Özet
Amaç;Bu çalışmada meme cerrahisinde etkinliği kanıtlanmış erektör spina düzlem
bloğunun farklı volümlerde, aynı konsantrasyonda lokal anestezik ile uygulanması
durumunda intraoperatif opioid tüketimi ve postoperatif analjezi üzerindeki etkinliği
araştırıldı.
Yöntem; Çalışmaya 18-70 yaş arası ASA I-III, elektif meme cerrahisi geçirecek hastalar
dahil edildi. Hastalar iki gruba randomize edildi. Grup I’deki 35 hastaya % 0.375lik
bupivakain hidroklorür 20 ml ile Grup II’deki 35 hastaya % 0.375 lik bupivakain
hidroklorür 30 ml ile ultrasonografi eşliğinde unilateral erektör spina düzlem bloğu yapıldı.
Ameliyat süresince cerrahi pletismografik indeks ile hastaların analjezik ihtiyacı
monitörize edildi. SPI 30-50 arasında olacak şekilde remifentanil infüzyon dozu % 10
oranında değiştirilerek ayarlandı. Hastaların postoperatif 10.dakika, 1.saat,6.saat,12.saat ve
24.saatteki NRS değerleri kaydedildi. NRS 4 ve üstü değerde ise yetersiz analjezi olarak
kabul edildi ve kurtarıcı analjezik uygulandı. Postoperatif ilk 24 saatteki toplam opioid
tüketimi ve toplam kurtarıcı analjezik ihtiyacı kaydedildi.
Bulgular; Hemintraoperatif hem de postoperatif opioid tüketimi gruplar arasında
benzerdi(p>0,05). Tutulan dermatom sayıları arasında istatistiksel olarak anlamlı fark
tespit edildi (p<0,05). Postoperatif NRS skorları arasında istatistiksel fark saptanmadı
(p>0,05). Postoperatif bulantı kusma ve kurtarıcı analjezik ihtiyacı açısından da anlamlı
fark tespit edilmedi (p>0,05).
Sonuç;Elektif meme cerrahisinde aynı konsantrasyonda farklı volümlerde uygulanan
erektörspina düzlem bloğu opiod tüketimi ve postoperatif analjezi üzerinde fark
oluşturmamaktadır.
Aim; In this study, the effectiveness of the proven efficacy of the erector spinae plane block in breast surgery was investigated on intraoperative opioid consumption and postoperative analgesia if applied with local anesthetic in the same concentration in different volumes. Method; In this study included patients with age ranged 18-70, American Society of Anesthesiologist physical status I-III, and scheduled for elective breast surgery. Usg guided erector spina plane block 35 patients in Group I were performed unilateral with % 0.375 bupivacaine hydrochloride 20 ml and % 0.375 bupivacaine hydrochloride 30 ml to 35 patients in Group II. During the operation, the analgesic needs of the patients were monitored with surgical pletismographic index. The dose of remifentanil infusion was adjusted to be between 30-50 SPI bychanging it by 10%. Numerical Rating Scale(NRS) score of patients in postoperative 10 minute and 1. ,6. , 12. and 24 th hour were recorded. NRS 4 and above were considered inadequate analgesia and rescue analgesics were applied. Total opioid consumption and total rescue analgesic requirements were recorded in the first 24 hours of postoperative. Results; Both intraoperative and postoperative opioid consumption were similar in two groups(p>0.05). Statistically significant difference was found between dermatomal spread(p<0.05). No statistical difference was found between postoperative NRS scores(p>0.05). No significant difference was detected in terms of postopative nausea vomiting and the rescue analgesics requirements(p>0.05). Conclusions; In elective breast surgery, erector spinae plane block applied in different volumes at the same concentration does not make a difference on opiod consumption and postoperative analgesia.
