Akne vulgaris hastalarında 577-NM PRO yellow lazer uygulamasının post-akne eritem üzerine etkinliğinin araştırılması

Özet
Amaç
Akne vulgaris en sık adolesanlarda ve genç erişkinlerde görülen pilosebase ünitenin
kronik inflamatuar hastalığıdır. Skar, postinflamatuar hiperpigmentasyon ve post-akne
eritem (PAE) gibi sekellere yol açarak bireylerde psikososyal problemlere neden
olabilmektedir. PAE tedavisi için literatürde sınırlı sayıda tedavi seçenekleri tanımlanmış
olup standart bir tedavi protokolü yoktur. Bu çalışmada; akne vulgaris hastalarında 577-nm
pro yellow lazer uygulamasının post-akne eritem üzerine etkinliğinin araştırılması
amaçlanmıştır.
Gereç ve Yöntem
Klinik olarak akne vulgaris ve post-akne eritem tanısı konulan, hali hazırda akne
tedavisi almayan, 14-40 yaş aralığında 50 hasta çalışmaya dahil edildi. Ancak COVID-19
pandemisi nedeniyle 14 hasta takiplerine gelemediği için çalışma dışında bırakıldı ve
çalışma 36 hasta ile tamamlandı. Hastaların rastgele seçilen yüz yarısına, tarama başlığı ile
(20-24 J/cm², 44 ms, %80) ayda bir olmak üzere toplam üç seans 577-nm pro yellow lazer
(QuadroStarPRO YELLOW® Asclepion Lazer Teknolojileri, Almanya) uygulandı. Diğer
yüz yarısı kontrol olarak kabul edildi. Tedavi öncesi global akne skorlama sistemine göre
her iki yüz yarısının akne şiddet skoru belirlendi. Hastalar son seanstan 1 ay sonrasına kadar
takip edildi. Hastaların başlangıçta ve her seanstan 1 ay sonra standart dijital fotoğrafları
çekildi. İki tarafsız dermatolog tarafından fotoğraflar karşılaştırılarak ve klinisyen eritem
değerlendirme ölçeği kullanılarak eritem değerlendirildi. Hastalar tarafından hastanın tedavi
sonucunu değerlendirme ölçeği kullanılarak değerlendirme yapıldı. Bu iki değerlendirmeye
göre tedavi etkinliği belirlendi. Ayrıca tedavi bitiminde GASS göre hastaların her iki yüz
yarısındaki akne skorlarındaki değişim de değerlendirildi.

Bulgular
Çalışmaya katılan 36 hastanın 27’si kadın (%75) ve 9’u erkek (%25) idi. Hastaların
yaşları 15-38 yıl arasında değişmekte olup ortalama 22.08±5.16 yıldı. 9 hastanın (% 25)
Fitzpatrick cilt tipi II iken, 27 hastanın (% 75) Fitzpatrick cilt tipi III idi. Başlangıca göre her
seanstan bir ay sonra yapılan değerlendirmede; lazer uygulanan yüz yarısında kontrol
tarafına kıyasla, eritemde istatistiksel anlamlı daha büyük değişim saptanmıştır (p<0.001).
Hastaların tedavi sonunda yapmış oldukları değerlendirmede istatistiksel anlamlı sonuç elde
edilmiştir (p<0.001). Başlangıca göre tedavi sonunda yapılan kontrolde, GASS göre akne
skorlarında gözlenen değişim bakımından lazer uygulanan yüz yarısı ile kontrol tarafı
arasında istatistiksel olarak anlamlı fark saptanmamıştır (p=0.264).
Sonuç
Çalışmamız 577-nm pro yellow lazerin post-akne eritemde etkili ve minimun yan
etki profili ile güvenli olduğunu göstermektedir.
Objective Acne vulgaris is a chronic inflammatory disease of the pilosebaceous unit, most commonly seen in adolescents and young adults. It can induce psychosocial problems in individuals by causing sequelae such as scarring, postinflammatory hyperpigmentation and post-acne erythema. A limited number of treatment options have been described in the literature for the treatment of PAE and there is no standard treatment protocol. It is aimed to investigate the effectiveness of 577-nm pro yellow laser on post-acne erythema in patients with acne vulgaris. Materials and Methods Fifty patients between the ages of 14-40 who were clinically diagnosed with acne vulgaris and post-acne erythema and who were not currently receiving acne treatment were included in the study. However, 14 patients were excluded from the study, because they couldn’t come to follow-up due to the COVID-19 pandemic so the study was completed with 36 patients. Each subject's face sides were randomly assigned to receive 577-nm pro yellow laser treatment (QuadroStarPRO YELLOW®, Asclepion Laser Technologies, Germany) using the scanner handpiece (20-24 J/cm², 44 ms, %80) for 3 sessions at 1 month intervals. Nontreated sides of the face was accepted as control. The acne severity scores of both side of face were determined according global acne grading system before the treatment. Patients were followed up to 1 month after the last session. Standard digital photographs of the patients were taken at the baseline and 1 month after each session. Erythema was assessed by two independent dermatologists by comparing the photos and using clinician erythema assessment scale. Assessment was made by the patients using the patient's treatment outcome assessment scale. Treatment efficacy was determined based on these two assessments. In addition, the change in acne severity scores on both side of the vii patients face according to the global acne grading system was also assessed at the end of the treatment. Results Of the 36 patients participating in the study, 27 were female (75%) and 9 were male (25%). The ages of the patients ranged from 15 to 38 years, with a mean of 22.08 ± 5.16 years. 9 patients (25%) have Fitzpatrick skin type II while 27 patients (75%) have Fitzpatrick skin type III. In the assessments made one month after each session compared to the baseline; a statistically significant greater change in erythema was found in the treated side of face compared to the control side (p<0.001). Statistically significant result was obtained assessment made by the patients at the end of the treatment (p<0.001). There was no statistically significant difference between the treated side of the face and the control side in terms of the change observed in the acne scores according to global acne grading system at the control performed at the end of the treatment according to the baseline (p=0.264). Conclusion Our study shows that 577-nm pro yellow laser is effective in post-acne erythema and safe with a minimal side effect profile.
