Özet
IGF' 1er GH' un periferik etkilerine aracılık eden polipeptid yapıda proteinlerdir. IGF' 1er özellikle hücre proliferasyonunda ve farklılaşmasında GH' a aracılık ederler. Malignitede ki etkilerini araştırmak amacıyla S.Ü.T.F. Göğüs ustalıkları ABD' ye müracaat eden ve akciğer kanseri, tüberküloz plörezi, nonspesifik plörezi tanısı alan toplam 60 hasta çalışmaya dahil edildi. Akciğer kanserli grupta 20 (6K,14E), tüberküloz plörezi20(10K,10E), nonspesifik plörezili grupta ise 20(10K,10E) hasta bulunmaktaydı. Akciğer kanserli hasta grubunda serum IGF-I, IGF-II, IGFBP-I, IGFBP-II, insülin, C-peptid ve GH değerleri sırasıyla 46,88 ±21,57 ng/ml, 456,04 ± 79,36 ng/ml, 39,74 ± 11,68 ng/ml, 40,21 ± 10,55 ng/ml, 15,92 ± 6,02 ulU/ml, 3,75 ± 1,91 ng/ml, 0,6 ± 0,35 ng/ml, tüberküloz plörezili grupta serum IGF-I, IGF-II, IGFBP-I, IGFBP-II, insülin, C-peptid ve GH değerleri sırasıyla 80,67 ± 35,82 ng/ml, 388,72 ± 124,65 ng/ml, 21,70 ± 1 1,52 ng/ml, 33,10 ± 15,99 ng/ml, 1 1,31 ± 3,77 ulU/ml, 6,17 ± 2,28 ng/ml, 0,97 ± 0,55 ng/ml, nonspesifik plörezili grupta serum IGFI, IGF-II, IGFBP-I, IGFBP-II, insülin, C- peptid ve GH değerleri ise sırasıyla 37,15 ± 15,34 ng/ml, 167,30 ± 55,89 ng/ml, 35,96 ± 13,93 ng/ml, 35,19 ± 10,09 ng/ml, 5,09 ± 2,76 ulU/ml, 5,21 ± 1,98 ng/ml, 0,92 ± 0,60 ng/ml olarak bulundu. Akciğer kanserli hasta grubunda plevral sıvıda IGFI, IGF-II, IGFBP-I, IGFBP-II, insülin, C-peptid ve GH değerleri sırasıyla 70,51 ± 12,29 ng/ml, 240,79 ± 44,45 ng/ml, 8,74 ± 2,59 ng/ml, 33,01 ± 8,95 ng/ml, 6,65 ± 2,54 ulU/ml, 5,79 ± 2,84 ng/ml, 0,51 ± 0,47 ng/ml, tüberküloz plörezili grubun plevral sıvıda ki IGFI, IGF-II, IGFBP-I, IGFBP-II, insülin, C-peptid ve GH değerleri sırasıyla 98,36 ± 36,66 ng/ml, 192,47 ± 45,59 ng/ml, 36,80 ± 14,76 ng/ml, 20,24 ± 6,02 ng/ml, 6,32 ± 2,12 ulU/ml, 6,23 ± 1,80 ng/ml, 0,82 ± 0,51 ng/ml, nonspesifik plörezili grupta plevral sıvı IGFI, IGF-II, IGFBP-I, IGFBP-II, insülin, C-peptid ve GH değerleri ise sırasıyla 33,39 ± 9,29 ng/ml, 66,42 ± 18,86 ng/ml, 44,59 ± 13,82 ng/ml, 8,06 ± 2,44 ng/ml, 7,13 ± 2,08 ulU/ml, 5,94 ± 2,67 ng/ml, 0,46 ± 0,29 ng/ml olarak bulundu. 53IGF-I serum ve plevra sıvısı değerleri malign grupta diğer gruplara göre anlamlı şekilde düşük bulundu. Serum IGFBP-I ve IGFBP-II malign grupta yüksek iken plevral sıvıda IGFBP-I düşük IGFBP-II ise yüksek bulundu. Malign grupta serum IGF-II seviyesindeki yükseklik istatistiksel açıdan anlamlı değilken plevral sıvıdaki IGF-II düzeylerinin artışı istatistiksel açıdan anlamlı bulundu(p<0,01).
LEVELS OF INSULIN LIKE GROWTH FACTORS AND BINDING PROTEINS IN LUNG DISEASE IGF' s are polipeptides which mediate peripheric effects of GH. They mediate especially, effects of GH on cell proliferation and differentiation. In order to determine their effect in malignancy, totally 60 patients with lung cancer, tuberculous pleuritis and nonspesific pleural effusion applied to department of chest disease of medical school of university of Selçuk were investigated. Of the patients, 20(6F,14M) were with lung cancer, 20(10F,10M) with tuberculous pleuritis and 20(10F,10M) with nonspesific pleural effusion. Serum IGF I, IGF II, IGFBP I, IGFBP H, insulin, C-peptide and GH levels of the cancer patients were found as 46,88 ± 21,57 ng/ml, 456,04 ± 79,36 ng/ml, 39,74 ± 11,68 ng/ml, 40,21 ± 10,55 ng/ml, 15,92 ± 6,02 uIU/ml, 3,75 ± 1,91 ng/ml and 0,6 ± 0,35 ng/ml respectively. The same parameters of the patients with tuberculous pleuritis were 80,67 ± 35,82 ng/ml, 388,72 ± 124,65 ng/ml, 21,70 ± 11,52 ng/ml, 33,10 ± 15,99 ng/ml, 11,31 ± 3,77 uIU/ml, 6,17 ± 2,28 ng/ml and 0,97 ± 0,55 ng/ml and those of the patients with nonspesific pleural effusions were 37,15 ± 15,34 ng/ml, 167,30 ± 55,89 ng/ml, 35,96 ± 13,93 ng/ml, 35,19 ± 10,09 ng/ml, 5,09 ± 2,76 uIU/ml, 5,21 ± 1,98 ng/ml and 0,92 ± 0,60 ng/ml respectively. Pleural fluid levels of IGF I, IGF H, IGFBP I, IGFBP IL insulin, C-peptide and GH of the cancer patients were found as 70,51 ± 12,29 ng/ml, 240,79 ± 44,45 ng/ml, 8,74 ± 2,59 ng/ml, 33,01 ± 8,95 ng/ml, 6,65 ± 2,54 uIU/ml, 5,79 ± 2,84 ng/ml and 0,51 ± 0,47 ng/ml respectively. Serum levels of IGFBP I and IGFBP II and pleural fluid IGFBP II levels of the cancer patients were higher where as pleural fluid IGFBP I level was lower. Also, pleural fluid IGF II level of the cancer patients was significantly higher while there was no difference between serum IGF II levels of the groups. There were no differences between the other parameters of the patients. In conclusion, binding proteins mediating effects of IGF' s were found to be increased, in malignancy and were different in pleural fluid than those of the other groups. IGF II, which is spesifically effective in malignancy was obviously higher in pleural fluid than that in serum.
