Bening prostat hiperplazisinde (BPH) transüretral rezeksiyon (TUR) sonrası uygulanan Terazosin tedavi sonuçları

dc.description.abstractBPH'ın cerrahi tedavisinde günümüzde halen TUR-P altın standart olarak kabul edilmektedir. Bunun yanında medikal tedavide de birçok ajan kullanılmaktadır. Bizde çalışmamızda prostatata TUR uygulanması sonrası terazosin tedavisi uyguladık ve sonuçlarını değerlendirdik. Kliniğimizde BPH tanısı konan ortalama 64,8 yaşında toplam 40 hasta çalışmaya dahil edildi. 20 hastaya TUR-P ve diğer 20 hastaya da TUR-P uygulanmasını takiben 6 ay süre ile günde 5 mg terazosin tedavisi başlandı. Tüm hastaların preoperatif ve post operatif altıncı ayda IPSS değerlendirmesi, uroflowmetrik ve sistometrik incelenmesi, rezidüel idrar tayini, prostat ağırlığının ölçülmesi ve seksüel fonksiyonları değerlendirildi. Her iki grup istatistiki olarak karşılaştırıldı. TUR-P grubuna göre TUR-P sonrası terazosin uygulanan hasta grubunda ortalama semptom skorunda azalma ve ortalama post operatif birinci ve altıncı aydaki maksimum ve average idrar akım hızlarında artış gözlendi. Bu bulgular Mann Withney U ve T testi ile istatistiki olarak karşılaştırıldı. Her iki grupta da istatistiki olarak anlamlı bir fark gözlenmedi (P>0.05). Sistometrik olarak postoperatif altıncı ayda TUR-P grubunda detrusor instabilitesi % 55,5 oranında düzelme gösterirken TUR-P sonrası terazosin uygulanan grupta detrusor instabilitesi % 72.2 oranında düzelme gösterdi. Çalışmamız da TUR-P sonrası terazosin uygulanması ile sistometrik parametrelerde daha erken ve daha olumlu düzelmelerin olabileceği sonucuna varılmıştır. Daha geniş çalışma gruplarıyla özellikle detrusor instabilitesi bulunan hasta gruplarıyla benzer çalışmaların desteklenmesi gerekir.en_US
dc.description.abstractTUR-P is currently known as the golden standard for surgical therapy of BPH. Along with TUR-P, a lot of agents are also used in medical treatment. The effect of terazosin after TUR-P has been reported in this study. 40 patients of 64.8 average age with BPH are included in our study. 20 patients have been treated with a daily amount of 5 mg terazosin for each time after the TUR-P for 6 months, while the other 20 have been subjected to TUR-P only. All patients preoperative and postoperative, IPSS (International Prostate Symptoms Scores) values, uroflowmetric and systometric studies, residual urine measurements, prostate volume and sexual functions have been evaluated and both groups have been compared statistically. A considerable decrease in symptom scores and increase in maximum and average postoperative urine flow rate in first and sixth months were the first findings in the terazosin applied group as compared to the TUR-P only group. These findings have been statictically compared with Mann Withney U and T tests. There were no statistically meaningful results these groups( p>0.05). Cystometrically, sixth month postoperatively detrusor instability in TUR-P only group had 55.5% recovery while after TUR-P terazosin user group had 72.2% recovery. There were no meaningful difference between the two groups with respect to sexual functions. After our study, it is concluded that there can be earlier and better recoveries in cystometric parameters of the patients by the application of terazosin after TUR-P. This study should be supported with studies conducted with bigger groups especially those with detrusor instability.en_US
