriş ve Amaç: Anksiyete, bireyin kendini güvende hissetmediği durumlara karşı geliştirdiği
doğal bir tepkidir. Yaş, cinsiyet, medeni durum ve eğitim gibi bazı sosyodemografik özellikler
anksiyete ile ilişkili faktörlerdir. Çalışmada, sağlık okuryazarlığının preoperatif anksiyete ve
eğitim düzeyi arasında saptanan farklı sonuçlarda etkili olabileceği düşüncesinden yola çıkarak,
preoperatif dönemde hasta sağlık okuryazarlığı ile anksiyete düzeyleri ve eğitim düzeyleri
arasında ilişki olup olmadığı, sekonder olarak da anksiyete ve sağlık okuryazarlığı ile
demografik ve temel sağlık durumu ile ilgili verilerin etkileşiminin araştırılmasını amaçlandı.
Gereç ve Yöntem: Anket çalışması, Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi
Hastanesi Kulak Burun Boğaz Bölümü’nde Eylül-Kasım 2019 tarihleri arasında elektif
operasyon planlanan, ASA I-II grubu, 18-65 yaş arası 155 gönüllü hasta üzerinde prospektif
olarak gerçekleştirildi. Operasyondan bir gün önce, hastalara Durumluk Kaygı Ölçeği (STAI-D)
ve Sağlık Okuryazarlığı Ölçeği (SOYÖ) uygulanarak sosyodemografik ve temel sağlık durumu
ile ilgili veriler kaydedildi.
Bulgular: Hastaların cinsiyet, medeni durum, meslek, sigara kullanma, ASA (American
Society of Anesthesiology), geçirilmiş cerrahi ve eğitim durumuna göre STAI-D puan
ortalamalarında istatistiksel açıdan anlamlı bir fark bulunmadı (p>0,05). Eğitim durumlarına
göre SOYÖ ile bilgiye erişim, bilgiyi anlama ve değer biçme alt boyutu puan ortalamalarında
istatistiksel açıdan ileri derecede anlamlı bir fark olduğu saptanırken (p<0,01), uygulama alt
boyutu puan ortalamasında anlamlı fark tespit edilmedi (p>0,05). STAI-D ölçeği ile SOYÖ
(r=-0,424) ve uygulama alt boyutu (r=-0,482) puan ortalamaları arasında negatif yönde orta
düzeyli bir ilişki ve STAI-D ölçeği ile bilgiye erişim (r=-0,335), bilgiyi anlama (r=-0,368) ve
değer biçme (r=-0,353) alt boyutları arasında negatif yönde düşük düzeyli bir ilişki olduğu tespit
edildi.
Sonuç: Hastalardaki preoperatif anksiyete düzeylerindeki artışta düşük sağlık okuryazarlığı
seviyesinin etkili olabileceği bu konu ile ilgili daha ileri çalışmaların yapılması gerektiği
kanaatine varıldı.
Introduction and purpose: Anxiety is a natural reaction that an individual develops to situations where he or she is not feeling safe. Some sociodemographic characteristics such as age, gender, marital status and education are factors associated with anxiety. Based on the idea that health literacy may be effective in different results between preoperative anxiety and education level, it was aimed to investigate whether there is a relationship between patient health literacy and anxiety levels and education levels in the preoperative period, and secondaryly the interaction of data related to anxiety and health literacy and demographics and basic health status. Materials and Methods: The survey study was carried out prospectively on 155 volunteer patients in the ASA I-II group, aged 18-65, who were scheduled for elective operation between September and November 2019 in the Necmettin Erbakan University Meram Medical Faculty Hospital Ear Nose and Throat Department. One day before the operation, the State Anxiety Scale (STAI-D) and Health Literacy Index were administered to the patients, and data about sociodemographic and basic health status were recorded. Results: There was no statistically significant difference in the STAI-D mean scores according to the gender, marital status, occupation, smoking, ASA, previous surgery and education level of the patients (p> 0.05). While a statistically significant difference was found in the mean scores of the Health Literacy Index, the sub-dimension of access, understand and appraise according to educational status (p <0.01), no significant difference was found in the mean score of the sub-dimension of apply (p> 0.05). There is a moderate negative correlation between the STAI-D scale and the mean scores of the Health Literacy Index (r = -0.424) and the sub-dimension of apply (r = -0.482) and also low-lewel negative correlation between the STAI-D scale and the sub-dimension of access (r = -0.335), understand (r = -0.368) and appraise (r = -0.353). Conclusion: It was concluded that the low level of health literacy might be effective in the increased preoperative anxiety levels in patients, and further studies is required on this subject.
