Özet
Bu çalışma, Elazığ bölgesinde Covid-19 tanısı almış hastaların serum vitamin D, vitamin B12,
folat ve ferritinin düzeylerinin, hastaneye sadece kontrol amacıyla gelmiş Covid-19 tanısı almamış
kontrol grubuyla retrospektif karşılaştırılması ile Covid-19 hatalığının tedavisinde vitaminlerin
gerekliliğinin araştırılması amaçlanmıştır.
Çalışmamız Covid salgını süresince 2020 ile 2022 tarihleri arasında Fırat Üniversitesi Eğitim
Araştırma Hastanesi Biyokimya Laboratuvarında analiz edilen Covid-19 ile enfekte olan 30 hasta ile ve
sağlık1ı kişiler arasından rastgele yöntemle seçilen 30 hasta ile toplamda 60 hastada gerçekleştirilmiştir.
Covid-19 tanısı almış hastalar ve kontrol grubunun vitamin D, vitamin B12, folat ve ferritinin düzeyleri
retrospektif karşılaştırılarak değerlendirilmesi yapılmıştır. Hastaların demografik (yaş ve cinsiyet),
klinik, laboratuvar ve tedavi verileri için hastane elektronik veri tabanından faydalanılmıştır.
Çalışmamıza alınacak hastaların yaşı 18 ve üzeri olarak belirlenmiş olup cinsiyet farkı gözetilmemiştir.
Laboratuvar parametrelerinin prediktif değerlerinin belirlenmesinde ROC eğrisi, ki-kare ve t-testi
uygulanmıştır. Sayısal verilerin değerlendirilmesinde aritmetik ortalama, standart sapma, minimum ve
maksimum değerleri; kategorik verilerin özetlenmesinde frekans dağılımları ve yüzdelikler
kullanıldı. Tüm istatiksel analizler IBM SPSS Statistics 22.0 programında a =0.05 anlamlılık seviyesi
düzeyinde analiz edilip raporlanmıştır.
Çalışma grubumuz %57 erkeklerden %43 kadınlardan oluşmuştur. Covid grubunun yaş
ortalaması 44, kontrol grubunun yaş ortalaması ise 34 olarak hesaplandı. Çalışmamızın bulguları Covid
grubu ferritin ortancası 355 ng/mL (244,592), folik asit ortancası 6.77 ng/mL (5.73, 8.32), vitamin B12
ortancası 318 pg/mL (268, 394), D vitamini ortancası 17 mg/mL (11, 21), şeklinde bulunmuştur. İki grup
kıyaslandığında ferritin ve D vitamini düzeylerinin anlamlı çıktığı saptanmıştır (p<0.001). Yapılan ROC
analiz sonucunda; ferritin 1.00; folik asit 0.64; vitamin B12 0.70; D vitamini 0.86 şeklinde istatistiksel
olarak anlamlı bulunmuştur. Duyarlılık bakımından ferritinin (1.00) en duyarlı parametre olarak göze
çarpmaktadır. Özgüllük bakımından ise ferritinin (1.00) ve D vitamini (1.00) öne çıkan parametreler
olarak bulunmuştur.
Serum ferritinin, folik asit, vitamin B12 ve D vitamini düzeyleri Covidli hastalarda düşük olduğu
tespit edildi. Covid-19 ile enfekte olan hastalarda genel tedavilere başlamadan önce veya esnasında
vitamin parametreleri mutlaka değerlendirilmelidir.
In this study, it was aimed to investigate the necessity of vitamins in the treatment of Covid19 disease by retrospectively comparing the serum vitamin D, vitamin B-12, folate and ferritin levels of the patients diagnosed with Covid-19 in the Elazig region with the control group not diagnosed with Covid-19, who came to the hospital for control purposes only. Our study was carried out with 30 patients infected with Covid-19 analyzed in the Biochemistry Laboratory of Fırat University Training and Research Hospital between 2020 and 2022 during the Covid epidemic, and 30 patients selected randomly from healthy people, in total 60 patients. The vitamin D, vitamin B12, folate and ferritin levels of patients diagnosed with Covid-19 and the control group were evaluated retrospectively by comparing them. The hospital electronic database was used for demographic (age and gender), clinical, laboratory and treatment data of the patients. The age of the patients to be included in our study was determined as 18 and over, and no gender difference was observed. ROC curve, chi-square and t-test were used to determine the predictive values of laboratory parameters. Arithmetic mean, standard deviation, minimum and maximum values in the evaluation of numerical data; Frequency distributions and percentages were used in summarizing categorical data. All statistical analyzes were analyzed and reported in IBM SPSS Statistics 22.0 program at a =0.05 significance level. Our study group consisted of 57%men and 43%women. The mean age of the Covid group was 44, and the mean age of the control group was 34. The findings of our study were found in the Covid group ferritin median 355 ng/mL (244, 592), folic acid median 6.77 ng/mL (5.73, 8.32), vitamin B12 median 318 pg/mL (268, 394), vitamin D median 17 mg/mL (11, 21) , was found in the form. When the two groups were compared, ferritin and vitamin D levels were found to be significant (p<0.001). In terms of sensitivity, ferritin (1.00) stands out as the most sensitive parameter. In terms of specificity, ferritin (1.00) and vitamin D (1.00) were found to be prominent parameters. Serum ferritin, folic acid, vitamin B12 and vitamin D levels were found to be low in patients with Covid. Vitamin parameters must be evaluated before or during general treatments in patients infected with Covid-19.
