dc.description.abstractSol Ventrikül Kitlesi ve Diyastolik Fonksiyona Obezite ve Hipertansiyonun Etkisi GİRİŞ: Yüksek kan basıncı olan hastaların büyük bölümü aşırı kiloludur ve zayıf kişilere oranla obezlerde hipertansiyon daha sıktır. Hipertansiyon yalnızca obez kişilerde daha sık olmakla kalmayıp genç insanlarda hipertansiyon gelişimi içinde en önemli risk faktörüdür. Özellikle obeziteye hipertansiyonun eşlik ettiği durumlarda erken sol ventrikül disfonksiyonu gelişebilmektedir. Çoğu kişide uyku sırasında kan basıncı en düşük düzeye inmektedir. Bazı kişilerde kan basıncı uyku sırasında düşmemektedir. Uyku sırasında kan basıncında düşme görülmeyen kişilerin prognozunun uyku sırasında kan basıncında düşme görülenlerden daha kötü olduğu öne sürülmüştür. MATERYAL ve METOD: Çalışmaya obez hipertansif, obez normotensif, nonobez hipertansif ve nonobez normotensif olmak üzere 120 hasta alınmıştır. Tüm bireylere standart ekokardiyografik inceleme ve ambulatuvar kan basıncı monitorizasyonu yapıldı. BULGULAR: Obez ve hipertansif hastalarda LV septum ve posteriyor duvar kalınlıklarında artma olduğu diyastolik fonksiyonlarda azalma olduğu tespit edildi.. Ayrıca hipertansif hastalarda obezitenin gece sistolik kan basıncı düşme oranım azalttığı tespit edildi. SONUÇ: Obezite LV üzerinde yapısal ve fonksiyonel değişikliklere neden olmakta, özellikle sol ventrikül kitlesinde artışa yol açarak LV fonksiyonlarım etkilemektedir. Ayrıca obezite hipertansif hastalarda nondipper durumuna katkıda bulunmaktadır.en_US
dc.description.abstractThe Effects of Obesity and Hypertension onthe Left Ventricular Mass and Diastolic Function INTRODUCTION: Most of the patients having high blood pressure (hypertension) are overweight people and in comprasion with non-obese people hypertension is seen more often in obese people. Hypertension not only existing in obese people but also obesity is the most important risk factor for young people in terms of increaseing hypertension.Especially incases in which obesity accompanied to hypertension early left ventricular disfunction is able to develop. For most people while the blood pressure decreases (falls) the lowest level during the sleep for some it dosent. It has been claimed that the prognose of the people whose blood pressure doesn't fall during sleep is worse than those of who have low blood pressure during the sleep. MATERIAL and METHOD: Consisting of obese hypertensive, obese normotensive, non-obese hypertensive and non-obese normotensive 120 patients were taken in to study. All the patients were implied to standart echocardiographic research and ambulatuwar blood pressure monitorization. FINDINGS: It was determined teat in patients suffering from obesity and hypertension there has been decrease in diastolic functions and increase in LV septum and posterior wall thickness. Beside this, it was determined teat obesity in patients who has hypertension decreased the falling rate of systolic blood pressure at nights. RESULT: Obesity causes structural and functional changes on LV, especially, by causing increase in left ventricular mass, effects LV functions. In addition, obesity contributes to nondipper in patients having hypertension.en_US
