Özet
Amaç: Arı alerjisi olan hastalarda serum bazal triptaz seviyesinin elektrokardiyografik
parametrelerle korelasyonu değerlendirilerek kardiyak aritmilere yatkınlığın ortaya
konulması amaçlanmıştır.
Yöntem: Necmettin Erbakan Üniversitesi (NEÜ) Meram Tıp Fakültesi Erişkin İmmünoloji
ve Allerji Hastalıkları polikliniğine Ekim 2020 ile Ekim 2021 tarihleri arasında
Hymenoptera venomu ile ilişkili geniş lokal veya sistemik reaksiyon öyküsü olan 18-65 yaş
aralığında 62 hasta grubu ve NEÜ Meram Tıp Fakültesi Kardiyoloji polikliniğine herhangi
bir şikayetle başvurmuş herhangi bir kardiyak patolojisi tespit edilmemiş 18-65 yaş
aralığında 50 sağlıklı kişi kontrol grubu olarak çalışmaya dahil edilmiştir. Hasta grubunda
başvuru anında venom spesifik immunglobulin E (sIgE), total immunglobulin E (IgE) ve
bazal triptaz; kontrol grubunda ise total IgE ve bazal triptaz tayini için serum örnekleri
alınmıştır. Hasta ve kontrol grubunda elektrokardiyografi (EKG) tetkiki yapılmıştır.
Elektrokardiyografik parametrelerin değerlendirilmesi yardımcı araştırmacı olan kardiyoloji
öğretim üyesi tarafından yapılmıştır.
Bulgular: Arı alerjisi olan hastaların yaş ortalaması 39.16±12.63 yıl olup %67.74’ü erkekti.
Kontrol grubunda yer alan sağlıklı bireylerin ise yaş ortalaması 38.18±8.96 yıl olup yine
%67.74’ü erkekti. Hasta grubunda ortalama serum bazal triptaz düzeyi 2.982±1.98 µg / L,
kontrol grubunda ise 1.915±1.43 µg / L olarak bulunmuştur. Hasta grubuna ait olgularda
sistemik reaksiyon şiddeti ile yaş ve serum bazal triptaz düzeyi arasında istatiksel olarak
anlamlı pozitif korelasyon bulundu (p<0.05). P dalga dispersiyon (PDD) değeri >40
milisaniye(ms) olan hastaların triptaz seviyeleri istatistiksel olarak anlamlı daha yüksek
bulunmuştur (p<0.05). Hasta grubunda P min, P max, Tp-e süresi, Tp-e / QT, Tp-e / QTc ve
İCEB değerleri ile triptaz arasında istatiksel olarak anlamlı bir ilişki saptanmamıştır
(p>0.05).
Sonuç: EKG bulgularında PDD değeri >40 ms olan olguların bazal triptaz düzeyi ortalama
3.3±2.31 µg / L olarak saptanmıştır. Bu hastaların kardiyak açıdan takibinin yapılması
gerektiği düşünülmektedir.
Objective: The goal is to determine if patients with bee allergies are more susceptible to cardiac arrhythmias by analyzing the correlation between serum basal tryptase levels and electrocardiographic parameters. Materials and methods: Between October 2020 and October 2021, 62 patients aged 18 to 65 years old with a history of extensive local or systemic reaction related to Hymenoptera venom and any patient were admitted to NEU Meram Faculty of Medicine Cardiology outpatient clinic at Necmettin Erbakan University (NEU) Meram Medical Faculty Adult Immunology and Allergy Diseases outpatient clinic. The study comprised 50 healthy persons between the ages of 18 and 65 as the control group. At the time of admission, the patient group had venom-specific immunoglobulin E (sIgE), total immunoglobulin E (IgE), and basal tryptase. Serum samples were obtained from the control group to determine total IgE and basal tryptase levels. In both the patient and control groups, electrocardiography (ECG) was performed. The assistant researcher cardiology faculty member evaluated electrocardiographic parameters. Results: The mean age of the patients with bee allergy was 39.16±12.63 years, and 67.74% of them were male. The mean age of healthy individuals in the control group was 38.18±8.96 years, and 67.74% of them were male. The mean serum basal tryptase level was 2.982±1.98 µg / L in the patient group and 1.915±1.43 µg / L in the control group. A statistically significant positive correlation was found between the severity of systemic reactions, age, and serum basal tryptase levels in the patients belonging to the patient group (p<0.05). Patients with P wave dispersion (PDD) value >40 milliseconds (ms) had statistically significantly higher tryptase levels (p<0.05). In the patient group, there was no statistically significant correlation between P min, P max, Tp-e time, Tp-e / QT, Tp-e / QTc, and ICEB values and tryptase (p> 0.05). Conclusion: The mean basal tryptase level was found to be 3.3±2.31 in subjects with a PDD value > 40 ms in ECG results. These patients are regarded to need to be followed up on from a cardiac standpoint
