| dc.description.abstract | Sistemik Lupus Eritematozus Tanılı Hastalarda Akut Faz Reaktanlarının Değerlendirilmesi, Dr. Cihad SOLAK, Uzmanlık Tezi, Konya, 2019 Amaç: Bu çalışmada, SLE tanısı almış kronik inflamasyonu bulunan hastalarda akut faz reaktanlarının ve serum protein elektroforezinin değerlendirilmesi ile bunların kendi arasındaki ilişkisinin araştırılması amaçlanmıştır. Yöntem: Çalışmaya toplam 85 SLE tanılı hasta dahil edildi. Çalışmada, tedavi değişikliği yapılmadan alınmış kan örneklerinden çalışılmış ESH, CRP, fibrinojen, serum protein elektroforezi değerleri kullanıldı. Hastalar APR değerlerine göre normal ve yüksek şeklinde ikişerli gruplara ayrılarak karşılaştırıldı. Bulgular: Hastaların yaş ortalaması 40.1±13.5 (18-69) olarak hesaplandı. Hastaların 79'u (%92.9) kadın, 6'sı erkekti (%7.1). Hastaların 30'unda (%35.3) ESH, 10'unda (%11.8) CRP ve 23'ünde (%27.1) fibrinojen değeri ve 9 (%10.6) hastada beta bandı yüzdesi yüksek bulundu. 57 (%67.1) hastada ise poliklonal gamopati tespit edildi. ESH yüksek olan grupta, serum CRP ve fibrinojen değerleri ile gama bandı yüzdesi, ESH normal olan gruptan anlamlı düzeyde yüksek bulundu (sırasıyla, p=0.055, p=0.019 ve p=0.003). Ki kare testi ile hasta oranlarına bakıldığında ise ESH yüksek gruba yalnızca fibrinojen yüksekliğinin eşlik ettiği görüldü (p<0.001). Poliklonal gamopatisi olan grupta ESH değeri, poliklonal gamopatisi olmayan gruptan anlamlı düzeyde olmasa da yüksek bulundu (p=0.134). CRP ve fibrinojen değerleri açısından gruplar arasında anlamlı fark yoktu (sırasıyla, p=0.750 ve p=0.801). Sonuç: SLE tanılı hastalarımızda ESH, CRP ve fibrinojen değerleri yüksek veya normal olarak tespit edildi ve karşılaştırıldı. Poliklonal gamopati, en yüksek oranda tespit edilen anormal bulgu idi. Poliklonal gamopatinin, ESH yüksekliğine katkı sağlıyor olabileceği düşünüldü. CRP ve ESH değerlerinin korele olmadığı gözlendi. ESH ve fibrinojen değerleri arasında anlamlı ilişki saptandı. Poliklonal gamopati varlığının, SLE tanı ve takibinde değerli bilgiler verebileceği düşünüldü. | dc.description.abstract | The Evaluation of Acute Phase Reactants in Patients with Systemic Lupus Erythematosus, Dr. Cihad SOLAK, Specialty Thesis, Konya, 2019 Aim: The aim of this study was to evaluate the acute phase reactants and serum protein electrophoresis and to investigate the relationship between them in patients with chronic inflammation diagnosed with SLE. Methods: A total of 85 patients with SLE were included in the study. ESR, CRP, fibrinogen and serum protein electrophoresis values which were studied from blood samples taken without treatment change were used in the study. The patients were divided into two groups as normal and elevated according to APR values. Results: The mean age of the patients was 40.1±13.5 (18-69) years. Of the patients, 79 (92.9%) were female and 6 (7.1%) were male. ESR level was detected elevated in 30 patients (35.3%), CRP in 10 (11.8%), fibrinogen in 23 (27.1%) and beta band percentage in 9 (10.6%) patients. Polyclonal gammopathy was detected in 57 (67.1%) patients. In the group with elevated ESR, serum CRP and fibrinogen levels and gamma band percentage were significantly higher than the group with normal ESR (p=0.055, p=0.019 ve p=0.003, respectively). When we evaluated the percentages of patients among ESR groups with Chi-Square test, the percentage of patients with elevated fibrinogen was significantly differed (p<0.001). Although not significantly, ESR value was higher in the group with polyclonal gammopathy than in the group without polyclonal gammopathy (p=0.134). There was no significant difference between the groups in terms of CRP and fibrinogen levels (p==0.750 ve p=0.801, respectively). Conclusion: In patients with SLE, ESR, CRP and fibrinogen levels were found to be high or normal and they were compared. Polyclonal gammopathy was the most common abnormal finding. It was considered that polyclonal gammopathy may contribute to ESR elevation. CRP and ESR values were not correlated. There was a significant relationship between ESR and fibrinogen levels. The presence of polyclonal gammopathy was considered to provide valuable information in the diagnosis and follow-up of SLE.
