dc.description.abstractDiz osteoartiti gibi sorunlar nedeniyle hasarlanmış eklem yüzeylerinin yeniden yapılandırıldığı bir tedavi yöntemi olan total diz protezi (TDP), yaygın uygulanan bir cerrahi girişimdir. TDP sonrası yaşam kalitesini azaltan ve yaşamı tehdit eden fiziksel ve psikolojik komplikasyonlar gelişebilmektedir. Bu komplikasyonların önlenmesinde, hasta beklentileri ve gereksinimleri doğrultusunda planlanmış hemşirelik bakımının verilmesi ve bakım memnuniyetinin değerlendirilmesi önemlidir. Bu bağlamda bu araştırmada, TDP uygulanan hastaların hemşirelik bakımından beklentilerini ve memnuniyet düzeylerini belirlemek amaçlandı. Tanımlayıcı tipteki bu araştırma, Temmuz 2017- Mart 2018 tarihleri arasında TDP uygulanan 120 hasta ile gerçekleştirildi. Araştırma verileri, literatür doğrultusunda oluşturulan veri toplama formu ve Newcastle Hemşirelik Bakımından Memnuniyet Ölçeği (NHBMÖ) ile taburculuk öncesi yüz yüze görüşme yöntemi ile toplandı. Hastaların yaş ortalaması 64.7±9.79 yıl olup, çoğunluğu (%83.3) kadındı ve %75'i okur-yazar değildi. Hastaların hemşirelerden en yüksek ilk üç beklentisinin sırasıyla"ameliyat sonrası ağrı olduğunda ağrı kesicinin hemen yapılması (%100)", "ameliyat sonrası mobilizasyon ve yemek yeme zamanı konusunda bilgi verilmesi (%99.2)" ve "hemşirenin güler yüzlü olması (%98.3)" olduğu belirlendi. En düşük ilk üç beklentinin sırasıyla "varis çorabı hakkında bilgi verilmesi (%2.5)", "derin solunum-öksürük egzersizlerinin öğretilmesi (%11.7)" ve "kan temini konusunda bilgi verilmesi (%15.8)" olduğu bulundu. Hastaların hemşirelik bakımından beklentilerinin karşılanma oranının %2.8-95.8 aralığında değiştiği saptandı. Tedavi ve bakım sırasında sorun yaşayan hastaların ve kadın hastaların hemşirelik bakımından beklentilerinin anlamlı olarak daha yüksek olduğu belirlendi (p<0.05). Hastaların NHBMÖ puan ortalaması 76.22±25.41 olarak bulundu. Yaş ve cinsiyet gibi tanıtıcı özellikler ile daha önce hastaneye yatma ve yatış süresi gibi özelliklerin hemşirelik bakımından memnuniyeti etkilemediği belirlendi (p<0.05). Hastaların hemşirelik bakımından beklentiler puanı arttıkça hemşirelik bakımından memnuniyet puanlarının anlamlı oranda azaldığı (p<0.05), beklentilerin karşılanma durumu puanı arttıkça memnuniyet puanlarının anlamlı oranda arttığı (p<0.05) saptandı. Sonuç olarak araştırmada, TDP'li hastaların hemşirelik bakımından beklenti puanlarının yüksek olduğu(77.19±13.87), beklentilerin karşılanma durumu puanının düşük olduğu (47.03±26.73), bakım beklentisi ve beklentilerin karşılanma durumunun hemşirelik bakımından memnuniyet puanını etkilediği saptandı.en_US
dc.description.abstractTotal knee arthroplasty (TKA), a treatment modality in which joint surfaces degenerated due to problems such as osteoarthritis are reconstructed, is a widely used surgical procedure. Physical and psychological complications that may decrease the quality of life and threaten life may develop after TKA. In the prevention of these complications, it is important to offer planned nursing care and to evaluate the satisfaction with care based on patient expectations and requirements. In this context, the aim of this study was to identify the nursing care expectations and satisfaction levels of patients who underwent TKA. This descriptive study was conducted with 120 patients who underwent TKA between July 2017 and March 2018. Data were collected using a data collection form designed based on literature review and the Newcastle Satisfaction with Nursing Scale (NSNS) administered through face-to-face interviews before discharge. The mean age of the patients was 64.7±9.79 years, the majority (83.3%) were women and 75% were illiterate. The highest three expectations of the patients from the nurses were "receiving immediate pain relief in case of postoperative pain (100%)", "being informed about postoperative mobilization and eating time (99.2%)" and "nurses being kind (98.3%)", respectively. The lowest three expectations were "being informed about compression stockings (2.5%)", "being trained about deep breathing-cough exercises (11.7%)" and "being informed about blood supply (15.8%)", respectively. The rate of fulfilment of the nursing care expectations was found to vary between 2.8% and 95.8%. It was determined that patients who had problems during treatment and care and the female patients had significantly higher expectations of nursing care (p<0.05). The mean NSNS score of the patients was 76.22±25.41. It was determined that descriptive characteristics such as age and gender, history and duration of previous hospitalization did not affect satisfaction with nursing (p<0.05). As the patients' score of nursing care expectations increased, their satisfaction scores with nursing decreased significantly (p<0.05), and their satisfaction scores increased significantly (p<0.05) as their expectation fulfilment scores increased. In conclusion, the results revealed that the TKA patients in the study had high expectation scores of nursing care (77.19±13.87) but their scores for fulfilment of their expectations were low (47.03±26.73), and their expectations of care and fulfilment of expectations affected the scores for their satisfaction with nursing care.en_US
