Özet
Amaç: Bu çalışmada inme sonrası fonksiyonel durumun depresyon, anksiyete ve D tipi kişilik
ile ilişkisinin değerlendirilmesi amaçlandı.
Gereç ve Yöntem: Çalışmaya Necmettin Erbakan Üniversitesi Tıp Fakültesi Fiziksel Tıp ve
Rehabilitasyon polikliniğine Mayıs 2022-Mayıs 2023 tarihleri arası müracaat eden inme tanısı
ile gelen hastalar dahil edildi. İnme tanısı almış hastaların fonksiyonel durumları; depresyon,
anksiyete ve kişilik özellikleri ile değerlendirildi. Bilgilendirilmiş onam formu alındı.
Hastalara Barthel Günlük Yaşam Aktiviteleri (GYA) İndeksi, Fonksiyonel Bağımsızlık
Ölçeği (FBÖ), Hastane Anksiyete ve Depresyon Ölçeği (HADS), Beck Depresyon Ölçeği, D
Tipi Kişilik Ölçeği (D14) uygulanarak veriler kaydedildi. Demografik bilgiler (cinsiyet, yaş,
eğitim düzeyi, çalışma durumu, medeni hal), lezyon lokalizasyonu, inme üzerinden geçen
zaman, dominant ekstremite, komorbidite varlığı hasta değerlendirme formuna kaydedildi.
Bulgular: Hastaların inme öncesine göre inme sonrası psikiyatrik ilaç kullanımı istatistiksel
olarak anlamlı saptandı. Beck Depresyon Ölçeği ve HADS skorları ile FBÖ ve Barthel GYA
İndeksi skorları inme üzerinden geçen süreyle korele olmadığı sonucuna varıldı. Kadınların
Beck Depresyon Ölçeği, HADS-Anksiyete ve HADS-Depresyon skorları erkeklere göre
istatistiksel olarak anlamlı yüksek saptandı. Kadınlarda D tipi kişilik özellikleri erkeklere göre
istatistiksel olarak fazla görüldü (p=0,005). Beck Depresyon Ölçeği, HADS skorları, D tipi
kişilik varlığı, FBÖ ve Barthel GYA İndeksi skorları ile lezyon lokalizasyonu arasında
anlamlı bir bulgu saptanmadı. Çalışan hastalarda FBÖ ve Barthel GYA İndeksi skorları ev
hanımı ve emeklilere göre istatistiksel olarak anlamlı derecede yüksek saptandı (p=0,025)
HADS-Depresyon skoru da diğer gruplara göre anlamlı derecede düşük bulundu (p=0,013).
Beck Depresyon Ölçeği, HADS-Anksiyete, HADS-Depresyon ve D tipi kişilik özellikleri ile
fonksiyonel durum ölçekleri olan FBÖ ve Barthel GYA İndeksi skorları arasında negatif
yönde korelasyon saptandı.
Sonuç: Çalışmamız sonucunda inmeli hastalarda fonksiyonel durum ve iyileşme sürecinde
depresyon ve anksiyetenin etkisi olduğu ortaya kondu. D tipi kişilik özellikleri taşıyan inmeli
hastaların fonksiyonel durumlarının daha kötü olduğu sonucuna varıldı.
Objective: The aim of this study was to evaluate the relationship between functional status and depression, anxiety, and type D personality in patients assessed after stroke. Materials and Methods: The study included patients who applied to the Department of Physical Medicine and Rehabilitation at Necmettin Erbakan University Faculty of Medicine between May 2022 and May 2023, with a diagnosis of stroke. The functional status of patients diagnosed with stroke was evaluated in terms of depression, anxiety, and personality traits. Informed consent forms were obtained. The Barthel Index of Activities of Daily Living, Functional Independence Measure, Hospital Anxiety and Depression Scale, Beck Depression Inventory, and Type D Personality Scale (D14) were administered to the patients, and the data were recorded. Demographic information (gender, age, education level, employment status, marital status), lesion localization, time elapsed since stroke, dominant extremity, and the presence of comorbidities were recorded on the patient evaluation form. Results: Statistically significant psychiatric medication usage was observed in stroke patients post-stroke compared to pre-stroke. It was concluded that the scores of Beck Depression Inventory, Hospital Anxiety and Depression Scale (HADS), Functional Independence Measure (FIM), and Barthel Index were not correlated with the time elapsed since the stroke. Females had significantly higher scores on Beck Depression Inventory, HADS-Anxiety, and HADS-Depression compared to males. Statistically, Type D personality traits were found to be more prevalent in females compared to males (p=0,005) There was no significant association found between scores of Beck Depression Inventory, HADS, D-type personality, FIM, Barthel Index, and lesion localization. In employee patients, the scores of FBÖ and Barthel GYA Index were found to be statistically significantly higher compared to housewives and retirees (p=0,025). The HADS-Depression score was also significantly lower compared to the other groups (p=0,013). Negative correlations were found between scores of Beck Depression Inventory, HADS-Anxiety, HADS-Depression, D-type personality traits, and functional status measures (FIM, Barthel Index). Conclusions: Our study revealed th It was concluded that stroke patients with Type D personality traits had worse functional status. e impact of depression and anxiety on the functional status and recovery process of stroke patients.
