Pediatrik olgularda katarakt cerrahisi sonuçlarının değerlendirilmesi

Özet
Bu çalışmada pediatrik katarakt cerrahisi geçiren olguların dosya kayıtları incelenerek aksiyel göz uzunluğu (AU), görme keskinliği (GK), refraksiyon (REF), göz içi basıncı (GİB), ambliyopi ve cerrahi komplikasyonlarının incelenmesi amaçlandı.Çalışma grubu 32'si kız, 55'i erkek toplam 87 olgudan oluşturuldu. Olgular 3 gruba ayrıldı; grup 1 tek taraflı konjenital katarakt, grup 2 çift taraflı konjenital katarakt ve grup 3 travmatik katarakt. Olguların dosyaları incelenerek preoperatif ve postoperatif aksiyel uzunlukları, görme keskinlikleri ve refraksiyonları gruplar arasında karşılaştırıldı.Gruplar içinde olguların operasyon öncesi ve sonrası AU'na bakıldığında 3 grupta da anlamlı ve doğru orantılı bir artış gözlendi. Aksiyel uzunlukları opere olan gözlerde postoperative gruplar arasında karşılaştırıldığında genel olarak anlamlı fark saptandı. Grup 1 ve 2'de opere olan gözde AU farkı preoperatif ve postoperatif olarak karşılaştırıldığında istatistiksel olarak anlamlı bulunmadı (p=0.28). Grup 1 ve grup 3 içinde opere olan ve olmayan gözlerde preoperatif ve postoperatif AU'u değerleri karşılaştırıldığında opere olan gözde postoperatif değerleri hariç anlamlı fark saptandı. AU'ları farkına bakıldığında anlamlı bir fark saptanmadı. Görme keskinliğinde grup 1 ve 2' de karşılaştırıldığında anlamlı bir fark saptandı (p=0.00). Grup 1 ve grup 3'te GK karşılaştırıldığında anlamlı bir fark saptanmadı (p=0.1). Refraksiyon sonuçlarında gruplar arasında preoperatif ve postoperatif bakıldığında anlamlı bir fark saptanmadı(p=0.14).Sonuç olarak bu çalışmada opere olan gözlerde miyopik kayma izlenmektedir, fakat bu kaymanın operasyon sonrası fakik göz ile karşılaştırıldığında artmadığı saptandı. Görme keskinlikleri yaşlarına ve operasyon yaşlarına göre değişmektedir. GK'leri grup 2 ve 3'te daha iyi düzeyde olduğu tespit edildi. Bu olgularda glokom sık görüldüğü için hayat boyu GİB'ı belli aralıklarla takip edilmelidir.Anahtar kelimeler: Konjenital katarakt, travmatik katarakt, aksiyel uzunluk, ambliyopi, afaki
In this study, evaluating the results of axial eye length (AL), visual acuity (VA), refraction (REF), intraocular pressure (IOP), amblyopia and surgery complications was aimed through examining the file records of cases that had pediatric cataract operation.The working group was composed of 32 girls, 55 boys total 87 patients. The cases were divided into 3 groups; Group 1: unilateral congenital cataract, Group 2: bilateral congenital cataract and Group 3: traumatic cataract. The cases files were examined and preoperative and postoperative axial lengths, visual acuity and refractions were compared among the groups.The preoperative and postoperative AL of patients was examined among the groups; a statistically significant and directly proportional increase was observed. Postoperative AL differences in the operated eyes were compared among the groups generally a statistically significant difference was determined. The AL difference of operated eyes in group 1 and 2 compared as preoperative and postoperative and it was not found statistically significant (p=0.28). Preoperative and postoperative AL values of group 1 and grup 3 were compared in the operated eyes significant differences were determined except postoperative values. The AL difference was evaluated where no significant difference was determined. The VA was compared between group 1 and 2 where a statistically significant difference was found (p=0.00). The VA was compared between group 1 and group 3 the difference found was not significant (p=0.1). Preoperative and postoperative refraction results were evaluated among the groups no statistically significant difference was found (p=0.14).Consequently, in this study, myopic shift is observed at the operated eyes; but, it was determined that this shifting didn?t increase when it was compared with the phakic eye. Visual acuity varies according to age and operation age. Visual acuity was determined in better level at Group 2 and 3. The patients must be observed during their lives and IOP must be checked.As a result, in this study, eyes operated on showed a myopic shift but this shift was not seemed to be increased postoperatively compared to the phakic eye. The visual acuity results changed according to the age & operation age. The VA in group 2 and group 3 showed better results. These cases should be controlled periodically for IOP as glaucoma is often seen.Key words: Congenital cataract, traumatic cataract, axial length, amblyopia, aphakia
