Özet
Akut lenfoblastik lösemili(ALL) hastalarda kullanılan antikanser tedaviler pek çok
potansiyel yan etkilere sahiptir ve bunlardan önemli biri de nefrotoksititedir. Geçtiğimiz
yıllarda nefrotoksititenin tespiti için yeni belirteçler tanımlanmıştır ve Kıdney Injury
Molecule-1(KIM-1) bunlardan biridir. KIM-1, renal tübüler hasar sonrası idrara geçen bir
tip 1 transmebran glikoproteindir. Biz bu çalışmada, ALL hastalarında idrar KIM-1’in
böbrek hasarını göstermede invaziv olmayan bir belirteç olarak kullanılabilirliğini saptamayı
hedefledik.
Çalışmaya Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Çocuk
Hematolojisi ve Onkolojisi Bilimdalı’nda son 5 yıl içerisinde ALL tanısı alan hastalar dahil
edildi. Kontrol grubu yaşları uyumlu çocuklardan oluşmaktaydı. Hasta ve kontrol
gruplarının her ikisinde de herhangi bir enfeksiyon veya üriner sistem anomalisi
bulunmamaktaydı. İdrar KIM-1 değerlerini tespit etmek için hasta ve kontrol grubunun idrar
örnekleri toplandı. İdrar KIM-1 değerleri ELİSA kiti ile ölçüldü ve ardından standardizasyon
için idrar kreatinin değerine oranlandı.
Çalışmaya ALL tanısı olan 50 hasta dahil edildi. Kontrol grubu 48 hastadan
oluşmaktaydı. Ortanca idrar KIM-1 değeri hasta grubunda (31,76 ng/g-kreatinin) kontrol
grubuna (16,08 ng/g-kreatinin) göre istatistiksel olarak daha yüksek tespit edildi.
Çalışmamız esnasında nefrotoksik antibiyotik (amfoterisin b, amikasin, asiklovir, kolistin)
alan hastalarımızı da analiz ettik, nefrotoksik antibiyotik alımı ve idrar KIM-1 değerleri
arasında istatistiksel anlamlı bir ilişki saptanmadı. Aynı zamanda idrar örneklerinin
toplanmasından önceki bir ay içerisinde veya herhangi bir zamanda nefrotoksik antibiyotik
tedavisi alımı açısından karşılaştırdık ve idrar KIM-1 değeri açısından istatiksel olarak

anlamlı bir fark saptanmadı. Çalışma süresi boyunca hastalarımızın hiçbirinde böbrek
yetmezliği gelişmedi.
Hastalarımızda uzun dönem izlemde, belirgin hematüri veya proteinüri olmamasına
rağmen idrar KIM-1 değerlerinin yüksek olması, idrar KIM-1’in renal hasarı erken dönemde
gösterebilen, güvenilir, noninvaziv ve gelecek vaat eden bir böbrek hasarı belirteci
olabileceğini göstermektedir.
Anticancer treatment in children with acute lymphoblastic leukemia (ALL) has several potential adverse effects and nephrotoxicity is an important matter among these effects. In the last years, new biomarkers have been identified for detecting nephrotoxicity such as urine Kidney Injury Molecule-1 (KIM-1). KIM-1 is a type 1 transmembrane glycoprotein which is shed into urine after the renal tubular injury. We aimed to determine urine KIM-1 can be used as a noninvasive biomarker of kidney ınjury in acute lymphoblastic leukemia. Patients who were diagnosed with ALL in the last five years were included in the study. The control group was comprised of age-matched children. Both patients and the control group didn’t have any infection or urinary system anomalies. Urine samples were collected for determination of urine KIM-1 levels. Urine KIM-1 levels were measured with ELISA kit, subsequently adjusted with urine creatinine for standardization. 50 patients who diagnosed with ALL were included in the study. The Control group was consisted of 48 children. Median urine KIM-1 was found to be statistically higher in the patient group (31,76 ng/g creatinine) compared to the control group (16,08 ng/g creatinine). We analyze the patients who had administered nephrotoxic antibiotics ( amphotericin B, amikacin, acyclovir, colistin) there is no correlation between the administration of antibiotics and urine KIM-1 levels. We also compare the patients who had administered nephrotoxic antibiotics whether at any other time or for a one-month duration before collecting the urine samples and there is no significance statistically for urine KIM-1 levels. Renal failure did not develop in any of our patients during the research period. vii Despite the fact that our patients had no hematuria or proteinuria, urine KIM-1 levels determined to be high indicate that urine KIM-1 might be a potentially reliable, noninvasive, and promising determinant of kidney injury.
