Fossa Pterygopalatina'nın Klinikle İlgili Morfometrik Analizi ve Komşu Paranazal Sinüslerle Hacimsel İlişkisi: BT'ye Dayalı Bir Çalışma Dr. Betül Digilli, Uzmanlık Tezi, Konya 2022 Amaç: Çalışmamızın amacı, fossa pterygopalatina'nın (FPP) ve çevresindeki boşluklarla bağlantı kuran foramen (for.) rotundum (FR), for. sphenopalatinum (FSP), canalis pterygoideus (CP), canalis palatovaginalis (CPV), canalis palatinus major (CPM), fissura pterygomaxillaris (FP) gibi anatomik yapıların morfometrik özelliklerini belirlemekti. Ayrıca çalışmamızda septum nasi deviasyonu (SND) ve septum sinuum sphenoidalium'un deviasyon (SSSD) tipleri detaylandırılarak bu tiplerin FPP'nin hacim ve morfometrik ölçümlerine ilaveten ona komşu olan paranazal sinüslerin hacim ölçümleri ile ilişkisinin bütüncül olarak incelenmesi amaçlandı. Yöntem: Bu çalışmada 18-79 yaş aralığındaki 260 hastaya ait (130 erkek, 130 kadın) bilgisayarlı tomografi (BT) görüntüleri retrospektif olarak incelendi. FPP'ye ait morfometrik ölçümler BT görüntüleri üzerinde aksiyal, koronal ve sagital düzlemlerde gerçekleştirildi. FPP, sinus maxillaris (SM) ve sinus sphenoidalis (SS) hacim ölçümleri ise 3D-SLICER yazılımı ile ölçüldü. Bulgular: FSP-FP uzaklığı kadınlarda erkeklerden anlamlı derecede yüksek, FR-FP açısı ise erkeklerde kadınlardan anlamlı derecede yüksek bulundu. CP-SM uzaklığı ve FR-FP açısı lateralizasyona göre sağ tarafta sol tarafa göre anlamlı derecede yüksek bulundu. CP-FSP açısı ve CPM-FPP açısı ise sol tarafta sağ tarafa göre anlamlı derecede yüksek tespit edildi. Hacim ölçümlerinin hepsi erkeklerde kadınlardan anlamlı derecede yüksek tespit edildi. Sadece FPP hacim ölçümü sağ tarafta sol tarafa göre anlamlı derecede yüksek tespit edildi. SM hacmi 18-34 yaş grubunda anlamlı derecede yüksekti. Hacim ölçümlerinden sadece SS hacmi hem SSSD hem de SND tespit edilen aynı tarafta daha küçük bulundu. Sonuç: Paranazal sinüslerle FPP hacminin pozitif veya negatif korelasyon ilişkisi bulunmadı. SS ve SM hacimleri arasında ise kuvvetli bir pozitif korelasyon ilişkisi saptandı. Mevcut çalışmamızın sunduğu verilerin özellikle endoskopik prosedürlerde radyologlara ve cerrahlara tanı ve tedavide faydalı olacağı düşüncesindeyiz. Clinically Relevant Morphometric Analysis of Pterygopalatine Fossa and Its Volumetric Relationship with Adjacent Paranasal Sinuses: A CT-Based Study Betül Digilli, MD, Dissertation, Konya 2022 Objective: Our study aimed to determine the morphometric features of the pterygopalatine fossa (PPF) and its anatomical structures (foramen rotundum (FR), sphenopalatine foramen (SPF), pterygoid canal (PC), palatovaginal canal (PVC), greater palatine canal (GPC), pterygomaxillary fissure (PMF)) connecting with the surrounding spaces. In addition, we aimed to detail the nasal septal deviation (NSD) and sphenoidal sinus septal deviation (SSSD) types and examine the relationship of these types with the volume and morphometric measurements of the PPF in addition to the volume measurements of the adjacent paranasal sinuses. Methods: In this study, we retrospectively analyzed computed tomography (CT) images of 260 patients (130 males, 130 females) aged 18-79 years. We performed morphometric measurements of PPF on CT images in axial, coronal, and sagittal planes. In addition, we measured PPF, maxillary sinus (MS), and sphenoidal sinus (SS) volume measurements with 3D-SLICER software. Results: We found the SPF-PMF distance significantly higher in females than males, and the FR-PMF angle was significantly higher in males than females. Also, we found PC-MS distance and FR-PMF angle considerably higher on the right side than on the left side according to lateralization. We determined that the PC-SPF and GPC-PPF angles are significantly higher on the left side than on the right side. All the volume measurements were substantially higher in males than females. Only PPF volume measurement was found to be significantly higher on the right side compared to the left side. MS volume was considerably higher in the 18-34 age group. Of the volume measurements, only the SS volume was smaller on the same side where both SSSD and NSD were detected. Conclusion: There was no positive or negative correlation between PPF volume and paranasal sinuses. A strong positive correlation was found between SS and MS volumes. We think that the data presented by our current study will be helpful for radiologists and surgeons in diagnosis and treatment, especially in endoscopic procedures.
