Özet
Özellikle uzun süre hastanede kalan kişilerde ciddi nozokomiyal
enfeksiyonlara sebebiyet veren Providencia türlerindeki çoklu ilaç direncinin bir
parçası olan aminoglikozid ilaç direncinin aydınlatılmasına yardımcı olmak amacıyla
yaptığımız çalışmada bu dirence sebep olan genlerin varlığını belirlemeyi amaçladık.
Çalışmamıza Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Tıbbi
Mikrobiyoloji Laboratuvarı’na gönderilen ve kültüre alınmış biyolojik
materyallerden Providenciaspp. üreyen ve aminoglikozid ilaç direnci gözlenen
bakteriler dahil edilmiştir. İlaç direncine neden olan genleri moleküler düzeyde
araştırmak amacıyla PCR yöntemi kullanılmış ve 33 genin varlığı araştırılmıştır.
Araştırmalarımız sonucunda yedi tanesi aminoglikozidmodifiye eden enzim
geni (aac(2")-Ia, aac(3)-IIa, aac(3)-IIc, aac(6')-Ib, aac(6')-Ib-cr, ant(2')-Ia, ant(3")-Ia) ve bir tanesi de 16S rRNAmetilasyon geni (armA) olmak üzere toplamda sekiz
adet gen pozitif olarak bulunmuştur.
Çalışmamızdan elde edilen veriler doğrultusunda Providencia türlerindeki
aminoglikozid ilaç direncine sebep olan birçok gen bir arada bulunmuştur. Endişe
verici olan bu bakterilerin diğer bakterilerden horizontal olarak ilaç direnç genlerini
alabildiği ve kendi DNA’sına entegre edebildiği, aynı şekilde kendi direnç genlerini
de diğer bakterilere aktarabileceği gerçeğidir. Bunun önüne geçmek ve direnç oranını
azaltmak amacıyla antibiyotik kullanımında daha dikkatli davranılmalı ve direnç
genleri göz önüne alınarak tedavi stratejileri geliştirilmelidir.
We aimed to determine the presence of genes that cause this resistance in our study to help elucidate aminoglycoside drug resistance, which is a part of multi-drug resistance in Providencia species that cause serious nosocomialinfections, especially in people who stay in hospital for a long time. Providencia spp., one of the biological materials sent to Necmettin Erbakan University Meram Medical Faculty Medical Microbiology Laboratory and taken to culture. Included are bacteria that grow and exhibit aminoglycoside drug resistance. PCR method was used to investigate the genes causing drug resistance at the molecular level and the existence of 33 genes was investigated. As a result of our research, a total of eight genes, seven of which are minoglycoside modifying enzyme genes (aac (2 ")- Ia, aac (3) -IIa, aac (3) -IIc, aac (6 ')- Ib, aac (6')- Ib-cr, ant (2 ')- Ia, ant (3 ")- Ia) and one of which is 16S Rrna methylation gene (armA), were found to be positive. In line with the data obtained from ours tudy, many genes that cause aminoglycoside drug resistance in Providencia species were found together. What is alarming is the fact that bacteria can take drug resistance genes horizontally from other bacteria and integrate them into their own DNA, as well as transmit their resistance genes too the bacteria. In order to prevent this and to reduce the resistance rate, more careful attention should be paid to the use of antibiotics and treatment strategies should be developed considering the resistance genes.
