Özet

Acil servisler, acil sağlık hizmetlerinin kesintisiz sağlanması amacıyla 7/24 çalışan özelleşmiş hizmet alanlarıdır. Acil servisten hasta akışının sağlanması için hızlı ve doğru tercihlerin yapılması gereken kritik karar noktaları vardır. Acil servislerde klinisyenlerin kullanımına sunulacak yapay zekâ tabanlı karar destek sistemlerinin geliştirilmesi problemi günümüzün en popüler araştırma konularından biridir. Bu modellerin geliştirilmesinde demografik veriler, vital bulgular, tıbbi özgeçmiş bilgileri gibi pek çok değişken kullanılmaktadır. Günümüzde beklenen yaşam süresinin artması ile birlikte acil servislere başvuran geriatrik hasta sayısı da hızla artmaktadır. Geriatrik hastalar acil servis hastaları içinde çoklu komorbidite, azalmış fizyolojik cevaplar, bilişsel ve fonksiyonel kayıplar, sosyal destek eksikliği gibi faktörler nedeniyle özellikli bir gruptur. Bu nedenlerle, tez çalışmamızda geriatrik acil servis hastalarının acil servisten hastaneye yatış sınıflandırma problemine, hemen her acil serviste kolay ve hızlı elde edilebilen değişkenler kullanılarak farklı yapay zekâ modelleri ile çözüm aranmıştır. Retrospektif, gözlemsel, tek merkezli olan tez çalışmamızın hasta popülasyonunu acil servise başvuran 3872 geriatrik hasta oluşturmuştur. Oluşturulan nihai veri setinde, bir hastanın acil servisten hastaneye yatışını öngörmek için 4 ana başlık altında 60 farklı özelliği içeren bir sınıflandırma problemi bulunmaktadır. Nihai veri seti kullanılarak LR, KNN, SMV, RF, AdaBoost, XGB ve MLP modelleri geliştirilmiştir. En yüksek makro ortalama F1 skorları LR, SVM, AdaBoost ve MLP modellerinde elde edilmiştir. En düşük makro ortalama F1 skoru ise KNN modelinde hesaplanmıştır. En düşük duyarlılık değerine sahip modelin KNN (%30) ve en yüksek duyarlılık değerine sahip modellerin LR ile MLP (%63) olduğu tespit edilmiştir. En düşük özgüllük değerine sahip modelin XGB (%76,2) ve en yüksek özgüllük değerine sahip modelin KNN (%91) olduğu tespit edilmiştir. SHAP analizlerinde ilk 20 içinde yer alan ortak özellikler oksijen satürasyonu, Glasgow koma skalası, bilinç değişikliği, dizartri/afazi, fokal nörolojik defisit ve nefes darlığıdır. Geriatrik hasta grubunda hastaneye yatış kararının karmaşıklığı göz önüne alındığında, farklı metotlar ile model performansının artırılması acil hasta bakım kalitesini artıracaktır.

Emergency departments (ED) are specialized service areas that operate 7/24 to ensure the uninterrupted provision of emergency healthcare services. There are critical decision points where quick and accurate choices must be made to ensure patient flow through the ED. Developing AI-based decision support systems for clinicians in EDs is one of today's most popular research topics. Many variables, such as demographic data, vital signs, and medical history information, are used in developing these models. With the increase in life expectancy today, the number of geriatric patients visiting EDs is also rapidly increasing. Geriatric patients are a distinct group among ED patients due to factors such as multiple comorbidities, decreased physiological responses, cognitive and functional losses and lack of social support. For these reasons, our thesis study sought solutions to the problem of classifying geriatric ED patients for hospital admission using different artificial intelligence models based on variables that can be easily and quickly obtained in almost any ED. Our thesis study, which was retrospective, observational, and single-center, included a patient population of 3,872 geriatric patients who visited the ED. In the final dataset created, there is a classification problem that includes 60 different features under 4 main headings to predict a patient's admission to the hospital from the ED. Using the final dataset, LR, KNN, SMV, RF, AdaBoost, XGB, and MLP models were developed. The highest macro-average F1 scores were obtained with the LR, SVM, AdaBoost, and MLP models. The lowest macro-average F1 score was calculated with the KNN model. It was determined that the model with the lowest sensitivity value was KNN (30%) and the models with the highest sensitivity values were LR and MLP (63%). It was determined that the model with the lowest specificity value was XGB (76.2%) and the model with the highest specificity value was KNN (91%). Characteristics among the top 20 in SHAP analyses were oxygen saturation, Glasgow coma scale, altered consciousness, dysarthria/aphasia, focal neurological deficit, and shortness of breath. Considering the complexity of the hospital admission decision in the geriatric patient group, improving model performance using different methods will enhance the quality of emergency patient care.
