Parkinson hastalarında konjonktival impresyon sitolojisi ve gözyaşı parametrelerinin değerlendirilmesi

Özet
Amaç: Parkinson hastalarında konjonktival impresyon sitolojisi (KİS), Schirmer testi,
gözyaşı kırılma zamanı (GKZ) ve Oküler Yüzey Hastalık İndeksi (Ocular Surface Disease
Index, OSDI) anketi ile oküler yüzeyin değerlendirilmesi amaçlanmıştır.
Gereç ve Yöntem: Bu randomize prospektif kontrollü çalışmaya 49 erken dönem Parkinson
hastası ve 45 sağlıklı kişiden oluşan kontrol grubu dahil edildi. Parkinson hastalarının
Hoehn-Yahr Skalasına göre klinik evrelemesi yapıldı. Çalışma ve kontrol grubuna tam bir
oftalmolojik muayene sonrası OSDI anketi, GKZ, Schirmer testi ve KİS uygulandı.
Konjonktival örnekler sitolojik olarak Nelson evreleme sistemine göre incelendi. Elde edilen
sonuçlar gruplar arasında istatistiksel olarak değerlendirildi.
Bulgular: Parkinson hastalarının 27’si erkek (%55.1), 22’si kadın (%44.9) idi. Kontrol
grubunun ise 29’u erkek (%64.4), 16’sı kadın (%35.6) idi. Ortalama yaş Parkinson grubunda
66.1±7.2 yıl ve kontrol grubunda 65.3±7.8 yıl idi. Parkinson grubunda Schirmer testi ve
GKZ sonuçları kontrol grubundan istatistiksel olarak anlamlı düzeyde daha düşüktü
(p<0.001, p<0.001). Parkinson grubunun OSDI skoru sonuçları ve KİS evresi kontrol
grubundan istatistiksel olarak anlamlı düzeyde daha yüksekti (p<0.001, p<0.001). Parkinson
hastalarında evre 2 grubunun yaş ortalaması, evre 1 grubundan istatistiksel olarak anlamlı
derecede daha yüksek bulundu (p<0.001). Schirmer testi ortalamasının evre 2 grubunda, evre
1 grubundan istatistiksel olarak anlamlı derecede daha düşük olduğu bulundu (p=0.009).
Parkinson grubunda KİS evresi ile; Schirmer testi (r=-0.477, p<0.001) arasında negatif yönlü
orta derecede korelasyon saptandı. Kontrol grubunda Schirmer testi ile; KİS evresi (r=-
0.449, p=0.002) arasında negatif yönlü orta derecede korelasyon saptandı.
Sonuç: Parkinson hastalığında GKZ ve Schirmer test değerlerinin azalması, konjoktival
goblet hücre kaybı ve skuamöz metaplazi gelişimi oküler yüzeyin etkilendiğini
göstermektedir. Parkinson hastalarında kuru göz hastalığı tanısının erken konulması oküler
yüzey hasarı ve görme bozuklukları gibi komplikasyonları önlemek açısından önemlidir.
Aim: The aim of this study was to evaluate the ocular surface with conjunctival impression cytology (CIC), Schirmer test, tear break-up time (TBUT) and Ocular Surface Disease Index (OSDI) in patients with Parkinson’s Disease. Materials and Methods: This randomized prospective controlled study included 49 early Parkinson's disease and a control group of 45 healthy subjects. Parkinson's patients were clinically staged according to the Hoehn-Yahr Scale. After a complete ophthalmological examination, OSDI, TBUT, Schirmer test and CIC were applied to the study and control group. Conjunctival samples were examined cytologically according to the Nelson staging system. The results were compared statistically between groups. Results: Of the 49 Parkinson's patients in study 27 (% 55.1) were male, 22 (% 44.9) were female. In the control group, 29 were males (64.4%) and 16 were females (35.6%). The mean age was 66.1±7.2 years in the Parkinson's group and 65.3±7.8 years in the control group. The results of the Schirmer test and TBUT were statistically significantly lower in the Parkinson's group than in the control group (p<0.001, p<0.001). The results of the OSDI score and CIC stage of the Parkinson's group were statistically significantly higher than the control group (p<0.001, p<0.001). In Parkinson's patients, the average age of the stage 2 group was found to be statistically significantly higher than the stage 1 group (p<0.001). The Schirmer test mean was found to be statistically significantly lower in the stage 2 group than in the stage 1 group (p = 0.009). Parkinson's patients, there was a negative correlation between CIC score and Schirmer test (r=-0.477, p<0.001). Control group, there was a negative correlation between CIC score and Schirmer test (r=-0.449, p=0.002). Conclusion: Decreased TBUT and Schirmer test values, conjunctival goblet cell loss, and the development of squamous metaplasia in Parkinson's patients indicate that the ocular surface is affected. Early diagnosis of dry eye disease in Parkinson's patients is important to prevent complications such as ocular surface damage and visual disturbances.
