dc.description.abstractBu çalışmada, astımı olmayan obez çocuklarda egzersizle oluşan bronkospazmın (EIB) derecesi ve sıklığının normal ağırlıktaki çocuklarla karşılaştırılması amaçlanmıştır. Çalışmaya yaşlan 11-15 arasında değişen, astım veya başka allerjik hastalığı olmayan 24 obez ve 26 kontrol olmak üzere toplam 50 erkek çocuk katıldı. Çocuklara önce istirahat solunum fonksiyon testleri (SFT) yapıldı. Bisiklet ergometresinde maksimum kalp hızının %85'ine ulaşacak şekilde 8 dakikalık egzersiz testi uygulandı. Egzersizin bitiminden 5 ve 15 dakika sonra SFT tekrarlandı. Elde edilen SFT değerleri, istirahat değerleri ile karşılaştırılarak azalma yüzdeleri hesaplandı. Obezlerde egzersiz sonrası 5. ve 15. dakikalarda FVC, FEVı/FVC, FEF25-75% ve PEFR değerlerinde meydana gelen azalmaların anlamlı olduğu; kontrol grubunda ise 5. dakikadaki FE Vı/FVC azalması dışında anlamlı değişiklik olmadığı tespit edildi. EIB oluşan vaka sayısı yönünden obezlerle kontrol grubu arasında fark bulunmazken egzersiz sonrası FVC, FEVı/FVC, FEF25-75% ve PEFR değerlerinde azalmanın obez grupta daha fazla olduğu görüldü. Obezite parametrelerinden BMI, subskapular deri kıvrım kalınlığı ve biseps deri kıvrım kalınlığının, egzersiz sonrası SFT değişiklikleri ile ters yönde anlamlı korelasyon gösterdiği tespit edildi (p<0.05). EIB'ın derecesi ve sıklığının, obez çocuklarda daha fazla olduğu, egzersiz sonrası SFT değişikliklerinin BMI ve deri kıvrım kalınlığı ile ilişkili olduğu, bu nedenle astımı olmayan genç populasyonda, özellikle obez çocuklarda EIB teşhisi ve tedavisinin egzersize tahammül gücünü, performansı ve sportif faaliyetlere katılımları düzeltebileceği düşünüldü.en_US
dc.description.abstractExercise Induced Bronchospasm in Nonasthmatic Obese and Nonobese Children In the present study, it was aimed to compare the degree and frequency of exercise- induced bronchospasm (EIB) on nonasthmatic obese children to normal weight children. 50 male children (24 obese and 26 control) whose ages varied between 11 and 15, with no history of asthma and other atopic disease, took part in this study. Pulmonary function tests (PFT) were performed on the children at rest. 8 minutes exercise test at cycle ergometer was applied until 85% of the predicted maximal heart rate. PFT were repeated 5 and 15 minutes after the exercise. This PFT data was compared to PFT data at rest and decrease rates were calculated. It was determined that, for obese children the decreases in the post-exercise FVC, FEVı/FVC, FEF25.750/,, and PEFR values at 5th and 15th minutes were significant and for the control group only significant change was the decrease in FEVı/FVC values at 5th minute. While, according to the number of EIB cases, there was no significant difference between obese and control group, the decreases in post-exercise FVC, FEVı/FVC, FEF25- 75o/o and PEFR values of obese group were determined to be more than that of control group. Post-exercise PFT changes were determined to have significant reverse correlation with BMI, subscapular skinfold and biceps skinfold (p<0.05). It is concluded that, since the degree and frequency of EIB is higher for obese children and post-exercise PFT changes are correlated with BMI and skinfolds in the young nonasthmatic population, especially in obese children, diagnosis and management of EIB may improve aerobic exercise endurance, performance and participation.en_US
