IgE aracılı besin alerjili çocuk hastaların retrospektif değerlendirilmesi

Özet
Besin alımını takiben ortaya çıkan immünolojik reaksiyonlar besin alerjisi olarak
adlandırılır. Besin alerjisi prevalansı gittikçe artan, çocuk ve yetişkinleri etkileyen önemli
bir halk sağlığı sorunu olarak karşımıza çıkmaktadır. İnsanların yaşam kalitesi ve
morbidite üzerinde önemli bir etkiye sahiptir. Bu çalışmada hastanemize başvuran 5 yaş ve
altındaki IgE aracılı besin alerji tanılı hastaların demografik özellikleri, tolerans gelişimine
etki eden faktörleri ve tanıda kullanılan belirteçlerin değerliliğinin gösterilmesi hedeflendi.
Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Hastanesi Çocuk Alerji ve
İmmünoloji Bilim Dalı’nda Ocak 2015- Aralık 2018 arasında besin alerjisi şikayetleri olan
430 vaka tarandı. Tüm hastalar cinsiyet, yaş, özgeçmiş ve soy geçmiş, klinik bulgular,
laboratuar bulgular, aldıkları diyet ve yanıtları değerlendirildi. Bu değerlendirme
sonrasında IgE aracılı besin alerjisi tanısı alan kayıtlarına ulaşılabilen 289 vaka çalışmaya
dahil edildi.
Toplam 289 hastanın %36,5’unda sadece süt, %22,2’sinde sadece yumurta
%36,5’unda süt ve yumurta birlikte, %2,1’inde kuruyemiş %2,6’sında ise çoklu besin
alerjisi olduğu belirlendi. Hastaların yaş ortalaması 14,54±12,32 ay ve ortancası 10 ay (1-
60) olarak belirlendi. Hastaların 109’u (% 37,7) kız, 180’i (% 62,3) erkekti. Bunların
259‘unda (%90) tolerans geliştiği gözlemlendi. Takip süreleri ortalama 11,93±12,45 ay
olarak saptandı.
Sonuç olarak 5 yaş altı hastalarda besin alerjisinde etken olarak süt ve yumurta ön
planda düşünülmelidir. Besin alerjilerinde tanı anı total IgE değeri tolerans gelişimini
öngörmede yardımcı olabilir
Immunological reactions which occur following ingestion of foods are called food allergy. Food allergy is confronted as an important public health issue, whose prevalence is getting increased and which affects children and adults. It has an important effect on individuals’ quality of life and morbidity. In this study, it was aimed to demonstrate demographical characteristics, factors influencing development of tolerance and valuableness of markers used for diagnosis of patients at and under 5 years of age who admitted to our hospital and diagnosed with IgE-mediated food allergy. 430 cases complaining of food allergy which admitted to Necmettin Erbakan University Meram Faculty of Medicine Division of Pediatric Allergy and Immunology between January 2015 and December 2018 were reviewed. All patients were evaluated in regard to gender, age, background, family history, clinical findings, laboratory findings, the diet they followed and their responses. After this evaluation, 289 cases which were diagnosed with IgE-mediated food allergy and whose records were reached were included in the study. Of a total of 289 patients; 36.5% were determined to have cow’s milk allergy alone, 22.2% egg allergy only, 36.5% concomitant cow’s milk and egg allergy, 2.1% nut allergy and 2.6% multiple food allergies. It was determined that patients’ mean age was 14.54±12.32 months and the median was 10 months (1-60). Of the patients; 109 (37.7%) were female and 180 (62.3%) were male. Of these; 259 (90%) were observed to develop tolerance. Median duration of follow-up was determined to be 11.93±12.45 months. In conclusion, cow’s milk and egg should be considered at the forefront as an agent of food allergy in patients under 5 years of age. Total IgE levels on admission may be helpful in predicting development of tolerance in food allergies
