Özet
Bu çalışmanın amacı periferik nöropatisi olan ve olmayan tip 2 diyabetli bireylerde el kas kuvveti, el becerisi ve el duyusunu değerlendirmekti. Diyabetik periferik nöropatisi olan (DPN+) (n=31) ve diyabetik periferik nöropatisi olmayan (DPN-) (n=26) toplamda 57 hasta ile çalışma tamamlandı. Parametrelerin değerlendirilmesinde hafif dokunma duyusu için Semmes-Weinstein Monofilament Testi (SWMT), vibrasyon duyusu için 128 Hz diapozon, iki nokta ayırımı duyusu için esteziometre; kaba kavrama kuvvetinin değerlendirilmesinde Baseline (NY 10602 USA) hidrolik el dinamometresi, parmak kavrama kuvvetinin değerlendirilmesinde Baseline pinç ölçer; el becerisinin değerlendirilmesinde 9 delikli peg testi kullanıldı. DPN+ ve DPN- gruplarda hafif dokunma duyusu incelendiğinde gruplar arasında anlamlı fark olduğu belirlenmiştir (p<0,05). Sağ 2. parmak (2,83) ve sol 5. parmak (2,83) değerleri DPN+ grubu lehine anlamlılık göstermiştir. İki grup arasında vibrasyon duyusu değerleri karşılaştırıldığında anlamlı fark görülmedi (p>0,05). DPN+ ve DPN- gruplar arasında iki nokta ayırımı duyusu değerlendirildiğinde sağ el 1. parmak ve sol el 2. ve 3. parmaklarda DPN+ grup lehine anlamlı olarak bulundu (p<0,05). Kaba kavrama kuvveti değerleri her iki elde de DPN+ grubunda anlamlı olarak daha düşük bulunmuştur (p<0,05). DPN+ grubunda sol 1. ve 2. parmak kavrama kuvveti değerleri istatistiksel olarak anlamlı bulundu (p<0,05). El becerisi değerleri DPN+ grubunda anlamlı ölçüde daha yüksekti (p<0,05). Diyabetik hastalardaki periferik duyusal sinirlerdeki etkilenmeler el kavrama gücü ve el fonksiyonunu azaltmaktadır. Diyabet, duyu ve kas kuvveti arasındaki bu bağlantıları anlamak, diyabetli hastaların yaşam kalitesini korumalarına yardımcı olmak için hedefe yönelik müdahale ve rehabilitasyon stratejileri geliştirme açısından önemlidir.
The aim of this study was to evaluate hand muscle strength, dexterity and hand sensation in type 2 diabetic individuals with and without peripheral neuropathy. The study was completed with a total of 57 patients with diabetic peripheral neuropathy (DPN+) (n=31) and without diabetic peripheral neuropathy (DPN-) (n=26). For the evaluation of parameters, Semmes-Weinstein Monofilament Test (SWMT) for light touch sensation, 128 Hz tuning fork for vibration sensation, esthesiometer for two-point discrimination sensation; Baseline (NY 10602 USA) hydraulic hand dynamometer for the evaluation of rough grip strength, Baseline pinch meter for the evaluation of finger grip strength; and 9-hole peg test for the evaluation of manual dexterity. When light touch sensation was examined in the DPN+ and DPN groups, a significant difference was determined between the groups (p<0.05). The values of the right 2nd finger (2.83) and left 5th finger (2.83) were significant in favor of the DPN+ group. When vibration sense values were compared between the two groups, no significant difference was observed (p>0.05). When two-point discrimination sense was evaluated between the DPN+ and DPN- groups, it was found to be significant in favor of the DPN+ group in the 1st finger of the right hand and the 2nd and 3rd fingers of the left hand (p<0.05). Gross grip strength values were found to be significantly lower in the DPN+ group in both hands (p<0.05). Grip strength values of the left 1st and 2nd fingers in the DPN+ group were found to be statistically significant (p<0.05). Manual dexterity values were significantly higher in the DPN+ group (p<0.05). Affects in peripheral sensory nerves in diabetic patients reduce hand grip strength and hand function. Understanding these connections between diabetes, sense and muscle strength is important in terms of developing targeted interventions and rehabilitation strategies to help diabetic patients maintain their quality of life.
