Özet
Primer progresif multipl skleroz (PPMS) klinik olarak herediter spastik parapleji (HSP), kalıtsal lökodistrofi ve mitokondriyal hastalıklar gibi nörodejeneratif bozukluklarla örtüşebilen, yavaş ilerleyen uzun trakt disfonksiyonunun klinik belirtileri ile kendini gösteren bir MS formudur. Bazı MS vakalarında immün sistem aracılı mekanizmalardan bağımsız olarak ilerleyici engelliliğe katkıda bulunabilecek patojenik genler tanımlanmıştır. Literatürde birkaç olgu sunumunda HSP' ye neden olan bazı gen mutasyonlarının PPMS hastalarında tespit edilmesi ve PPMS ile HSP' de uzun traktlarda benzer aksonal kayıp profilinin olması bu iki hastalık arasında patogenetik bir ilişki olduğu fikrinin yaygınlaşmasına sebep olmuştur. HSP genlerinin; PPMS' e olan duyarlılığını ve hastalık şiddeti üzerindeki rolünü belirlemek ve spastik parapleji kliniğine yol açan motor yollar üzerinde olası etkilerini açıklamak amacıyla bir PPMS kohortunda spastin (SPG4) ve paraplejin (SPG7) gen mutasyonlarının taramasını amaçladık.
Primary progressive multiple sclerosis (PPMS) is a form of MS that manifests clinically with clinical manifestations of slowly progressive long tract dysfunction that may overlap with neurodegenerative disorders such as hereditary spastic paraplegia (HSP), hereditary leukodystrophy, and mitochondrial diseases. Pathogenic genes have been identified that may contribute to progressive disability independent of immune-mediated mechanisms in some MS cases. The detection of some gene mutations causing HSP in PPMS patients in a few case reports in the literature and the similar axonal loss profile in long tracts in PPMS and HSP have led to the idea that there is a pathogenetic relationship between these two diseases. We aimed to screen for spastin (SPG4) and paraplegia (SPG7) gene mutations in a PPMS cohort to determine the role of HSP genes on susceptibility to PPMS, disease severity, and to explain the possibility of their effects on motor pathways leading to spastic paraparesis.
