dc.description.abstractSağlık finansmanı, sağlık sektöründe finansal kaynakların harekete geçirilmesi ve kullanılmasını içeren geniş kapsamlı bir kavramdır. Sağlık hizmetlerinin finansman sorunu her ülke için oldukça önemli ve çözümü zor bir konudur. Sağlık hizmetlerinin finansmanında kullanılan yöntemlerden biri olan tanıya dayalı paket fiyat ödeme yöntemi, sık karşılaşılan hastalıkların tanı ve tedavisi sırasında hizmet içeriği bakımından hasta bazında çok fazla değişiklik göstermeyen ve bu nedenle yaklaşık maliyeti önceden tahmin edilebilen tüm işlemler için tek bir fiyat belirlenmesi olarak tanımlanmaktadır. Bu çalışmada paket fiyat ödeme kapsamında olan obezite cerrahisinin maliyet analizini yaparak hizmet maliyetinin belirlenmesi ve geri ödeme tutarının maliyeti karşılayıp karşılamadığını belirlemek amaçlanmıştır. Araştırma evrenini Konya ilinde bulunan Selçuk Üniversitesi Tıp Fakültesi Hastanesi ve Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Hastanesinde 01.01.2019-31.12.2019 tarihleri arasında obezite cerrahisi olan tüm hastalar oluşturmaktadır. Çalışmada üniversite hastanelerinde obezite bypass ve obezite sleeve hizmeti alan 70 hasta tespit edilirken ikinci bir ameliyat kodlanan hastalar çalışma kapsamından çıkarılmış ve 66 hasta maliyet analizine dahil edilmiştir. Araştırma sonucunda demografik özelliklere göre ortalama maliyetin değiştiği tespit edilmiş; erkek, kronik hastalığı olan, yatış günü uzun ve yaşı büyük olan hastaların ortalama maliyetinin daha yüksek olduğu belirlenmiştir. Ameliyat türüne göre inceleme yapıldığında obezite bypass geçiren hastaların daha fazla maliyete yol açtığı tespit edilmiştir. Maliyet unsurları içerisinde ise malzeme maliyetinin en yüksek paya sahip olduğu bulunmuştur. Yapılan analizler sonucunda obezite cerrahisinin hizmet üretim maliyeti ile geri ödeyici kurumun ödediği tutar karşılaştırıldığında ilgili hizmet için üniversite hastanelerinin zarar ettiği tespit edilmiştir. İleri düzey sağlık bakım hizmeti veren üniversite hastanelerinde hizmet çeşitliliği göz önüne alındığında yalnızca obezite cerrahisi için edilen zarar azımsanamayacak derecede büyüktür. Sağlık kurumlarında tanıya dayalı paket fiyat ödeme yönteminin maliyetleri karşılayamadığı ve SUT fiyatları ile hizmet veren üniversite hastanelerinin mali sürdürülebilirliklerini tehdit eden sonuçlara ulaşılmıştır.en_US
dc.description.abstractHealth financing is a comprehensive concept that includes the mobilization and use of financial resources in health sector. The financing problem of health services is very important and problem to solve for every country. Package price payment method based on diagnosis, one of the methods used in the financing of health services are defined as a single price determination for all procedures whose cost can be approximately estimated in advance because of the fact that they do not vary much in terms of service content on a patient basis during the diagnosis and treatment of frequently encountered diseases. In this study, it is aimed to determine the service cost by analyzing the cost of bariatric surgery, which is within the scope of package price payment, and to determine whether the reimbursement amount covers the cost or not. The research population consists of all patients who underwent obesity surgery between 01.01.2019 and 31.12.2019 at Selcuk University Medical Faculty Hospital and Necmettin Erbakan University Meram Medical Faculty Hospital in Konya. While 70 patients who received obesity bypass and obesity sleeve services in university hospitals were identified in the study, patients who were coded for a second surgery were excluded from the study and 66 patients were included in the cost analysis. As a result of the research, it was determined that the average cost varies according to demographic characteristics; it was determined that the average cost of male, chronic disease, long hospitalization days and older patients were higher. When examined according to the type of surgery, it was found that obesity caused more costs for patients undergoing bypass. It has been found that the material cost has the highest share among the cost elements. As a result of the analysis, it was determined that the cost of service production of obesity surgery and the amount paid by the paying institution were compared with the university hospitals for the relevant service. Considering the variety of services in university hospitals that provide advanced health care services, the damage done only for obesity surgery is substantial. It was concluded that the diagnosis-based package price payment method in health institutions could not cover the costs and threaten the financial sustainability of university hospitals serving with SUT prices.en_US
