Özet
Amaç: Çalışmada intermitan ekzotropya tanısı alarak opere olan hastalardan
bilateral lateral rektus (LR) geriletme cerrahisi ile tek göz LR geriletme medial
rektus (MR) kısaltma cerrahisi uygulananların postoperatif sonuçları karşılaştıldı.
Gereç ve Yöntem: Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi
Hastanesi Göz Hastalıkları Kliniği’nde 2013-2019 tarihleri arasında intermitan
ekzotropya tanısı alarak opere olan hastaların kayıtları retrospektif olarak incelendi.
Çalışmaya dahil edilen 100 hastadan 60’ına tek göz LR geriletme MR kısaltma,
40’ına bilateral LR geriletme cerrahisi uygulandı. Diagnostik kapama testi sonrası 92
hasta temel tip, 8 hasta psödodiverjans tip intermitan XT olarak gruplandırıldı.
Hastaların preoperatif ve postoperatif takiplerinde görme keskinlikleri, refraksiyon
ölçümleri ve ortoptik muayeneleri yapıldı. Postoperatif takiplerde kayma açısı -/+10
PD değerleri arası ise cerrahi başarılı, >10 PD ise overkorreksiyon, <-10 PD ise
subkorreksiyon olarak değerlendirildi. Araştırmada elde edilen veriler IBM SPSS
Statistics 24 programı kullanılarak analiz edildi. Elde edilen p değerleri için; p<0.05
ise istatistiksel olarak anlamlılık vardır şeklinde değerlendirme yapıldı.
Bulgular: Araştırmada değerlendirilen 100 hastanın 47’si (%47) kadın, 53’ü
(%53) erkekti. Ortalama opere olma yaşı 13.16±8.48 (3-35) yıldı. Preoperatif uzak
kayma açısı ortalama 32.78±8.73 prizma diyoptri (PD) (-50.00/-16.00), yakın kayma
açısı ortalama 32.21±9.61 PD (-55.00/-14.00) olarak tespit edildi. Postoperatif
ortalama takip süresi tek göz LR geriletme MR kısaltma grubunda 16 ay, bilateral
Purpose: In the study, the postoperative results of the patients who were operated on with the diagnosis of intermittent exotropia, who underwent bilateral lateral rectus recession (BLR) surgery and one eye lateral rectus recession medial rectus resection (R&R) surgery were compared. Materials and Methods: The data of patients who were operated with the diagnosis of intermittent exotropia in Necmettin Erbakan University Meram Medical Faculty Ophthalmology Department between 2013-2019 were analyzed, retrospectively. Of the 100 patients included in the study, 60 patients underwent R&R, and 40 patients underwent BLR surgery. After the diagnostic cover test, 92 patients were grouped as basic type and 8 patients as pseudodivergence type intermittent XT. Visual acuity, refraction measurements and orthoptic examinations were performed in the preoperative and postoperative follow-ups of the patients. If the deviation angle was between -/+ 10 PD values, the surgery was considered successful, if> 10 PD was evaluated as overcorrection and if <-10 PD was considered as subcorrection. Data obtained in the study was analyzed by using the IBM SPSS Statistics 24 program and p <0.05 for results were considered statistically significant. Results: Of the 100 patients evaluated in the study, 47 (47%) were female and 53 (53%) were male. The mean age of operation was 13.16 ± 8.48 (3-35) years. Preoperative distance angle of deviation as average 32.78 ± 8.73 prism diopter (PD) (-50.00 / -16.00), near angle of deviation as average 32.21 ± 9.61 PD (- 55.00 / -14.00) had been determined. Postoperative mean follow-up time was 16 months in the R&R group and 20 months in the BLR group. In the postoperative 1st month and 6 th month follow-ups, the success rates in distance and near deviation were found to be more successful in patients who underwent BLR surgery compared to R&R surgery. Subcorrection was more common in patients who underwent BLR vii surgery. In the 1st year, 2 nd year and 3 rd year controls, no significant difference was found in terms of success, overcorrection and subcorrection in both groups. There was an increase in stereopsis rates after surgery in both groups. In postoperative follow-ups, 49 patients continued to have binocular vision, while 14 patients repaired their binocular vision. Conclusion: Although R&R surgery was found to be more successful than BLR surgery in the early period, no significant difference was found between the two surgical methods in the long term.
