Özet
OSB de temel belirti olarak kabul edilen iletişim ve etkileşim becerilerinde yetersizlik ve ilgi
alanlarında kısıtlılık problemlerinin hafif şiddette olan formları literatürde geniş otizm fenotipi, eşik
altı otistik belirtiler ya da otistik traitler olarak isimlendirilmektedir.
Otizm Spektrum Bozukluğu, Dikkat Eksikliği Hiperaktivite Bozukluğu ve Özgül Öğrenme
Bozukluğunda çekirdek belirtiler birbirinden farklı olmasına rağmen bu bozuklukların birbirleriyle
örtüştükleri birçok ortak alan bulunmaktadır. Ayrıca OSB, DEHB ve ÖÖB tanılı çocukların ve yakın
akrabalarının özellikle toplumsal iletişimde ve etkileşimde normal popülasyona göre daha fazla zorluk
yaşadıkları görülmüştür. Bu durum etiyolojileri hala tam olarak açıklanamayan OSB, DEHB ve
ÖÖB’nin ortak genetik etmenlerden kaynaklanabileceğini düşündürmekte ve bu konudaki
araştırmaların önemini artırmaktadır. Bu sebeple bu çalışma da OSB, DEHB ve ÖÖB tanılı çocukların
ebeveynlerinin GOF düzeyleri karşılaştırılmıştır.
Araştırma Konya Eğitim ve Araştırma Hastanesi Çocuk Psikiyatri bölümüne başvuran ve
DEHB, ÖÖB ya da OSB tanısı alan çocukların gönüllü anne babaları ile gerçekleştirilmiştir. Anne
babalara GOF düzeylerini ölçmede kullanılan OSA anketi ve araştırmacı tarafından oluşturulan
demografik bilgi formu uygulanmıştır.
Araştırma sonucunda anne ve babalar bir bütün olarak değerlendirildiğinde, ayrıntıya dikkat
ve hayal gücü alt testlerinde OSB, DEHB ve ÖÖB’li anne babalar arasında anlamlı fark
saptanmıştır(P<0,05). Farkın hangi gruplar arasında olduğunu belirlemek için yapılan çoklu
karşılaştırma analizi sonucunda ayrıntıya dikkat alt testinde OSB- DEHB ve ÖÖB-DEHB arasında,
hayal gücü alt testinde ise DEHB-OSB arasında anlamlı fark bulunmuştur. OSB, DEHB ve ÖÖB’li
anne babaların OSA toplam puanı, sosyal beceri, dikkati kaydırma ve iletişim alt ölçeklerinde gruplar
arasında anlamlı fark bulunmamıştır(P>0.05).
Çalışmaya katılan babaların alt ölçeklerinin karşılaştırmasında dikkat kaydırma alt testinde
gruplar arasında anlamlı fark bulunmuştur. Farkın hangi gruplar arasında olduğunu belirlemek için
yapılan çoklu karşılaştırma sonucu ÖÖB ile OSB arasında anlamlı fark bulunmuştur. (p=0.03).
Çalışmaya katılan annelerin OSA alt ölçek puanlarının karşılaştırmasında ayrıntıya dikkat ve hayal
gücü alt testinde gruplar arasında anlamlı fark bulunmuştur. Ayrıntıya dikkat alt testinde ÖÖB ile
DEHB(P<0,001) ve OSB ile DEHB(P=0.040) grupları arasında anlamlı farklılık gözlenmiştir. Hayal
gücü alt testinde ise DEHB ile OSB(P=0.020) grupları arasında anlamlı farklılık gözlenmiştir.
Cinsiyet açısından değerlendirildiğinde GOF özelliklerinin OSB’dekine benzer şekilde;
erkeklerde kadınlardan daha yüksek olduğu ve OSB’li grupta diğer engel gruplarına göre daha yüksek
oranda görüldüğü sonucu elde edilmiştir.
Subthreshold forms of inadequate communication and interaction skills and limitations in areas of interest, which are considered to be the main symptoms in autism spectrum disorder (ASD), arenamed as broad autism phenotype, sub-threshold autistic symptoms or autistic traits in theliterature. Although the coresymptoms of ASD, attention deficit hyperactivity disorder(ADHD) and specific learning disorder(SLD) are different from each other, there are many common are as where these disorders overlap. In addition, it was observed that children diagnosed with ASD, ADHD and SLD and their close relatives experienced more difficulties especially in social communication andinteraction compared to the normal population. This situation suggests that ASD, ADHD and SLD, may result from common genetic factors and increases the importance of research on this subject. Therefore, this study compared the broad autism phenotype levels of the parents of children with ASD, ADHD, and SLD. The study was conducted with the voluntary parents of children who applied to Konya Training and Research Hospital Child Psychiatry Departmentand were diagnosed with ADHD, SLD or ASD. The Autism Spectrum Quotient (ASQ) was used to measure broad autism phenotype(BAP) levels and the demographic information form created by the researcher were applied to the parents. As a result of thestudy, while mothers and father were evaluated together, a significant difference was found between parents with ASD, ADHD and SLD in attention to detail and imagination subtests (P <0.05). As a result of the multiple comparison analysis performed to determine between which groups the difference was, a significant difference was found between ASD and ADHD and SLD and ADHD in the attention to detail subtest and between ADHD and ASD in the imagination subtest. There was no significant difference between the groups in the ASQ total score, social skills, distraction and communication subscales of the parents with ASD, ADHD and SLD (P> 0.05). In the comparison of subscales of fathers who participated in the study, a significant difference was found between the groups in the attention shift subtest. A significant difference was found between SLD and ASD as a result of multiple comparisons made to determine which groups the difference was between (p = 0.03). In the comparison of ASQ subscalescores of the mothers who participated in the study, a significant difference was found between the groups in the attention to detail and imagination subtest. In the attention to detail subtest, a significant difference was observed between SLD-ADHD (P<0.001) and ASD-ADHD (P = 0.04) groups. In the imagination subtest, a significant difference was observed between ADHD-ASD (P = 0.02) groups. When evaluated in terms of gender, the characteristics of GOF are similar to the ASD; It was found that it is higher in men than women and it is seen at a higher rate in the group with ASD compared to other disability groups.
