dc.description.abstractİnme; vasküler neden dışında görünürde başka bir neden olmadan, aniden yerleşip, fokal (veya global) serebral disfonksiyona yol açan, 24 saat veya daha uzun sürebildiği gibi ölümle de sonuçlanabilen klinik bir durumdur. Matriks metalloproteinaz (MMP) ailesi, ekstrasellüler proteinazların önemli bir üyesidir. En önemli görevleri ekstrasellüler matriksin (ECM) yıkımıdır.MMP'lerin aktivitelerindeki kontrolsüz artışların ECM degradasyonu yoluyla akut ve kronik hastalıkların patogenezinde rol oynadığı düşünülmektedir. Bu çalışmanın amacı, matriks metalloproteinaz (MMP) ailesinde yer alan MMP2 nin serebral enfarkt ve serabral hemorajide tanı ve prognoz ile olan ilişkisini ortaya koymaktır. Yöntem:Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Hastanesi Acil Servisine başvuran 18 yaş üstü, travma hastası olmayan ve çalışmaya katılmayı kabul eden, laboratuvar ve radyolojik tetkikler ile serebral enfarkt ve serebral hemoraji tanısı alan toplam 119 kişilik hasta ve kontrol grubunu içermektedir. Kontrol grubunu, belirtilen yaş aralığında olan, acil servise baş ağrısı ile başvurmuş, ayırıcı tanıda düşünülmesine rağmen serebral enfarkt ve hemoraji dışlanan hastalar oluşturmuştur.Hastalara ait demografik özellikler, çeşitli laboratuvar tetkik sonuçları ve MMP2 ölçüm değerlerinin inme tiplerine göre karşılaştırmaları yapılmıştır. Çalışmanın analizleri SPSS paket programında yapılmış olup grup karşılaştırmaları ve değişkenler arasındaki ilişkiler için uygun analiz yöntemleri kullanılmıştır. Analizlerde p<0.05 değeri istatistiksel olarak anlamlı kabul edilmiştir. Bulgular:Hastalardan ilk anda alınan MMP2 değerleri gruplar arasında farklı bulunmuştur. Ayrıca morbidite, semptom ve motor semptomların gruplar ile ilişkili olduğu görülmüştür. ROC analizi sonucunda ilk anda ve 3 saat sonra yapılan MMP2 ölçümlerinin tanı koymada yeterli olmadığı anlaşılmıştır. Sonuç: MMP2 ölçüm değerleri, SVO tanısı konmasında, hem iskemi hem de hemorajide anlamlı bulundu. Ancak iskemi ve hemoraji ayrımında anlamlı bulunmadı. Ayrıca prognoz göstergesi olarakta kullanılabileceği ile ilgili bir işarete rastlanmamıştır. Ancak semptom ve ağrı durumlarında gruplar arasında farklılık göstermesi vaka sayısının daha yüksek olduğu durumlarda diagnoz göstergesi olabileceğini düşündürmektedir. dc.description.abstractAccording to the world health organisation, stroke is a clinical state of isolated vascular origin, with sudden onset, leading to focal or global cerebral dysfunction that persists beyond 24 hours or is interrupted by death. The matrix metalloproteinase (MMP) family is an important member of the extracellular proteinases. It's most important function is destruction of extracellular matrix (ECM).Uncontrolled increase of the activity of mmps play role in the pathogenesis of acute and chronic diseases via ECM degradation. In this study, it is aimed that the diagnostic and prognostic potential of MMP2 in acute ischemic stroke and hemorrhage by means of neurological examination and MRI/CT. Methods:This study includes 119 cases containing control and patient groupswho admitted to Necmettin Erbakan University,Emergency Department of Meram Medical Faculty Hospital. They agreed to participate in the study, and over 18 years old, non-traumatic, cerebral infarction and cerebral hemorrhage diagnosed by radiological examinations and laboratory findings. Control group includes the patients indicated the age range, applied to the emergency department with headache excluding hemorrhage or cerebral infarction. The demographical, laboratorial and MMP2 results were compared between the stroke groups. All analyses were performed by SPSS software applying suitable comparison and relation analysis methods. p<0.05 was considered as statistically significant. Results: There was a significant difference between the groups for first MMP2 measures. Moreover, mobidity, symptoms and motor symptoms were related with groups. ROC analyses of first and after 3 hours MMP2 measurements were found insufficient to diagnose. Conclusions:There was no sign showing that MMP2 values indicate the diagnosis and prognosis of cerebral infarction and hemorrhage, but it is sufficient to diagnose between healthy individuls and CVE patients. However, it can be concluded that MMP2 can indicate the diagnosis of cerebral infarction and hemorrhage in case of the sample size is high since MMP2 values were different between the symptom and pain groups.
