Özet
Uyku yoksunluğu yoğun bakım hastalarında sık görülen ve iyileşmeyi etkileyen problemlerden bir tanesidir. Bu nedenle uyku yoksunluğuna sebep olan faktörlerin etkilerini azaltmak iyileşmenin hızlanmasında büyük öneme sahiptir. Bu çalışma, yoğun bakım hastalarında kitap dinlemenin uyku kalitesi ve yaşam bulguları üzerine etkisini incelemek amacıyla yapılmıştır. Çalışmada ön test son test desende yarı deneysel bir tasarım kullanılmıştır. Çalışma Konya'da bir Devlet Hastanesi Dahiliye Yoğun Bakım Ünitesinde Ocak-Haziran 2022 tarihleri arasında gerçekleştirilmiştir. 28 kontrol, 28 müdahale olmak üzere toplam 56 hasta örneklem büyüklüğünü oluşturmuştur. Veriler Tanımlayıcı Özellikler Formu, Uyku Değerlendirme Formu ve Richard Campbell Uyku Ölçeği (RCUÖ) kullanılarak toplanmıştır. Çalışmada ilk gün müdahale ve kontrol gurubuna standart bakım ve rutin tedaviler uygulanmıştır. İkinci gün müdahale grubuna hikâye dinletilmiş kontrol grubuna ise standart bakım devam ettirilmiştir. Çalışmanın etik kurul izni, kurum izni hastaların onamları alınmıştır. Veriler SPSS 25.0 paket programı kullanılarak tanımlayıcı özellikler (ortalama, standart sapma, sayı ve yüzdelik dilim) şeklinde verilmiştir. Ayrıca homojenlik Levene, normallik varsayımları Shapiro-Wilk testi ile incelenmiştir. Verilerin analizinde, Ki Kare testi, Mauchly testi, Mann Whitney U testi, Sphericity Assumed testi, Huynh-Feldt testi, Greenhouse Geisser testi, mixed design varyans analizi ve Bonferroni- Dunn testi kullanılmıştır ve anlamlılık düzeyi p<0,05 kabul edilmiştir. Çalışmada kontrol grubu yaş ortalaması 60,18+/-12,23, müdahale grubu yaş ortalaması 58,96+/-11,54 olarak bulunmuştur. İki grubunda demografik özellik, hastalık bilgisi ve uyku durumu benzer dağılım gösterdiği görülmektedir (p>0,05). RCUÖ skorları incelendiğinde kontrol grubunun ön test puan ortalaması 19,61+/-7,34, son test puan ortalaması 15,36+/-7,87 olduğu, müdahale grubunun ölçek puan ortalamasının ise ön test 18,04+/-7,21, son test, 53,04+/-13,42 olduğu belirlenmiştir. Yaşam bulguları zaman grup etkileşiminde nabız ve kan basıncında anlamlılık bulunmuş olup ateş ve solunum sayısında ise değişiklik gözlenmemiştir. Grup içi değerlendirmede ise müdahale grubunun son test sonuçları ön test sonuçlarından daha düşük olduğu saptanmıştır. Sonuç olarak, yoğun bakım hastalarında kitap dinlemenin uyku kalitesini arttırdığı ve yaşam parametrelerinde olumlu sonuçlar oluşturduğu tespit edilmiştir. Sonuçlar doğrultusunda, yoğun bakımda kitap dinleme faaliyetlerinin arttırılması, hastalara uyku saatlerinde dinleyebilecekleri çeşitli hikâye seçenekleri sunularak hastanın bakıma dahil edilmesi, bu tür müdahalelerin hemşire eğitimleri ile desteklenmesi önerilmektedir.
Sleep deprivation is one of the most common problems in intensive care patients and affects recovery. Therefore, reducing the effects of factors that cause sleep deprivation is of great importance in accelerating recovery. This study was conducted to examine the effect of listening to a book on sleep quality and vital signs in the intensive care unit. In the study, a quasi-experimental design was used in the pre-test post-test design. The study was carried out in a State Hospital Internal Medicine Intensive Care Unit in Konya between January and June 2022. A total of 56 patients, 28 controls and 28 interventions, constituted the sample size. Data were collected using the Descriptive Characteristics Form, the Sleep Evaluation Form, and the Richard Campbell Sleep Scale (RCSQ). Standard care and routine treatments were applied to the intervention and control group on the first day of the study. On the second day, the intervention group listened to the story, and the control group continued standard care. Ethics committee permission and institutional permission of the study were obtained from the patients. Data were given as descriptive features (mean, standard deviation, number and percentile) using the SPSS 25.0 package program. In addition, homogeneity was examined by Levene and normality assumptions were examined by Shapiro-Wilk test. In the analysis of the data, Chi-Square test, Mauchly test, Mann Whitney U test, Sphericity Assumed test, Huynh-Feldt test, Greenhouse Geisser test, mixed design analysis of variance and Bonferroni-Dunn test were used and the significance level was accepted as p<0.05. In the study, the mean age of the control group was 60.18+/-12.23, and the mean age of the intervention group was 58.96+/-11.54. Demographic characteristics, disease information and sleep status showed similar distribution in the two groups (p>0.05). When the RCSQ scores were examined, the pretest mean score of the control group was 19.61+/-7.34, the posttest mean score was 15.36+/-7.87, and the scale score average of the intervention group was 18.04+/-7.21 in the pretest, posttest, 53 It was determined to be .04+/-13.42. Significance was found in pulse and blood pressure in the interaction of vital signs and time groups, and no change was observed in fever and respiratory rate. In the within-group evaluation, the post-test results of the intervention group were found to be lower than the pre-test results. As a result, it has been determined that listening to books in intensive care patients improves sleep quality and creates positive results in life parameters. In line with the results, it is recommended to increase the activities of listening to books in the intensive care unit, to include the patient in the care by offering various story options that they can listen to during bedtime, and to support such interventions with nurse trainings.
